Doctor :
Are you there?
User :   dear doctor, I have a small girl child having 19 months old, all of a sudden she got a problem of ataxia and she is not able to stand and walk I consult with doctors and they are saying it is case of viral celebretis and they have gone for MRI to check any mass contains are there in brain and it is found nothing but alreadu 1 months 10 days gone but she is unable to stand and walk also I require your advise
Doctor :
let me read. please wait for a while
User :   in the mean time she was taking deflazacot 6mg for 1st 15 days 1.5, next 7 days 1 mg and now taking0.5 mg . I am really looking for any suggestion how I can better treat my child
Doctor :   ok

User :   In the mean time she is taking calcium and vitamin E also
Doctor :   ok
Doctor :
I will help you in this
User :   in the initial stage the head and hand are vibrating but now it has stable but she is not able to stand
Doctor :   ok
Doctor :
let me explain you the Cerebellum and its function
Doctor :
for better understanding of the recovery process
Doctor :
it is a part of the brain which controls the posture, coordination of movements of our body
Doctor :   when there is some infection of cerebellum

Doctor :   it will present in the form of ataxia
User :   yes sir, previously she has suffered from checken pox as doctors saying it may due to the post infection of checken pox
Doctor :   yes it is the usual cause
Doctor :
now the worry is how long she will take to recover
User :   yes that my worry
Doctor :   all neurogical diseases takes some time for recovery
Doctor :
usually it will be in some weeks
User :   but already 40days gone
User :   she is active in all respect but she is not able to stand
Doctor :   ataxia was the first sign to appear as you said earlier
User :   may I know what is the real cause for this and any actve medicine for that what I know deflazcot only an anty inflammation , dose it help to rebuild the cerebellum
Doctor :   viral infection of cerebellum is uaually a direct infection of virus or as a post viral infection
Doctor :   like chicken pox was the first infection
Doctor :
and followed by the chicken pox she developed this cerebellitis
Doctor :
it is the inflamation of the cerebellum which is the cause for the disease
Doctor :
and the drugs deflazacot are the drugs used to treat this inflamation
Doctor :   it will be reducing the inflamation and will help in building up the cerebellum back

Doctor :   are you there?
User :   yes sir but how many days she will take this

Doctor :   now the dose is been reduced

User :   will be she able 100% cure
Doctor :   yes usually people recover from the cerebellitis completely in 3 to 6 months
User :   what should be done whether dose to be reduced or not
Doctor :
it will be reduced and gradually withdrawn
User :   is there any other medicine to be advised
Doctor :
can i know what other drugs she is taking
User :   my querries whether the line of treatment is going proper or we need to change

User :   nothing only vitaminE and calcium that to from last 15 days

Doctor :   what is the diet she been advised?

User :   she is taking normal milk, cerelac, rice, vegetable mixture and corn flakes

Doctor :   ok

Doctor :   the deflazacot is used to reduce the inflamation
Doctor :
and calcium is given to increase the build up of the cerebellum function
Doctor :
and vitamin E is to prevent the damage and to build up the cerebellum
Doctor :   as the nervous tissue take lot of time to recover
Doctor :   usually it will be 3 months or more for the good result to be appreciated by the patient
User :   is there any better medicine to be advised
Doctor :
for the age given and for the situation and for the condition the present line of management is the best
Doctor :   she will recover completely in 3 to 6 months.
User :   is the physical therapy is required

User :
or any other testing required to check any thing more irregularities
Doctor :   Ok

User :   what I explain is it the symptoms of viral infection or any thing other which current doctor unable to diagnose
Doctor :   i understood your concern
Doctor :
The present condition perfectly fits into the cerebellar functional dereangment
Doctor :
and the cause is viral
Doctor :
it is been confirmed by the CSF analysis
User :   is this ataxia will degrade the normal growth of child

Doctor :
usually not
User :   what will help in CSF analysis

Doctor :
it will delay the achievement of mile stone, but the child will catch up the normal growth levels in 6 months time
Doctor :
CSF is the fluid in the brain
Doctor :
a test is been done where a fluid is removed from back and sent for analysis
Doctor :   was this test done in your child?
User :   No not yet

User :   it will delay the achievement of mile stone, means
User :   is it a pain ful testing

Doctor :   yes it is painful testing
Doctor :
so MRI as been done in your case
Doctor :
mile stone means,
Doctor :   child will start making sounds
Doctor :
than she will say ma ma
User :   is csf means proteins contains and glucose contains
Doctor :   may i have the details of all the test reports PLZ
User :   what will be the effect if it deviate the normal range

