Can Herpes Causes Painful Sores At The Base Of The Glans?

Question: I have a sore at the base of the head of my penis. It's gotten progressively worse over time and is very painful. I have had herpes for 30 yrs but this is a constant sore and has only come about in the last several years. Anytime I get an erection it feels like it's rips or tears the skin and hurts like crazy. Help with what this is?

Pics have been attached. Also, I would assume this isn't tied to the herpes as with it if I have an outbreak it goes away after a few days. This is continually like this and painful. Also when I touch the end of my penis (hole) it is always very sore too.

Yes I am circumcised. I've gained weight over the years and seems like I'm not due to the skin going over the penis head now most of the time. I've had this for quite awhile now. Maybe a few years. At first is was just a little sore but has grown over time and now everytime I get an erection or masturbate it puffs up and then feels like my skin is split open. It's really hurts to pull back the skin over the penis head for several days after. Naturally when I go pee if any gets in the open sore it's very painful as well.

I have reviewed these images fully. While they are not suggestive of a sexually transmitted infection, it shows signs of a long standing wound. With continued erections, healing becomes a lot difficult and fragilised.

I recommend a consultation and aggressive wound management. You would certainly need to reduce your sexual activity especially that involving or resulting in erections.
