Suggest Treatment For Disc Protrusion

Question: Auto accident.

31-32 year old healthy male,

Head on, right leg hard on brakes.

L5/s1 disc protrusion, and SI concerns.

1.5 years later... still 6 to 8 pain levels, intermittently, right leg and lower right back.

Consistent symptoms:
-Calf, hamstring and quad feel like a "blob" hanging from my leg.
-Fail cold/hot and wetness tests.
-buttock muscles, calf, and hamstring all feel tight without any pain.
-pain in rotator muscles.
-pain in right testicle.
-unpredictable, inconsistent spasms in legs muscles to foot, like"fireworks", especially after strenuous activity.
-anal sphincter sore.
-muscles above coccyx and sacrum, right side only, feels sore, level 5 to 8, like "some one is pinching them.
-pain in right pelvic floor when erected penis, seeing of chain reaction to spamming leg muscles and punching sensation in right muscles above coccyx and sacrum.
-popping or cracking on right side "hip" only, when moving right leg, twisting, or other Normal movements (e.g. standing from sitting or laying, etc.).
-miss-firing leg and
buttock muscles, especially when walking or standing from sitting or laying.

What is not:
-i seem to control bowel and urinary functions.

What makes it worse (all can set of symptoms almost immediately):
-aggressive driving
-driving in stop and go traffic (car is automatic transmission)
-moving car seat closer to steering wheel
-ejaculating worsted all symptoms.
-walking
-pelvic thrusting, especially during sex
-manipulation of right side lower discs
-muscles massage in anyof theright sideaffected areas.
-stretching any ofthe rightside affected area.
-soccer, and any other activities, other than treating flat on back.
-sitting for more than 10 min
-standing for more than 10 min

What makes it better:
-sleeping on back
-resting, laying on back fully supported.
-electro therapy, 1 to three hour after, feels great for a day or less

What was supposed to help, but did not:
-pain killers (oxy, hydro, ib profen, aleve, aspirin)
-nerve "killer" (gabapentin)
-muscle relaxers (benzocycl...)
-physical therapy (four different therapists, different treatments, as explain within this entry)
-chiropractic care
-traction table (lower back pulled from upper body by machine at 80 lbs, re-presented lower back disc issues but did not correct or improve leg and pelvic issues, but made worse both).
-dry needle therapy, into buttock and rotator muscles (before my psychology stopped the session
-psych therapy (for possible depression, self diagnosed [i still think this can be fixed, and i was diagnosed add not depressed, this i must Not be psychosomatic])

Treatments and diagnostics:
(Doctors first suspected L5/S1 disc protrusion found after first few images, to be the cause)
-Xray of lower spine, came back "normal", day of accident
-ct of lower spine, came back "normal", until reviewed weeks later by a family practitioner with a neuro background, protrusion identified and diagnosed.
-physical therapy 1: focus on lower spine
-chiropractic care: focus on lower spine
-physical therapy 2: focus on lower spine
-dry needling therapy: focus trigger points in side of leg, made everything much less tolerable.
-steroid injection 1: L5/S1, focus on lower spine, no improvement
-steroid injection 2: L5/S1, focus on lower spine, no improvement
-steroid injection 3: SI Joint, minor improvement, for a couple of days until scheduled for next SI injection.
-steroid injection 4: SI J point, major improvement, ability to feel hour and cold and wetness for about 25 days after swelling dissipated, until i played soccer and rough housed with the kids.
-Referred to SI Fusion special surgeon
-surgeon refers to physical therapist specializing in pelvic floor injury for 6 to 8 weeks.
-physical therapy 3: focus on SI joint and pelvic floor and buttock muscles, no significant unproven after 8 weeks of different treatments, ultrasound, electro, manipulation, manual traction, internal and external massage, physical training, etc.

Now, have one more pt session next Tuesday, with no significant improvement. A CT guided, pudendal block, via steroid injection, on Thursday. AND another dry needling session in early September, focusing on leg muscles.

What i know:
-i want to feel better (back to normal) and NEED to be better SOON (mental stress, relationship, back to normal life, unable to perform job and housework functions).
-leaning over a counter to wash dishes or bed to fold clothes, sets off symptoms immediately.
-driving aggressively or in sup and go traffic causes symptoms gradually.
-this sucks

HELP, please. What could be causing these seemingly neuro symptoms?

Thank you for a detailed explanation of the symptoms. This is awesome description for a doctor's perspective. You had given everything what triggers the symptoms and what stops it. What benefits and what does not according to evidence based approach. But please understand that all the evidence based therapies are based on the average patient's response and cannot be said to a particular individual if they work on not. I know ultimately you look for ease of symptoms and quality of life.

I think you should try acupuncture/acupressure for the muscular symptoms. I am unable to draw any particular phenomenon for the neurological symptoms.

In my opinion if all the holistic and conservative care options are used up, surgical treatments and post surgery therapy remain as last resort.
However you should have chosen a specialist spine surgeon opinion on this than a primary care physician. They are the people who would deal such complicated cases with extensive experience and knowledge.
