Does Arip For Anxiety Cause Weight Gain?

Question: Background: My daughter is 16 years 4 months old. She is studying in class 12th standard. She had severe cough problem since last 17 months with violent barking sound. I had tried allopath, homeopath and ayurveda with no relief. In my family, there is a close relative, who is jealous of us and goes to Tantrik. I tried through few Tantriks and exorcists for curing the cough problem. They had shown some surprising things and claimed that Black Magics and Spirits were removed and took many thousands of XXXXXXX Rupees. However these didn’t help the cough problem.
Since then she developed severe psychological problems (since last 7-8 months) and feels someone else is controlling her and even if she tries, the Black magic effect cannot make her prosper. Slowly, she developed difficulties in concentrating for Studies and fear in going to schools or coaching centers. She is now scoring low in examinations and gets anxious that she will come last in the examination. She used to Studies in the Hospital during last month of exam time, where she has been taking treatment from a Psychiatrist. Since last 2 months, she had been under constant treatment of an ENT specialist and for last 15/ 20 days cough is under control. However, too much anxiety is working in her mind & symptoms related to anxiety like palpitations, shortness of breath trouble her.
Present Status: She is under the treatment of a Psychiatrist and presently taking 1) Arip MT 15 mg, 2) Fulnil 20 mg and 3) Bexol 2 X 2mg. Arip MT was started since 3rd Mar’15. Dose was increased gradually. Earlier, she was under small doses of Flunil, Sizodon and Bexol. She now can study in home, however, if she goes to school / coaching center, she feels nervous and has to take Alprax 0.25 mg. She has already developed weight ( 80 kG). Her sleep pattern has become erratic now. She rises early in the morning while passing urine and can’t sleep thereafter. However she sleeps regularly after school / lunch. I see now her face as pale and seeing her eyes, it seems she is very tired and feeling sleepy. She is also undergoing Counselling.
Question: Psychiatrist has prescribed to increase the dose of Arip MT to 20 mg. I am not able to understand, whether it will be prudent to increase the dose so fast, particularly. It seems that during last 3 weeks of Arip MT, she has developed more weight. I am also not sure whether Flunil / Arip MT is helping , as she is feeling anxious to sit for a long time in the school / coaching center and wants to avoid. Your guidance will be appreciated to save my daughter

I read your query and understand your concerns about ill health of your young daughter.

As I can see, she is still symptomatic and all her symptoms are not under control, there is need to increase the dose of current medication. Although I agree with your statement that Arip may caused weight gain but in fact, Arip usually helps in weight and do not cause weight gain for majority of patients. Reverse may be true but not as a rule, as an exception.

Considering her symptoms and her tender age, it is essential to treat her aggressively and 20 mg of Arip is recommended considering her age. The normal dose of Arip is 10-30 mg and if she is tolerating it well and responnding, there is no matter of concern.

If I have been her psychiatrist, I should have considered increase in dose of Arip and decrease in dose of Bexol. I do not understand why bexol is prescribed as it may be the cause of weight gain, rather than Arip MT.

Thanks. I have got the confidence to continue with Arip MT 20 mg. She is now with this dose since around a week. Though she feels comfortable at home, while in school, she feels that somebody is coming from behind and is trying to catch hold of her and feels suffocating. How will the feeling go? Does Flunil 20 mg help or unnecessarily she is on another medication? She has lines of moustache visible on upper lips. Does such Hormonal problem create this type of psychological problem & obesity ?

Thanks for reverting back to me.

Although psychological problems can occur due to hormonal problems but reverse is more likely. Once somebody develops psychological problem their life becomes dysregulated and they tend to eat disproportionally. In addition to this medicine used to treat these disorders imbalance serum prolactin resulting in weight gain and development of masculine features.

The current medication do not have these hormonal imbalance side effect in majority of patients and it can be hoped that she will be better in few months.

I do not think flunil is unnecessary as she has severe anxiety problems and early morning awakening (indicative of depression). In both these conditions flunil helps for a large extent.

I like to suggest you to talk your psychiatrist for estimation of serum prolactin. If it is elevated (possibility is there due to use of sizodon in past) it can be reversed with simple medications.
I also like to suggest you to watch her exercise regimen. Encourage her to do physical exercise for a period of 45 minutes per day and 200 minutes total in a week. Also look after her eating pattern so she is able to lose unnecessary problem. These simple things are not easy tasks as appears and have huge benefit to patient in long run.

My daughter was on Arip MT 20 mg , Flunil 20 mg, Bexol 2 mg. She was on Arip MT 20 mg for around 3 weeks. Though she was comfortable at home, while going to school, she feels that somebody is coming from behind and is trying to catch hold of her. She used to take Alprax 0.25 mg during once / twice a week particularly on exam time at school. The earlier doctor has increased the dose to Arip MT 30 mg, Risdovia 0.5 mg (addition), Bexol 2x2 mg. She took the increased dose yesterday. I am seeing she is feeling more sleepy, face is more pale and tired. I am disturbed too much on the increased dose as well as on her condition. Please suggest.
I forgot to tell that Flunil 20 mg has been asked to continue. She has much dryness in mouth. I am not aware of other side effects as she doesn't disclose.

I need to repeat the same thing that I stated 10 days back. The need of bexol is controversial and it is not necessary (Probably) with current set of medications. The purpose of bexol is to avoid some side effects which do occur with antispychotiics like Arip or risdovia. But fortunately most of the patients who receive arip do not require it as the particula side effects are uncommon with Arip MT. The second medication risdovia may cause that side effects but at the dose of 0.5 mg the requirement is rare.

Considering these two facts I must inform you that bexol need to be either stopped or there is need of decreasing the dose. The side effects of bexol are dry mouth, dry eyes, decreased sweating, increased temperature, urine retention, excessive thirst and many more.

Considering all these facts there is possibility that her current side effects are due to bexol. If she have some side effects of Arip MT or risdovia (for which it is used) she may continue with 1 mg two times a day. In case no such side effects due to anti psychotics, bexol can be stopped all together.

I agree that flunil can cause dryness of mouth but it have at least 10 times less chance than bexol.

The nest point is that there is generally no rationale to prescribe two drugs from same class like arip mt and risdovia. Please talk to your doctor in that regard if there is any valid reason.

Thank you very much. I felt relieved. Should I continue with Arip MT 20 mg and Flunil 20 mg for more days to see the improvement or do I need to increase Arip MT to 30 mg right now. She is also on Zarp D before breakfast for a few months. How long does it take to see the improvement ? I would also like to change the Doctor. Can you suggest a good psychiatrist preferably in XXXXXXX or XXXXXXX ?

Thanks for reverting back to me.

Regarding Arip and Flunil both are in therapeutic range and may be used in dose range of 10-30 mg for Arip and 20-60 mg for Flunil. So I will not be in position to question 30 mg of Arip and 20 mg of Flunil.

In my view in first episode of psychosis, Arip 20 mg is enough dose in majority but the requirement sometime go upto 30 mg. I hope current psychiatrist examined the needs of the patient and he prescribed rightly 30 mg of Arip Mt. In case there is any side effect due to it you can revert back to lower dosage but if there is no such problem follow instruction of your doctor.

Regarding flunil 20 mg is minimum possible dose to start and treat.

The kind of these disorders take a long time and sometimes even months but most of them need at least 8-12 weeks to see the full effect. So if your doctor showing even some of the effects probably you are on right track.

Meanwhile get yourself educated with all information related to problem of your daughter so you can help her better.
