User:My daughter, 8, had a urine test and it tested positive for high count white blood cells & blood in urine.. sent for a culture, nothing to abnormal bacteria wise... so referred for blood test, & everything within range except for AST 38. Referred to specialist. Frequency of urination, no meds, fasted night before, nighttime bedwetting and zero UTI. Some pains lower abdomen headache zero fever. Worried mom!
Doctor:I am Dr. XXXXXXX
Doctor:Please give me a minute to go through your query
User:Pale skin also and significant weight gain
Doctor:Blood in the urine is significant
Doctor:and white blood cells and red blood cells in urine without a positive culture for bacteria suggest a problem with the kidneys
User:She has had blood in urine every yr since Kinder but this time it is "gross" and akways high white cell count but has always tested for UTI so pedi never referred by he thought it was from UTI.
User:But this time high amounts and no UTI and no bacteria
User:Going to bathroom every hour and she has to be within 20 feet of restroom or accident
Doctor:In some kidney conditions, where the kidney does not filter properly, white cells and red cells can escape into the urine
User:And I feel like she is not emptying
Doctor:I strongly suspect a rather serious issue
User:Is 38 AS high high? Paper said High but some other sites say within range. 8 yr old female
Doctor:like a bladder tumor
Doctor:#8 is not high for an 8 year old
Doctor:I mean, it is not high for an 8 year old to have AST at 38
User:Okay. What should a pedi urologist do then? Appointment tomorrow. Urine always stinky. Drinks lots of water good diet never has had soda.
User:How do you rule out tumor?
Doctor:Pediatric urologist will have to see the insides of the bladder through a small scope
Doctor:this will rule out the tumor
User:Okay. So what is most significant? White cells and blood?
Doctor:What is significant is the absence of bacteria in the urine when there are plenty of white and red cells
Doctor:that is the one important finding that is pointing towards other cause such as kidney conditions or bladder tumor
User:Would she have any other symptoms for tumor? Pedi pressed all around and no pain.
User:But early morning she complained of a stabbing pain, and my mother caught her holding her abdomen but didn't say anything... but she likes to hide that she feels fine.
Doctor:Pain is not a common symptom in tumors till late in the disease
User:Would tumor cause nighttime bedwetting too or unrelated? Family hx.
Doctor:Tumor is not responsible for nighttime bed wetting, incidental or just the family history playing its part
User:Ok. Would ultrasound tell Dr anything?
User:I think nurse scheduled that first but I'd like to hurry with the process.
Doctor:Yes, ultrasound should show if there is any overtly large tumor in the bladder
Doctor:Ultrasound is the first step, because it is non invasive
User:Okay. Scope would be under anesthesia going up the female part?
User:Or make incision?
Doctor:Yes, since she is a child, it would be preferable to give her general anesthesia
Doctor:no incision
Doctor:The scope will be passed through the urethral opening
User:Okay. Do you think they will do another blood test again first?
Doctor:the scope will be a smooth tube, which will be small enough to pass through the urethra (The small opening through which one passes urine)
User:Can be benign or cancerous?
Doctor:They might do some blood tests which are a must before any invasive procedure, these may include a complete blood counts and such
Doctor:Can be benign or cancerous
Doctor:can not be sure at this point
User:Okay. Well thank you so much and prayers that this will all be fine. Love my baby girl!!!!!!
Doctor:I can understand your concern for you daughter
Doctor:I sincerely hope it all turns out fine
User:Thank you so much! Us too :(.
Doctor:Please go through what the doctors say
Doctor:I can't offer you my prayers as i don't believe in that, I am sure her doctors will be able to take care of the problem
Doctor:I wish you and your daughter all the very best
Doctor:is there anything else I can help you with?
Doctor:Your chat time is about to expire
Doctor:I hope I was able to answer all your queries
Doctor:thank you for using the service
Doctor:Bye, take care
