Posted on Wed, 5 Nov 2014
Question: hello Doc,
My question is:
I am a XXXXXXX man and I had 2 nightstands, one was yesterday and one 3 days ago, just soft sex (fully)
But I am afraid that I've got a chlamydia since I got it twice a few years ago.
The problem that I have a partner and I don't want to transmit anything to him, though there are no symptoms so far, like discharge
Tthis morning, me and my boyfriend, we kissed and he sucked me for a few seconds. Is there any risk that I have transmitted something to him? this afternoon I took a shot of Zithromax. Please tell me if i must take another antibiotic for more protection.

N.B. I am HIV positive, but undetectable and he knows about it.

Brief Answer:
Zithromax is good drug, use of condom is also safe

Detailed Answer:
Hello XXXX, I understand your concern. I am Dr. Arun Tank & I am infectious disease specialist answering your concern.

- Genital chlamydia may remain undetectable in 50 % of men so there is chance you have silent infection with chlamydia, since you acquired it before also.

- Zithromax is the best treatment for the genital chlamydia only single dose of 1 gm is enough. Follow up should be given after 2 to 3 weeks for residual infection.

- Both partner should be treated and sex is avoided during the course of treatment.

- You can think of using condom also during the course of treatment, as it protects both the partner simultaneously. It also prevents transmission of other STD's like HIV, gonorrhea etc. As you have HIV using condom will prevent your partner from getting HIV also.

If you have anything else to ask I will be very happy to answer that. We wish you a best health at healthcare magic.
Thank you,
Dr. Arun Tank

Hello,
There are 2 points:
1- I always use condom with my boyfriend, so no worries about this issue. My lonely concern, is in case i caught it (since I have no symptoms) is there a possibility that i have transmitted it during this short period and the tiny soft action? in other way, is there a possibility to transmit it to my boyfriend within a few hours from exposure?

2- Concerning the following:
Both partner should be treated and sex is avoided during the course of treatment.

Since I have no symptoms, it is just about more precautions by taking the zithromax...Why should I treat my partner if there are no symptoms at both of us? should I wait to see if he will be facing some symptoms? because telling him the truth will definitely destroy our relationship. then sex will be avoided because i am travelling again for 2 weeks starting tomorrow.

pls advise.
thx

BTW, is this conversation gonna be posted online? in case yes, please remove it.
Thank you

Brief Answer:
Treatment for both partner is good and helpful.

Detailed Answer:
Hello XXXX,

- I am answering in the manner you asked query to me.

1. It is good that you are using condom. If you are asymptomatic carrier, even gentle sex or a few hour of sex can transmit the infection.

2. As I mentioned it earlier 50% of the patients are asymptomatic. You may transfer the infection asymptomatically to your partner. So as if you both take the treatment it will be helpful to both of you. There is no harm also in taking the treatment only mutual understanding is necessary.

I will be happy to answer your further queries, kindly come up with queries.
Wish you a best health at healthcare magic.
Thank you,
Dr Arun Tank.
