User:I have a problem from about 1 month and this is becoming now a routine.I get a acute pain in chest something like a heart attack and it stays from few minutes to a hour..once the gas moves out of mouth as barbs then I get some relief..This is followed by stomach pain and stays for hours till gas move out. This use to happen on non regular basis earlier but now a days everyday I am having this problem irrespective of what I am eating. And interestingly this happens only at night something from 7pm till midnight.. Why am I having this problem at such a early age..What I need to do both as a daily routine and for eradication of this problem all together.
Doctor:Dr. XXXXXXX here
Doctor:Let me read your post.
Doctor:May I know your age?
Doctor:Do you smoke and drink?
Doctor:Have you had similar problems in the past?
User:my age is 30
User:i dont smoke or drink
Doctor:Have you had similar problems in the past?
User:not like now..i use to have some gasteric problem ..once a while but now its become regular
Doctor:Yes, this seems to be a worsening of gastritis
Doctor:or an added acid reflux issue.
Doctor:It usually resolves with medications.
User:so what I need to do?
Doctor:I suggest my patients to use esmoprazole tablets such as nexpro 40mg once a day before breakfast
Doctor:along with Syp Mucaine gel 1 tsp thrice a day
User:for how long?
User:any side effects of it?
Doctor:This medicines may be taken for 2 weeks
Doctor:It is usually well tolerated medicine.
User:will i develop habit of it or will it fix the gastric problem?
Doctor:and most of my patients have not reported any side effects.
Doctor:Gastritis and acid reflux is a chronic condition
User:Also why is this happenning?I mean any changes in lifestyle or food habits?
Doctor:the symptoms would improve with medicines
Doctor:but condition can recur.
Doctor:Life style and diet change can control the condition on a long run.
Doctor:Life style and diet modification that are needed are:
Doctor:1. Physical exercise - early morning exercise
Doctor:2. Avoid spicy food
Doctor:3. Restrict beverages - coffee/ tea
Doctor:4. Eat food at regular interval
Doctor:5. Sleep well
Doctor:atleast 6-8 hours rest is necessary.
Doctor:6. Sleep with head end elevated under 2 pillows
Doctor:7. Your dinner should be atleast an hour before meal.
User:do get pain in middle of chest due to this consdition?
Doctor:These diet and life style changes can keep acid reflux and gastritis problems in control.
User:What you mean by "Your dinner should be atleast an hour before meal. "?
User:will drinking lot of water help ?
Doctor:Heart burn (pain in the middle of chest) is a common complaint of with acid reflux.
Doctor:* I meant dinner should be atleast an hour before you hit the bed.
Doctor:Drinking lot of water is definitely sufficient.
Doctor:* I see patients who drink less water ending up with acid reflux
Doctor:so taking atleast 6-8 glasses of water is recommended.
Doctor:Have you followed?
Doctor:Do you have any other questions?
User:yes doctor..I was thinking if just by talking lot of water solve this problem or taking tablets is also necessary
Doctor:Life style change, diet changes and lots of water over a long run will be able to control these symptoms.
User:I had one more query on a seperate topic
Doctor:But if you have this problem every day, I would add medicines to the above three measures
Doctor:And medical drugs prescribed in this case is for 2-3 weeks.
Doctor:If medicines for couple of weeks and the changes don't help you
Doctor:then you need to see a doctor and plan for endoscopy.
Doctor:Hope it is clear to you now.
Doctor:We can discuss about your second concern.
User:endoscopy for what? can you elobrate
Doctor:Upper GI endoscopy to be specific.
Doctor:It is a test done by gastroenterologist
Doctor:to rule out H pylori infections
Doctor:Do you have any more questions?
User:is that for ulcers?
User:H pylori infections ?
Doctor:The test is done to confirm acid reflux problems and to rule out other condition.
Doctor:Since ulcer treatment is almost the same as mentioned earlier
User:even the stool is not hard..its more like in case of lose motion
Doctor:i would like to rule out H pylori infections
Doctor:if nexpro is not able to take care of your symptoms.
User:can this happen at age of 30
Doctor:Gastritis and acid reflux has varied symptoms throughout the abdomen.
Doctor:One can feel improper bowel movements, fullness of stomach, indigestion and so on
Doctor:besides the heart burn.
Doctor:Some of them also develop breathing issues, throat irritation, chronic cough with acid reflux issues.
User:one can have lose motions as well?
Doctor:Loose stools may be related as well.
Doctor:as I said earlier bowel problems is an associated complaint.
Doctor:Do you have any more questions?
User:this can happen at 30 yrs of age
Doctor:Age is not a factor.
User:The other separate topic is For my mom ..she has here stomach lose
User:I mean
User:she is around 55 yrs old
Doctor:Based on your symptoms, I feel you suffer from acid reflux and gastritis.
Doctor:she has here stomach lose ?
User:comming to that
User:she has had 5 children and she feels stomach streched up and losen
User:so she ties a cloth around her belly to keep it tight
User:is there any solution for this
User:you can ask me any other questions
User:abt it
Doctor:Abdomen laxity may be due to multiple pregnancy
Doctor:it may also be part of increased weight and hormonal problems.
Doctor:If she has no other symptoms and her general weight is under normal limits
Doctor:then this is most likely due to abdominal laxity.
Doctor:In any other young patients, I would have recommended then to practice abdominal toning exercise
Doctor:through a trained physical trainer or at gym.
Doctor:I am not sure if your mother would be able to perform those exercise now.
Doctor:The other option to treat it through surgical means.
User:her weight is under control infact she is not fat
User:only her stomach is bulged
Doctor:The stomach bulge then is most likely the effects of multiple pregnancy.
Doctor:It rarely cause hernia problems; but in most women
Doctor:it doesn't cause any trouble.
Doctor:As long as she is not cosmetically bothered, you need not worry.
Doctor:If she is bothered cosmetically, she can try exercise.
User:she sometime complains of pain in lower abdominal
Doctor:surgery is the next resort.
User:what kind of excersise is recomended and what is done in siurgery?
Doctor:She can continue using abdominal support using abdominal belts or clothes to prevent hernia.
Doctor:Exercise should be performed by physical trainer.
Doctor:you can contact a physical trainer and learn about these exercise.
Doctor:As I mentioned earlier, I wouldn't recommend exercise for any patients at this age.
Doctor:but if she is able to tolerate physical exercise, she can try one or two abdominal toning exercise.
Doctor:Surgically removal of excess fat and toning can be performed.
Doctor:It is done by cosmetic surgeons.
Doctor:Have you followed?
Doctor:Do you have any more questions?
User:yes..thanks doctor
Doctor:Thanks for consulting me.
Doctor:Bye for now.
