User :   I am 24 yr old Computer Engineer. 2.5 month back on One morning, when I woke up I felt vertigo. Then I had

dizziness, tendency to fall on looking up, drinking water with head up from bottle, picking objects from height.

Later I found out that I have vertigo/dizziness in certain positions which were looking up, drinking water with head

up, going down to bed, rising from bed, turning head to right and left while lying in bed straight and when turning

over to left or right sleeping position. Vertigo were of small duration 5-10 sec. ENT doctor told me its Cervical

and sent me to physiotherapy. Later I had MRI done of cervical spine and it said a small anterior osteophyte,

c4-c5,c5-c6,c6-c7 disc bulge which on nerve root compression gives 0.9cm canal diameter. I went to Orthopedic doc

with MRI, he said there is no cervical spine issue and some degree of abnormality lies in all ppl. I went to

different Orthpodeic doc, he said there is small cervical issue but it shouldn't lead to vertigo / dizziness, asked

for oblique(L,R) view of cervical spine which came out to normal. Then I went for SWD daily, physiotherapy for 2

weeks. Now I don't had vertigo but whenever I look up, go down to bed, get up from bed, turn over in bed or look

sideways while lying on bed, I feel vertigo like sensation for a split second(0.1-0.2sec) and then normal. But

everytime, I repeat above mentioned position i feel the same. Now after 2 months of physiotherapy, I still have

above mentioned symptoms but sometimes nothing happens while looking straight up but sometime happens. But while

standing or sitting, if I turn my head right of left, i feel nothing and things are normal. Only when I look up or

in lying on bed positions(go down to bed, get up from bed, turn over in bed or look sideways while lying on bed), I

feel vertigo like sensation. It feels like vertigo will come
User :   and i can sense a very very tiny shift in view but then

it stops immediately. 2 days back, when I got up from bed from my right side(I always get up from my left side), I started feeling like falling over(which stopped when I stopped or sat and started again when I started walking) and it continued for 30 mins or so until I lied down on bed again and slept for 1 hour. Now yesterday, neurology doctor told me that its Positional Vertigo and refused to see MRI/reports that its not needed and told me that its not due to cervical. Can you please help me in this? I want to

know whether I have cervical or not. In case not, I can try PV exercises(major of which require neck extension which

my 1st ENT doc who diagnosed cervical without any MRI/X-ray asked me to avoid).
Doctor :   Hi
Doctor :
Let me read it
User :   ok
Doctor :   Ok

Doctor :
I went through your history
Doctor :
this is a benign positional vertigo
Doctor :
and absolutely no worries at all
User :   and not cervical spondylosis?
Doctor :
Cervical cause is unlikely
Doctor :
So you can stick to PV exercises
User :   bt MRI showed 9mm canal?
Doctor :
that is more than enough to correct it
Doctor :
and also slight sensation of vertigo
Doctor :
is normal
Doctor :
and is present in many people
Doctor :
so you should tune yourself
Doctor :
not to change positions very fast
Doctor :
be slow in doing it
Doctor :
like getting up from bed
Doctor :
turning your head
Doctor :
at those times
User :   hmm
User :   ok
Doctor :
give 2-3 seconds for your vestibular apparatus to settle down
Doctor :
and do practice the exercise
Doctor :
you shall benefit out of it
User :   but whoever sees my MRI report, even physician, asks me to do cervical exercises
Doctor :   Since it is interrelated

User :   actually one physician says its nothing and other one says its cervical
Doctor :
and also doing neck exercises also helps
Doctor :
they suggest that
Doctor :
You can do those exercises
User :   yeah, doing cervical exercises is good
Doctor :
but no need of any higher forms of treatment
User :   but problem is when i look up or do neck extension, i feel that vertigo and some one told me that its because of disc pressing on cervical cord
User :   and if you do that, you can damage ur cord
Doctor :   It is because of postural and circulatory changes

Doctor :
thats all
Doctor :
Dont worry much about it
User :   then i guess that 9mm cervical canal stenosis is also nothing to worry about?
User :   as i am doing exercises since 2 mnths, that would also have improved right?
Doctor :   Right

User :   so doing physiotherapy and exercises, actually pushes the disc back?
Doctor :   It does help you

Doctor :
in realignment
Doctor :
and keeping things in position
User :   but it doesn't improve the structure and disc won't heal?
User :   because i read sumwhr that herniated disc can heal
Doctor :
It can be realigned to some extent
Doctor :
there is nothing to heal there
User :   hmm.. so disc bulge won't get inside..
User :   thats fine
Doctor :   Correct

User :   one more thing, sometimes even while sitting or just relaxing or luckily just now, sometimes i feel heavy headed
Doctor :   Ok

