User:Hi, My cousin is having a problem of pain in back from about 1 month. She is newly married about 1 month back. On getting a ultrasound done the report said Right ovarian complex cyst of size 3.4 X2 cm in right ovary of hyper echoic contents. Is this problem get solved with oral medications or you suggest anything else to be done. I can attach the reports if you let me know how I can do that.
Doctor:The term right ovarian complex cyst is quite descriptive
Doctor:Now she has just started family life
Doctor:The treatment is surgical
User:why does this happen in first place?
Doctor:There is no specific cause
Doctor:It may have grown out of no reason
Doctor:The question should be that what is the best time to do surgery
User:tell me
Doctor:The approach nowadays is conservative that is to save the ovary and take out the cyst
Doctor:Since ovaries are needed to make ova and get pregnant
Doctor:If her symptoms are bad then she may opt for cystectomy now
User:ok can this happen due to intercourse?
Doctor:Otherwise she can always wait and get the surgery done after completing the family
User:is it possible for the cyst to be there from months or years?
User:what are the general symptoms of this?
User:can there be pain in stomach as well as in back?
Doctor:yes it is possible
Doctor:In fact the size is so small
Doctor:that she can forget about it and repeat the ultrasound again in 1 year
Doctor:The only issue is that is it is complex
Doctor:so some chance is there that it may be ominous
User:as per report the cyst is about 3.4 X2 cms while her ovary is about 3.4X2.3.Is it not big in medical terms almost close to ovary size?
Doctor:Did she get the CA125 levels done?
User:no what is CA125 levels ? and what is ominous?
Doctor:Rather than size I would be more worried about the symptoms she is having
Doctor:There are some chances that it may be cancerous
Doctor:though the chances are very less
Doctor:CA 123 is raised in some of the ovarian cancers
Doctor:CA 125*
Doctor:Cancers also have symptoms like weight loss, fatigue, etc
Doctor:I am sorry to be using these terms
User:can a cyst of about 3.4 X2 cm grow in 1 month time of it takes several months for it to develop?
Doctor:Being a doctor I would rule out ominous causes of complex cyst
Doctor:It depends on the type of cyst
Doctor:Simple cyst means that the cyst if thin walled and is filled with fluid
Doctor:If that grows more than 7 cm in sze it is taken out surgically
Doctor:Complex cyst usually requires immediate surgical evaluation
User:can this be resolved though oral medication?
Doctor:Surgery is the best option
Doctor:It is mostly done with a laparoscope
User:ok since she is having problem from only 1 month would suggest to first have oral medicine and after period of time go for surgery?
User:whats your suggestion if its detected recetly
Doctor:Which medicine are you thinking of?
Doctor:I would suggest that she goes for surgical evaluation
User:can i share with you the report with medicine prescription she is having?
User:how can i share with you?
Doctor:You can send these at YYYY@YYYY
User:shall i send it right away, you will get it in seconds
Doctor:or you can just write the name of the medicine here
User:sent you may get it any moment
User:what are the general symptoms of this?
User:so that i can ask her if she is having those issues as well
Doctor:Firstly a complex cyst means that the cyst has both solid elements and fluid in it
Doctor:If a cyst is present, mostly the symptoms are related to its size
Doctor:like pain in pelvis, back pain,
Doctor:In complex cysts some fluid may be detected in the culdesac on ultrasound
Doctor:Even this fluid will result in pain
Doctor:If it is cancerous than symptoms of cancer will be present
User:for a child birth birth both ovaries are required or one is also sufficient?
User:meanwhile if you have got my email it has blood report, prescription she is taking, ultrasound report
Doctor:I am still waiting for reports
User:you can have a check and let me know what you prescribe
Doctor:One is also sufficient
User:should health care magic guys should forward it to you or you have access to email id you sent me?
Doctor:Got the reports
Doctor:Give me a minute to go through them
User:ok sure go in detail and suggest me
Doctor:Went through the reports
Doctor:It seems like a hemorrhagic cyst and non cancerous
Doctor:In hemorrhagic cyst it can be monitored every 2 - 3 months with an ultrasound
Doctor:Now about the medicines she is on a pain-killer, antibiotics, antacid medicine and multivitamin
User:so you suggest another medicine or should we continue for same
Doctor:I would just suggest a pain-killer
User:if so for how long more
Doctor:that too occasionaly only when she has back pain
Doctor:I think she may continue to have some back pain till the cyst is taken out
User:will the cyst self heal if she continues with this medicine or how will it heal
Doctor:No it will not heal with this medicine
Doctor:Pain0killer is for pain
Doctor:I dont know why antibiotic was prescribed
Doctor:Pantoprazole is for acidity that may be induced by pain-killer
Doctor:and multivitamins can always be taken
User:then why wait for 2-3 month and monitor
User:if the issue does not get fixed through oral medicine
Doctor:It is the patients call
Doctor:It is not urgent to get it out
Doctor:It can be watched for upto 5 cm size
Doctor:Do you have any question here?
User:yes will it keep growing?
User:if she is having pain how will she be able to have baby
Doctor:It may grow
Doctor:Is the pain severe?
User:yes sometimes when it happens it happens with acute pain
Doctor:Then the treatment can be surgical
Doctor:She can consult a gynecologist and know the risks and benefits of the treatment
User:ok thankyou
Doctor:I have seen many patients living with the symptoms for a long time
Doctor:Thanks for consulting
