Question: Last Friday night I began to experience pain under my ribcage, upper abdomen, straight across, not specifically on the right or left side. I walk like I'm pregnant with my shoulders way back because I'm so uncomfortable. Loss of appetite, nauseous, burping. I feel like my stomach is distended, not sure that it is. New symptom last night, pain and tightening in left breast. I'm a woman, 56 years old, excellent health otherwise. 123lbs, works out and eats healthy.

Dyspepsia is a common disorder that affects up to 30% of the general population.
Symptoms of dyspepsia include upper abdominal pain or discomfort which may radiate to chest and frequently include symptoms of heartburn(retrosternal behind breast bone and ribs), pressure, or fullness often, but not necessarily, related to meals.Loss of appetite and nausea is because of production of excessive acid which constantly irritates the mucosal layer leading to nauseating feeling.

Other common symptoms include early feeling of fullness (satiety), nausea, belching, and bloating.
Treatment options for functional dyspepsia generally include one or a combination of:
Diet
Acid-lowering medications
Prokinetic(Domperidone) and antiemetic agents(Zofran).

Patients with nausea will respond to medications that target that specific symptom. Limited data suggests that symptoms of early feeling of fullness, upper abdominal fullness, and discomfort after eating may respond to the use of medicines that increase gastrointestinal motility (prokinetic agents). The drugs metoclopramide (Reglan) and domperidone (Motilium) are now the most commonly employed agents.


As of now start taking
Prisolec 20 mg twice daily with Antacid(phazyme) twice daily.
For nausea take Zofran 4mg twice daily
Take a probiotic with them.
For regaining appetite external enzymes like Aristozyme can be taken.
All are OTC drugs.

Take light food.
Avoid oily and spicy food.
Drink a cup of cold milk daily before going to bed.
Avoid fruit juices.
Avoid smoking and alcohol.
Please discuss the options with your gastroenterologist and start therapy you would start feeling better in 5 days other wise prokinetic drugs would be added.
Follow up after 5 days.