User :   not like vertigo but jusy heavy headed or sumthng inside..
User :   that also because of bppv
User :   and nothing to worry about?
Doctor :
Do you get headaches?
User :   no..
Doctor :   Ok
User :   MRI of brain has been also done
User :   and revealed nothing
Doctor :
Now with circulatory changes
User :   i dont have any pain
User :   nor headache
Doctor :
there can be congestion of vessels outside the skull
Doctor :
I mean the superficial blood vessels
User :   ok
Doctor :
and that time you do feel head heaviness
Doctor :
it does have a link with vertigo
Doctor :
as they are compensatory to each other
User :   hmm
Doctor :
like blood supply decreased inside the brain
User :   yeah, sumtimes i feel like blood rushing inside my head
Doctor :
and increased outside resulting in heaviness
User :   mainly when lying down on bed..
User :   and many times i feel nothing when going down to bed but after i lie down on my back and after few secs-mins, i feel that vertigo sensation
Doctor :   Ok

User :   but i feel that once only
Doctor :
Ok
Doctor :
with sudden change in gravity
Doctor :
it is obvious for anyone
Doctor :
and since you are bit more sensitive
Doctor :
it shall be slightly more
User :   so that vertigo sensation can come even after mins of going down to bed right and may be not immediately?
Doctor :   Yes, with increased blood supply to your head

User :   cool
Doctor :
and also it can fluctuate with stress levels as well
User :   so i can safely assume that its nothing to with cervical
User :   *related to
Doctor :
True
Doctor :
your cervical stenosis is also to a very small extent
User :   ok
User :   and everytime i do that pv exercises of looking up activities, i feel that vertigo sensation. So, I should continue with exercises even with these problematic positions?
Doctor :   Yes

Doctor :
With those exercises
Doctor :
your threshold shall increase gradually
User :   hmm
User :   thats what was scaring me..
User :   whenever i do these exercises, my threshold actually decreases. I mean at the start, I feel nothing
Doctor :
Are you taking any medications?
User :   then I start feeling vertigo sensation when I look till straight up
User :   and later it comes, even when I look up to a small extent
Doctor :   ok

User :   after which i stop exercises due to fear of cervical issue
Doctor :   Ok

User :   bt after i sleep, things again come back to normal
Doctor :   Ok

User :   is that normal in this case?
Doctor :   Quite normal
Doctor :

Are you on any medications at present?
User :   yeah
User :   yeah, I am taking Vertin 16mg BD
Doctor :
ok
Doctor :
any other medications?
User :   yeah,
User :   with this vertigo issue, 1 out of 3 times, my BP came high
Doctor :   ok

User :   after various lot of tests, I gotta know that i have single kidney by birth
User :   but since last 3 weeks bp is in control
Doctor :   ok

User :   and medication for bp is also now reduced to Amtas 5mg OD
Doctor :
fine
User :   BP was 145/98 when it was found high
Doctor :
how about your sleeping habits?
User :   i sleep 6.5-8 hours in the night
User :   sumtimes 9 hours
Doctor :
Ok
Doctor :
Fine
Doctor :
those things are well maintained
User :   since last 1 month or so, go for morning walk, yoga in the morning and cervical exercises 3-4 times a day
Doctor :
and continue with the same habit
Doctor :
You are following a good lifestyle
User :   yeah, since last 2 month actually
User :   if ENT doc would hav told me its PV in the start, i wont have gotta know about all this stuff :D
Doctor :   Ok

Doctor :
but meanwhile you got some tests done
Doctor :
which is good atleast in terms of preventive health
User :   yeah, many blood tests, urine and ultrasounds mri
User :   infact, i had kidney infection as well as proteins came high in urine test
User :   but now all is fine
Doctor :   thats nice to hear

User :   all these discoveries because of vertigo issue :)
Doctor :
Correct
User :   thanks a lot for helping me out
Doctor :
One last thing
Doctor :
Have you got an ECG done?
User :   Cervical issue was really troubling me since last 2 mnths
User :   yeah, ECG, Echo
Doctor :
Fine
User :   2 times ECG was fine
Doctor :
That completes the spectrum
User :   one time it showed abnormality like left ventricular hypertrophy and right atrial abnormality
User :   but later i had echo done
Doctor :   that is because of BP as well

Doctor :
so nothing specific
User :   and doc told me that its nothing and most probably because of BP
User :   yeah, true
Doctor :   Is there any thing else I can help you with?

User :   yeah, one last thing.. should i continue Vertin as prescribed by doctor?
Doctor :   Since you are still having slight vertigo

Doctor :
you should continue
Doctor :
till when have you been prescribed that?
User :   i started vertin since one week back
User :   before that i took vertin for 1.5 week in the start ( sum 2 mnths back) but that din't had any effect on my vertigo
User :   i mean nothing improved because of medicine..
User :   and in last one month, nothing improved even with cervical exercises etc.
User :   but i guess now it may with pv exercises
User :   i have been asked to take vertin for one month
User :   and then visit physician again
Doctor :   Ok

Doctor :
You should continue as per that
User :   ok
User :   thanks a lot
User :   thats all i have to ask
Doctor :
Please fill the feedback form say done when done.
User :   thnx a lot
User :   and bbye
Doctor :   Thanks for consulting me. Bye for now. Please refer your friends to our service.

User :   yeah sure
User :   how to close the session?
User :   or just exit the window?
Doctor :   I shall close it
