Can T-bact Or Neosporin Be Applied For Redness Between Buttocks In A Child?

Question: HELLO SIR,
My son is 15 months old..11 kg..recently due to constipation he passed hard stools and it led to a tear on the outside of his anus..we are applying xylocaine gel and t bact but it is not getting healed. Earlier we used xylocaine gel and neosporin to cure it and it always healed.Nowadays neosporin is not available in the market so dont know what to use to get it cured.Please suggest me some ointment so that the tear can heal.Also i would like to ask u whether use of tbact or neosporin in anal region is harmful for such small kids? A picture of the area is ttached along with this question marked with a circle around it...please help

Thanks for writing to us.
I have seen the attached picture.
Definitely there is a tear in the anus.
T bact is good and better than neosporin ointment.
You can use it but not more than 10 to 14 days.
Xylocaine can be applied to decrease pain.
But my suggestions
1. Treat constipation with lactulose syrup. I usually prescribe 3 ml twice daily for 2 weeks. Increase fruits and vegetables. Restrict milk to two glasses per day.
2. Consult pediatric surgeon. If ointments don't help he may need minor procedure for treating fissure.
3. Avoid prolonged use of tbact or any other ointment as they all have side effects if used for more than 2 weeks.
