i need a pshyciatrist,,, i ve been married for 32 yrs ,,, my husband ever since i got married was always annoyed over small things ,, he used to be on canabis at that time ,, after taking marijuana cigerette he used to be a wonderful husband ,,, without taking that he used to be naggng angry and violent,,, after 25 yrs he bacame with or without nasty always ... he had a mild stroke last yera and left smoking drinking very concious about his walks etc but is always fighting with me,,, he insults me in front of family and friends if i defy his minor order too...if i retaliate he throws a tantrum shouts and abuses,,, my emotional state is rotting by the day pl advise
apart from me 70 percent people are on his hatred list whom he picks argument with..people who are in dominating position are always those whom he nvr picks fight and stays submissive

Your husband used to take cannabis cigarette for about 30 years. When not on cigarette he used to become violent and irritable on you. This is a normal behavior of any person who abuses drug. While not on drug the individual usually becomes restless, anxious and irritable because of craving for drug. Fortunately he was taking Cannabis which doesn’t cause much physical dependence and withdrawal symptoms are very low. With time any drug abuser becomes irritable because of underlying guilt associated with drug use. He may not be able to perform his expected tasks due to chronic abuse and out of this frustration individuals personality changes.
Cannabis also increases risk of stroke.

As you have mentioned that after stroke he has become more irritable, insults you in front of anyone, show tantrums (violent behavior). He considers most of the people on his hatred list (paranoid ideas) and picks up fight with anyone who defy his order (grandiose ideas). These symptoms may be due to two possibilities-

First thing that we can think of is Dementia. After stroke clinical features of dementia are observed in a number of individuals. Classical symptoms are memory loss, but in some people irritability, paranoid ideation, violent behavior, argumentativeness etc. are seen.

Second thing we can think is late onset bipolar disorder. As he has become irritable, have paranoid ideas (people in hatred list), fights with anyone who defy him (grandiose ideas), these features may be seen in late onset Mania in elderly.

Control of these symptoms is possible with use of medicines and warm supportive care.

As per my advice kindly consult a good psychiatrist for quick relief of his symptoms.

his neuro has given him anti depressent too,,, he is very mean towards me in my sickness and some times very nice ,, he had a session with phsyciatrist too when he had a stroke but refused to carry on with him later,, pl guide me wat to do,,, but how long can i gulp my self respect ,,, he is actually verrry argumentative now ,,and throws a huge tantrum ,,, he s more like a spoilt child now but performs well in meetings etc and very cleverly gets nice to people whom he has sm interest with,,,, i am feeling guilty of talking about him like this ,,, am i to keep patience ,,,ol elaborate ,,, i ll do anything to keep him healthy
can we have a meeting with u ??? i ll bring him along
he used to be just like this when off smoke ever since i remember and very very violent if i argue and very abusive if i defy minute order too and shouts if i confront about his insults ,,, i get night terrors ,,panic attacs now
but can it actually be dimentia when he has always been like this without cannabia ,,, hating me so much as if i look obnoxious that too when i am considered well maintained person and i manage my house well

Ma'am I can understand your feeling and stress you are having. Antidepressants will work but for control of his violent and aggressive behavior addition of some low dose antipsychotic or mood stabilizer drug is must. First step in our goal for his treatment is to bring his condition to a state so that he becomes receptive for Behavioral Management and counseling. A lot of drugs like Atypical Antipsychotics in low dose (Resperidone, Zipracedone etc) not only control the aggression but will also improve his cognition. Mood stabilizers are second choice, but for extreme aggression.
After decreasing his aggression he can be started on counseling and behavior therapy.
So kindly visit a good Psychiatrist and take opinion.

Dementia is a disease of elderly, the present symptoms like hating every body, argumentativeness etc. may be the part of dementia. While rest of things like irritation on you, insulting you etc. are related to his drug abuse behavior

One thing I want to tell you that due to this type of problem in family usually care givers also start to show symptoms of irritability, anxiety, restlessness, poor sleep etc. This is not only harmful to patient but also hampers in proper functioning of care giver. So as per my advise kindly indulge your self into some relaxation exercises or XXXXXXX breathing exercises or yoga. This will decrease stress in you.
