User:Hi Doctor, two weeks ago I had a hard time passing stool. I ate some pizza and usually I will take a lactose tablet because of the heavy cheese. I didn't and therefore the next day I had a hard time passing a stool to the point it felt like something was blocking it. I thought it caused me to have hemorrhoids but now it appears to hurt inside when I try to pass stool. It even hurts when I cough and I am feeling chilled.
Doctor:Hi
User:Hi Doctor,
Doctor:Okay
User:I have my son on the phone, he is in Nashville, TN. He is the one experiencing the problem.
Doctor:it appears like you had a tear inside
Doctor:Okay
User:Why does it hurt when I cough
User:or laugh
Doctor:Because laughing or coughing increases the pressure inside the tummy
Doctor:that causes the anus and rectum to move a bit
Doctor:that movement causes pain
User:What should he do?
User:He said it feels like something is blocking him inside now when he tries to go to the bathroom.
Doctor:He should get examined by a proctologist or a general surgeon
User:He is in Nashville, do you recommend someone there. He is near Vanderbuilt hospital.
Doctor:Yes, that can happen too because of the tear
Doctor:we call that a fissure in ano
Doctor:Probably your son has this fissure in ano
Doctor:this needs to be confirmed by examination
Doctor:Once confirmed there are different ways of dealing with the problem
User:Do you recommend he go to the emergency room versus trying to schedule an appointment. I am concerned as is mother because both of his grandmothers died of colon cancer and my father of pancreatic cancer.
Doctor:The most important thing to remember is to always take the lactase tablet
Doctor:He can go to the ER if the pain is severe
Doctor:if he can schedule an appointment within the next 2 days then he can wait
Doctor:but it is not advisable to wait any longer
Doctor:because without proper care the fissure will deepen and will cause many more problems
User:He has been in pain for two weeks and thought it was just hemorrhoids until now it hurts to cough or laugh and he is feeling chilled
Doctor:The feeling of chills is because if the infection
Doctor:Since it has already been 2 weeks, I think it is best to go to an ER
Doctor:because scheduling an appointment may take time
User:I am telling him to go right now. Thank you very much.
Doctor:Okay
Doctor:Is there anything else I can help you with?
Doctor:Ok
Doctor:Thanks for consulting me here
Doctor:have a good day ahead
Doctor:bye
User:You too, thank you so much.
Doctor:You are most welcome
Doctor:take care
Doctor:bye bye
