Are Persistent Hiccups, Bloating And Diarrhea Signs Of GERD?

Question: Hi,
My brother is 30 years old and mentally challenged by birth. For past 4 days, he is having persistent hiccups with soapy loose motions. The condition started with vomiting and loose motions, after a doctor prescribed some tablets to reduce the stomach acidity, the vomiting and loose motions stopped, but he picked up hiccups. He hardly eats anything due to the hiccups (I guess) but he drinks lots of water because of the hiccups. So we mixed electrolytes in the water as per doctor advice and giving it to him. Since he is not a normal kid, and he doesn't like to see a doctor it is hard to get him diagnosed. His stomach also looks little bloated, so I'm assuming this could be a symptom of GERD/Reflux. Is my assumption correct? Can we give me any GERD tablet of light dosage and see if his hiccups stop? If so, can you please prescribe a tablet for that? He lives in XXXXXXX and we need medication which is available in XXXXXXX Many thanks in advance.

Forgot to mention that, he feels like throwing up and he keeps spitting saliva. The hiccups start slowly after noon time and gets vigorous by the end of the day. When he lies down starts he hiccups even in sleep, but when he turns on the side, then the hiccup stops.

Welcome To HCM. I am Dr Anshul Varshney. I understand your concern.
Your brother is suffering from acid reflux only following the episode of loose stools.
Persistent hiccups occur due to irritation of diaphragm by the gas.
I recommend following in such cases:
1. A combination of Pantoprazole and Domperidone.
2. Anti acidity syrup like Digene or Gelusil.
3. ENO sachets that are available at every general store helps a lot in relieving hiccups due to gas.
4. In resistant cases we use Baclofen.
However at present I don't think that Baclofen is required.
Start with these suggestions and make him drink plenty of fluids.
Write back to me with the progress.

Thanks Dr XXXXXXX for your response. Small clarification - If I've understood correctly, is the loose stool symptom of the acid reflux as well?
Also, can you tell me how much dosage and how frequently these tablets should be taken? Thanks again.

Detailed Answer:
Hello, he had loose motions following which he has Acid reflux now.
Pantoprazole and Domperidone capsule is usually given once a day and Syrup Digene can be taken two teaspoon full thrice a day.
ENO XXXXXXX can be taken 3-4 times a day in a glass of water.
These are the routine doses.
