Question: Dear sir
i'm 34 yrs old.My mri scan revealed periventricular/centrum semiovale chronic small vessel ischemic change 4 months ago. I was suffering positional vertigo due to this. My neurologist carried out lipid profile test , count blood cell test, vep, ecg, ecg, echo, holtel measurement test, antibody determination test, homocysteine measurement test to know the root cause of the problem . Later he confirmed no multiple sclerosis present Becoz all test parameters r found to be within safe limit. Then he recommended to continue homin tab, meaxon vaccine . He also injected me iv methyl prednisolone for 4 days. Then ivpred tab for 15 days. My query to know Is it a long run cure process. Still some % ge of dizziness is present. How much timeit will take to be fully ccured.

Brief Answer:
Symptoms might require 12weeks to 6months or more to resolve significantly

Detailed Answer:
Hi XXXX,
Thanks for writing in to us.

I have read through your query in detail.
Please find my observations below.

1. It is unusual to have periventricular and centrum semiovale chronic small vessel ischemic changes at 34 years of age. These are findings which are seen after 60 years age and in those with diabetes, hypertension and high cholesterol.

2. Multiple is one of the diagnosis which needs consideration and by doing test, your doctor has ruled out the possibility.

3. Methyl prednisolone is a steroid and acts on the brain by treating any cause that might have led to the small vessel ischemic changes. There is a possibility of narrowing of small vessels due to vasculitis which might have improved on taking the steroids. Vitamin B12 deficiency might also have been there which has subsequently improved.

4. The medicine called homin and meaxon is mecobalamin are vitamin B12 derivatives and have brain protective function.

5. Please take nutritious balanced foods rich in vitamins and essential minerals. Complete cure might take 12 weeks to 6 months or more. There is a rare possibility of symptoms worsening after few months to years. If that happens then you might require repeat MRI scan.

Please feel free to correct any oversight in my interpretation of your problems and discuss them in detail as per your requirements.

Hope your query is answered.

Thanx sir. Is there any possibility of nerves damage which causes positional vertigo. If any damage occurs can it be repaired by taking these medicines. Is it life threatening.

Brief Answer:
Please find details below

Detailed Answer:
Hi xxxxx,
Thanks for writing back with an update.

1. Small areas of nerve damage might cause vertigo. This is very small and unlikely to affect your health in a significant way.

2. The damage is less than 1 percent and taking vitamin B12 will try and grow the nerves or preserve the function.

3. At the moment these are not life threatening features. You can lead a normal life and do not worry much as long you follow up with your doctor and follow a good lifestyle.

Hope your query is answered.

Is there any specific exercise or yoga process for making blood flow smooth in that region. I'm making jogging for 30 minutes daily . Will it be sufficient or I have to participate in any outdoor games like badminton cricket etc.
