Question: Hi Dr.
My 23 months old daughter was fine till Saturday morning, we had taken her to doctor for routine checkup on Friday eve. She was completely fine. But she started coughing and little restless Saturday eve. She got 101 fever early sunday morning and little cold. She passed stool on Sunday morning which was normal. She had very light food and later she again got fever around 10 PM night along with cough and cold. I felt her nose getting blocked while sleeping and her breath rate is b/w 32 to 42.
We visited doctor on monday eve. She suggested Nasal drop ( 1-1-1-1 ) and Citrizin ( 3.5 ml , 1-0-1 ) after spotting Upper Respiratory Infection in throat. My baby started crying, so she could not eximine her chest well, but there was no wheeze .We were giving XXXXXXX junior as cough Syrup.
We didn't see much improvement and she is having very restless night. She is not having constantly discharged from her nose. She is sneezing once in a while and sometime it will be followed by some nose discharge.
She is hardly eating anything and hardly taking milk too. She is having urge of water and she drinks little water at regular interval.
Her doctor suggested to change cough syrup to Asthalin ( Salbutamol sulphate syrup ) yesterday eve.
We have given 2 dose of Asthalin syrup yet, giving her steam and nasal drop.
But do not see any improvement and she is still getting fever every 8 hours and fever will be aroung 101 degree. I'm planning to visit pediatrician today eve as well.
Please suggest if we should start Amoxliyn ( Antibiotic ), 2 months back she was treated for pneumonia without wheeze .

baby age: almost 2 years
blood group: o+ve
cough: 4 days
cold: 4 days
fever: 3 days ( currently having 102 )
wt: 13.6Kg
City: XXXXXXX
Birth City: XXXXXXX
chill: no
Last time passed stool: S
She was identified as Vitamin-D deficiency last month, We given one injection and started oral for it.
she was active till yesterday but little dull since yesterday night.

Please suggest . Let me know if I should give COSCOPIN PAEDIATRIC syrup rather than XXXXXXX junior or Asthalin. My brither is GP and he suggested Coscopin, but I have not started yet.

Thanks a lot in advance, please let me know, if any further details needed.

Brief Answer:
I would like to get some investigations done

Detailed Answer:
Hello

Thanks for asking on this platform
I can understand your concern.
The most common cause of fever with cough and cold in this age is some viral infection.
In viral infection the fever usually starts declining after 3-4 days and general well being of the baby also starts improving.
In your case as fever is still there with no improvement in cough and general condition of the baby, the possibility of bacterial infection should be ruled out.
Coscopin is a cough suppressant and it is not recommended in this age.
I would suggest you to consult your pediatrician for clinical examination and discuss about the need of complete blood count,C reactive protein and chest xray to rule out any bacterial infection and also to assess for the need of antibiotic.
Continue giving asthalin and nasal drop till then.

I hope this helps.
Please let me know if you have any more query.
Regards
