Can Meftal P Be Given For High Fever?

Question: My daughter is 2 years n having fever 103.5. Gave her meftal p with sponge...hope its fine. Forehead is hot n palm and leg is cold

5 ml of Meftal p contains 100 mg of mefenemic acid( an antipyratic ).
The average weight of 2 yr old baby is around 12 kg.The dose of mefenemic acid is 5-8mg/kg/dose and it can be given three times a day.
So if we calculate the dose of 5 ml will be sufficient for her.
Paracetamol/acetaminophen is much safer than mefenemic acid for bringing down the temperature.
I advise mefenemic acid only when fever does not respond to paracetamol.
5-6 ml of paracetamol(120mg/5 ml) will be adequate and it can also be given three times a day.
You have mentioned that you are applying cotton on forehead only.The ideal way of sponging is to wipe the baby's whole body will wet cotton(wring out excess water) so that each time a layer of water film forms and when it evaporates the temperature comes down.Repeat it till the temperature comes down.For sponging use water at room temperature(not hot or cold).

Now coming to the cause of fever.
The most common cause of fever in this age is due to some viral infection.
Fever may be the only manifestation for initial one to two days and then other symptoms like cough/cold or loose stool follows.
Urinary tract infection is another common cause and is suggested by difficulty in micturition.
In viral infection fever usually shows a declining trend after 48-72 hours.
So better give paracetamol for fever.Monitor the temperature and if fever follows the same trend or increases in severity in next 72 hours consult your physician to look into the cause.
