Can Metal Plates In Face Post Reconstructive Surgery Cause Asymmetry Of Lips?

Question: Hello,my name is XXXXXXX Long story short i had an accident 7 months ago,and had orbital floor and zygomatic fractures all over the left side of my face.

Did a reconstructive surgery 20 days after.

Now,almost 7 months after the surgery i have metal plates all over my left side of my face..

I want to ask.Because i am sure that my bones healed more upwards-outwards and more leftwards than they used to be before the surgery-accident,can the asymmetry on my lips and my smile can be caused because of the change of the bones??Or is it just muscle contracture and change that makes me look like that ??(see pics)
I mean when i smile the left side is getting pulled more upwards and when i don`t ,an extra tooth shows*except the two bif frontal)that didn`t use to show before

Welcome and thanks for posting your query to the forum.

I have gone through your complete query and analyzed all the pictures. I can make out your stress regarding the asymmetry of the face.

As it is 7 months since the accident and the surgery so is good time that your facial tissue recovered well form the incidence. I must say that as per your history of reconstruction orbital floor and zygomatic area and left side of face, your surgeon had operated very nicely. After analyzing the pictures I can see that the left side of face has bit flattening and more prominence of nasolabial fold with inclination towards left side.

As per the pictures the things are not very evident but as per my clinical experience I can make out that these must be more prominent on active dynamic expressions of smiling or while making gestures or eating or any related facial expression.

TO guide you regarding your present problem let you understand a simple fact of our facial symmetry according to aesthetic or cosmetic surgery- when we divide our face into two equal halves then we see mirror image of one half with the another. But while making any expression we all have asymmetry of face on minor to major side. On making gestures all of us have this asymmetry which we may or may not notice.

So first and foremost do not get tensed or self conscious regarding the asymmetry. You can check out in gathering or social function that how many of people are indicating you towards all the things that you notice. Secondly sit in front of your trusted friend and ask him if he/she notice any of these things.

Now for the present part of problems let me guide you certain important things:

- Do facial exercises such as blowing, whistling and neck stretching daily for multiple times. It is many times seen that the healing process goes on for multiple of moths to years together in case especially if the bones and muscles are involved.

Mild weakness of muscles can be treated by all the above mentioned facial exercises and therefore would help in improving the facial contour.

- For the part of asymmetry of left face and nasolabial folds- I prefer to do filler injections to fill the areas of asymmetry to match up with other side of face. Fillers are simple hyaluronic substance which fills the symmetrical areas.

- It would be better if you can increase your weight. Though in general I suggest my patients to reduce their weight !!!! but for you it would be better to increase your weight. A simple logic is that on increasing weight there would be increase in fat content of face. This increase in fat will be giving a better symmetrical contour to the face.

Overall I reassure you my sear young buddy that the things are not that bad. Give yourself positive suggestions and do not see yourself in bad image. We all have a tendency to notice the bad things first, so this habit should be avoided and see the positive side. I can see in the pictures that you are smart young man and this is phase of life which you can overcome easily by self efforts as well as various suggestion provided above.

Thanks a lot Dr Kanodia,i appreciate your answer..
I know noone has a perfect face,but TRUST ME even with the natural asymmetry that i got born,things were never as bad or even never show at all.I used to take compliments that i have a cute face and now i get nothing!
I know it seems funny to you,but for me,it`s a shock.I am very depressed and very ashame of the way i look.
I never had a worse smile than that.Things in my pictures look even better than they do in real life
Truth is that when i tell people that i had such a surgery,noone believes me in the begining,but people that used to know me before,they say that my face has slightly changed(but its not that bad).
It`s not that bad for me means UGLY and i hate myself(specially for getting into a fight,which is the main reason all these started up)
I saw an orthondotist today(because of my ugly smile and teeth and i got all the symptomps of TMJ,except the pain)and told me and showed me how we can fix my smile and at least have a perfection on my teeth straightness(which i think it would help overall)
But i am very concerned about three teeth of mine that still feel bit numb cause of the blows i received in that area,and maybe can cause problems through the orthondotist treatment process.
Goals for my at the moment are:
Lower eyelid tightness surgery(to camouphlage my exophthalmos)
Lip fillers to the doctors incision area(to camouphlage as i can the assymetry of my smile),but i dont know if it worth it because ive been told there are not permament lip fillers,and
maybe then go for braces to start the orthondotist treatment
What`s your opinion on that Doctor?

Welcome again to the forum and thanks for your swift follow up.

I can very well understand your concern for all the problems which you are facing. It is really distressing for every young men of your age to get unusual things.

I am also very much empathic to changed looks of yours.

For the part of fillers- I suggest you to go for it first. You can meet an expert dermatologist or aesthetic surgeon who can decide about the exact correction required. In the beginning it would be better to start with temporary fillers as the effects are reversible. If you like the things and effect then can replace it later by permanent one.

It is a good idea of your to get orthodontic treatment for the teeth and the smile. In a proper hand and expertize you will definitely regain your good smile as well as confidence.

In my clinical settings for your type of cases I always have a word with orthodontic surgeon and if possible make a common meeting where the part of treatment options can be decided. A combined approach of Orthodontic surgeon for teeth correction and dermatologist for fillers makes a good idea of possible way of correction on best side.

For the part of surgery for lower eyelid tightness- I generally prefer to go for exercises and physiotherapy methods before proceeding to surgery. It is better to talk to your treating surgeon for pre and post pictures as well as grade of improvement before proceeding for surgery.

It is a process of life to have change in our looks with the progress of life. At some part or the other we always get older and get our best hings slowly vanished- which is a rule of life. So do not get depressed and dejected as we always say "beauty is not outside but inside a person". So explore your inner-self and come out with best of your things. DO not feel dejected as face is not the only important part of our beauty.

I know all the above things might be sounding just like a "lecture" but I tell you my dear buddy that you are still quite smart and this is just your look out for yourself which his making major difference.
