Are RCIN Syrup And Solonex Given Empty Stomach For TB In Children?

Question: Hi,
My child with age 6 years is undergoing treatment for TB for last 3 weeks. He has swollen lymph node just above the neck.
The medication is from Infectious disease specialist Dr Suneetha Nareddy in Apollo Hospitals XXXXXXX Following are the medication.
1 - RCIN syrup 10 ML once daily empty stomach.
2 - Solonex 100MG once daily after food.
3 - Pyrazinamide 500MG once daily after food.
4 - Combutol 400MG once daily after food.
5 - Benadin 40MG (Monday and Thursday after food).

1 - We do not give any food after 5PM
2 - Giving RCIN at 7.30 PM(Time gap between food and RCIN 2.30 hours).
3 - Giving him food at 8.45 PM(Time gap b/w RCIN and food is 1.15 hours)
3 - At 9.30 PM(Time gap b/w food and the remaining medicines is 30 min) we give all the remaining medicines mentioned in the above list.
I have just checked on NET that all these medicines works effectively when given empty stomach.
I am very worried now if Dr Suneetha suggested the correct medicines and time of dosages.
Could you please let me know if the treatment is okay and the time of medicine is correct.
Also please let me know if I can change the dosage time for all the medicines to morning before breakfast or at 7.30PM(We give RCIN at 7.30PM daily as mentioned above). Please suggest if changing time for the dosages may change the TB to Drug resistant TB.

I have gone through your question and I understood your problem.

Over the past 2 decades, over a dozen studies and clinical trials of treatment of tuberculosis in children have been published.

These trials have shown that the basic regimen of 6 months of isoniazid and rifampin, supplemented during the first 2 months with pyrazinamide, cures over 99% of cases of drug-susceptible pulmonary tuberculosis in children.

Most experts in the United States initially start three drugs in children and add a fourth drug only if there is increased risk of drug resistance because of epidemiologic characteristics of the child or the adult source case, or if the child has a life-threatening form of tuberculosis such as meningitis or disseminated disease.

Rifampin, pyrazinamide and isoniazid should be taken on an empty stomach to ensure complete absorption of the medications.

Your doctor has suggested you the right medication.In some patients there are few side effects if they are taken empty stomach especially in children. But still to promote their absorption I will suggest you to give them on an empty stomach.

If taking the medications on an empty stomach in the morning is making you nauseous, you can try having a meal first, then waiting two hours before taking the medications to see if that helps.

Nausea is a common side effect of this mixture of medications. You may notice some improvement by altering the timing slightly, but the medications themselves may be causing your nausea.

I still have confusion as your first line of reply says that the timing of medicine should not be changed but in your detailed reply you suggested that Rifampin, pyrazinamide and isoniazid should be taken empty stomach.

The timings I am using to give medicines in the evening are as follows.
1 - We don't give any food item after 5PM in the evening.
2 - Giving RCIN at 7.30 PM(Time gap between food and RCIN is almost 2.30 hours).
3 - Giving him food at 8.45 PM(Time gap b/w RCIN and food is 1.15 hours)
3 - At 9.30 PM(Time gap b/w food and the remaining medicines is usually 20 min) we give all the remaining medicines mentioned in the above list.

My concern is that I am giving all the medicines 20 min after taking food at night except RCIN as you can see above the timings of food intake and medicines. I am worried if the medication is working at all as I am giving them after food in night except RCIN. Could you please let me know that what could be the problem if I change the timings of these medicines as same as I give RCIN at 7.30PM empty stomach in the evening?

I wanted to say don't change the timings but try taking medicines empty stomach as these medicines have more absorption over empty stomach. To be clear. Rcin contains rifampin and isoniazid. You are already taking both these drugs before food. As per the current schedule, pyrazinamide is being given after food. You may try taking this before food if you are able to tolerate it. Else I am happy with you taking it after food as per current schedule.
