Is Creatinine Level Of 9.5 Mg/ Dl In Blood A Cause For Concern?

Question: Dear sir / madam, my brother in law is suffering from kidney disease, he had creatinine 9.5 mg/ dl on 4/1/16, now he has 8.5mg/ dl on 8/ 1/16, serum Pottassium is normal, blood urea 91mg / dl, fever 98. 5, no sugar, no swelling, normal urine, he is like a normal person.
Shall we go for dialysis

You have asked a rather difficult question. Here are my thoughts

1. it's an absolute indication to initate on dialysis once the creat reaches 10 mg/dl
2. the indication to start with creat less than 10 is when one has loss of appetite or neurological manifestation like depression, hallucination, etc ( unlikely in your brother in law with urea less than 100
3. volume over load ( breathlessness, edema, etc )

At this point if he was my patient I would have created an a v fistula and monitored him closely over next 4 - 6 weeks

Thanks for getting back. I am sorry for using technical terms. I will try and simplify it as much as possible

1. I don't think there is a need to initate on dialysis right away if the patient is eating well and has no breathlessness. Potassium has be monitored closely
2. keeping in mind that creat is very high he has to undergo a small surgery known as a v fistula surgery. This surgery enables us to connect the dialysis machine to the body for dialysis. Please get it done, everyone with such high creat needs this
