Question: Hi MD, I am a 38 years old feamle. I was pregnant my LMP was on 11/28/2013. I had a D&C for a missed abortion on 01/31/2014. My HCG levels has been dropping well. My HCG levels were 4656 on 02/12/2014, 1457 on 02/18/2014, 331 on 02/25/2014. I had heavy bleeding with clots which I believe was my first menstrual cycle on 03/09/2014 which lasting until 03/16/2014. My next HCG levels were 51 on 03/17/2014, and INCREASED to 69 on 03/28/2014. I took a home Preg test that same day which was positive. I am not sure if the test was positive due to previous hormones still in e or is this a new pregnancy. I have been having unprotected intercourse with my boyfriend since a week after my D&C which was on 01/31/2014. my boyfriend made sure to pull out each time and not ejaculate inside me.Now we are unsure if I am pregnancy again now or if I am having complications from the previous miscarriage. I am scheduled for a repeat BETA on 04/03/2014. My next menstrual cycle is due to start 04/04/2014. Can you please give me your advise on what can be the cause for the HCG increase? Thank you.

Brief Answer: REPEAT BETA-hCG and UNDERGO ONE TVS SCAN. Detailed Answer: Hello, Thanks for writing to us. Unprotected sex with pulled out method has high failure rate (around 15-27%) and pregnancy can occur from pre-ejaculatory fluid. Here, I suggest to repeat blood beta-hCG as per schedule and undergo one Trans Vaginal Ultrasound scan (TVS) if you miss your upcoming period. It will confirm whether you are pregnant or not. Without these investigations, it is quite DIFFICULT to comment whether rising of beta-hCG from 51 to 69 mIU/ ml is due to your fresh pregnancy or previous miscarriage as last result is NOT much raised. Thank you for answering my question. I wanted to know if I could be pregnant due to my levels dropping then the increase from 51 to 69. Thank you I will see what the numbers state from my results tomorrow. If the numbers raise i take it I am with a new fresh preganacy. If they have decreased I guess my levels are dropping from the previous pregnancy.

Brief Answer: DETAILS ARE GIVEN BELOW. Detailed Answer: Hello, Thanks for follow up query. 1) Beta-hCG should double every 48-72 hrs in fresh pregnancy if you conceived truly. In that case, beta-hcg value would be 2-3 times higher than previous one or at least more than 100 mIU/ ml. 2) If you get increased value of blood beta-hCG , I suggest to undergo one Trans Vaginal Ultrasound Scan (TVS) additionally in absence of period. It will confirm your present pregnancy (100%). 3) If you get DROPPING result of blood beta-hCG, then NO ultrasound scan is required additionally. It will indicate previous miscarriage and you should repeat blood beta-hCG on next week until you get normal level. Hope, it helps for you to understand properly. Kindly, close the discussion.
