Do Shoulder And Rib Pain Indicate A Gallbladder Issue While On A Ketogenic Diet?

Question: My alkaline phosphatatase is 144 and my ALT 94. I think I may have gallbladder issues as I have a pain under my ribs, shoulder pain and am very lethargic. I am 51yrs, 200lbs and am 5.4. I recently tried the ketogenic diet and this brought on the pain badly. My question is ... is this serious?

I carefully read your question and understand your concern.

Pain at the area under the rib can be caused by the gallbladder and also by other causes. Gallbladder might cause some problems with the alkaline phospatase and ALT if a stone is moving and is causing some blockage in the way out and this might cause the liver fluids to build up the pressure backwards with the above tests getting a little abnormal.

Still even though clinically it might seem like a gallbladder problem you will need to have an ultrasound to have the gallbladder visualized and the potential stone also. The ultrasound will evaluate the other structures also like the liver, the vessels within the liver, the pancreas and all the surrounding structures to rule out any issues related to them. If the ultrasound does not identify it than CT scan might be needed.

As for pain medications you can take ibuprofen to help with pain, it will not hurt you in case you use the proper dose. This is to bring some relieve but you need to have the ultrasound done and if the stone is identified you will have to have it removed.

So to conclude i would say:
1. It is good you are cutting on fatty foods, this will help you with the pain as it is generally worse after taking fatty food and will help you with health in general.
2. take some ibuprofen for pain, it is quite safe in therapeutic doses.
3. have an ultrasound done to visualize the structures there, CT scan might be needed if ultrasound does not give enough information.
4. a surgeon might need to be consulted if the stone is visualized.
