Can A SIS Procedure Under General Anesthesia Help In Curing Infertility?

Question: I am aged 36 and my wife 31, married for the last 5 years. We are trying for a child and for this we have been vising a fertility clinic for the last 1 year as well as trying ourselves to conceive naturally. Relevant tests have been performed on both of us. My sperm count is 127 Mn/ejaculate and motility is 80% which I think is ok as per medical standards and the doctor has also confirmed this. My wife's USG has shown a Polycystic right ovary but the doctor has said it is nothing serious and she has her regular menstrual cycles as well in a gap of 28 days. She has been advised to undergo as SIS procedure under General Anesthesia. I want to know what is this procedure and whether any obstruction is detected, can it be rectified my medicines or do we need to undergo procedures as IUI or IVF.

SIS is saline infusion sonography which is an invasive procedure where saline is infused into the uterus and ultrasound is performed simultaneously to rule out any submucosal fibroid, polyps or even a septate or arcuate uterus. If found then they can be surgically removed.
As your counts and her cycles are regular , though she is polycystic, an ovulation induction and IUI cycle can be safely offered to you increasing your chances of conception further as there may be unknown cervical factors which can be the cause of unexplained infertility sometimes , which can be removed with an IUI procedure. Also as you have been trying for last one year already, you should get an IUI done.

Thanks for the answer. I want to know what is success rate of IUI procedure. I have heard that if not successful after more than 2 attempts, IVF is suggested. Is it natural to feel stressed or tensed after the relevant procedures as to whether it is successful or not.

Hello,
The success rate for all assisted reproductive techniques is usually 30-35% only. The protocol is 3 IUI cycles, then one diagnostic laparoscopy, then 3 IUI again and then if failed , IVF is the last option.

Dear Sir,
Thank once again for your prompt reply. The fertility clinic where we are consulting ask couples to engage in intercourse from day 10- day 18 of the women's cycles either daily or on alternate days. I want to know as to whether one should engage daily or on alternate days to increase the chances of conceiving naturally and whether engaging in sexual intercourse daily reduces the sperm count. Lastly, if we are suggested to opt for an IUI procedure after the SIS procedure or if we are in the middle of an IUI procedure, can one engage in sexual activity then or is it advisable to refrain from it.

Ideally day 10-18 is considered as the fertile period and hence the more frequent the intercourse the better are the chances actually. During IUI procedure you should avoid intercourse for 24 hrs at least and then you may start again.