User :   how can I send this sir

User :   but she has not gone for CSF resting

Doctor :   i know there will be a long list of tests

Doctor :
you said about glucose and proteins
Doctor :   in CSF analysis we do that
Doctor :   you can mail the reports to YYYY@YYYY
User :   right sir

Doctor :   ok
User :   in CSF what can be analysed
Doctor :   it is a fluid in the brain and spinal cord
Doctor :   it will be usually obtained from the lower back by putting a needle into the spine
User :   is it possible acyclovir she can take if HSV infection is there

Doctor :   Acyclovir is given in active chicken pox infection
Doctor :   in your case it is post chicken pox as you told earlier
Doctor :
now to continue with the CSF analysis
Doctor :
apart from glucose and proteins as mentioned earlier
Doctor :
virus, bacteria and inflamatory cells are looked for in the sample
Doctor :   Are you there?
User :   yes

Doctor :   ok

User :   ok how can be checked the virus cells bacteria cell

Doctor :   from the inflamatory cell analysis

Doctor :
if it is neutrophils more in the CSF sample than it is bacterial
User :   is it thru the CSF analysis or any other test

Doctor :
IF it is neutrophils in CSF than it will be viral
Doctor :   than the bacteria is confirmed by the microscopy studies of the CSF sample
Doctor :
and viral agent is isolated by PCR tests performed on CSF sample
User :   if it is viral is ther any other medicine to be advised

Doctor :   Did the treating doctor advised ACYCLOVIR at any point of the time

User :   is it from PCR test we can able to know which virus affected is it vericell zoster virus for checken pox

User :   when she was suffered from checken pox she was taking acyclovir
Doctor :   ok

Doctor :   let me put it like this
User :   ok

Doctor :
if a person comes to a doctor with ataxia, clumsy speech and rotating eyes we call medically nystagmus
Doctor :
than a MRI of brain is done
User :   ok sir

Doctor :
than SPINAL TAP is done to diagnose the causitive agent for the cerebellar dysfunction
Doctor :   in your case the child had chicken pox which was diagnosed earlier
Doctor :   and the complications of the chicken pox are
Doctor :
pneumonia, encephalitis meningitis
Doctor :
and cerebellar ataxia
Doctor :
please fill the feed back form
User :   already filled

Doctor :
patient also may have arthritis
Doctor :   as the cerebellar ataxia is one of the complication of chicken pox
User :   ok

Doctor :   it will usually occur during the active chicken pox stage or some times latter after the disease is cured
Doctor :
and regarding ACYCLOVIR in cerebellar ataxia
Doctor :
most of the studies done usually have no favourable opinion on the usage of drug
Doctor :   regarding the prognosis of the cerebellar ataxia post chicken pox

Doctor :
usually with supportive treatment in the form of rest and good nutritious diet with no other medications also will recover completely in 7 to 9 months
Doctor :
with the treatment with antiinflamatory drugs the recovery period is shortened
Doctor :
and usually it takes 6 months
Doctor :
now as the patient is 19 months old, the neurological system was in active development phase
Doctor :   development i mean here is the child nervous system was learning how to sit, how to talk, how to walk and all

Doctor :   because of inflamation of the part of nervous system whose function is to assist in walk, talk, hand movements

Doctor :
these movements are little slow to develop
Doctor :   the cerebellum will recover from this infection and than only it will start learning those skills

Doctor :   so as told it will take some 6 months for the complete recovery of the cerebellum infection

User :   is it require physical therapy and good nutrition mean what can she take

Doctor :   physical theraphy is done to move the body
Doctor :
as long time not moving the limbs will cause stifness of joint
Doctor :   it is not absolutely necessary in child

Doctor :
it can be given
Doctor :
but before starting the therapy it should be consulted with the treating physician
Doctor :
now regarding the diet the protein rich diet is to be given
User :   I am talking about walking with support whether she can do

Doctor :
absolutely she can do
Doctor :
no restrictions
Doctor :
in fact if it is done it is very good
User :
any specific restriction of diet which can delay the growth
Doctor :
no restrictions in diet
Doctor :
i would advise to give more of protein diet for faster development
Doctor :
of the affected cerebellum
User :   protein rich mean egg and milk or any thing else
Doctor :   egg white the 100% complete protein
Doctor :
it is the standard and reference protein
Doctor :
apart from that the sprouted pulses
User :   Ok sir nice to talk to you , hope your advice and wishes will cure my child very soon

Doctor :   sure

Doctor :
your child will be back to normal in other 4 to 5 months, i hope let her recover much sooner
Doctor :
with the support she is getting from her parents she will defenitely improve faster
Doctor :
