User :   i think that i may be having a miscarriage but im am not sure. i started bleeding on monday but before that i had no cramps or anything like that. now i have been bleeding for 3 days kinda heavy. more heavy than my period with strong cramps and lower back pain. the blood is bright red and has clots. the cramps and back pain are worse at night

Doctor :   Hi
Doctor :
Let me read it
User :   hello
Doctor :   Ok

Doctor :
when was your last menstrual period?
User :   it was Dec 5 09
User :   my cycles are irregular as well.
Doctor :   Ok
Doctor :
Usually how are your cycles in duration?
User :   well i have them every other month. for example. in oct i can have it on the very last day and continue til the 3 of november but then come december i wont have it til the last day
User :   and they last about 3 -4 days
Doctor :   Ok

Doctor :
Is it monthly?
User :   not usually
User :   i had a baby 9 months ago and since then i tend to go every other month
Doctor :   Ok

Doctor :
So for now, did you have a pregnancy check?
Doctor :   as you are telling miscarriage

User :   i have not had a check yet since i can not afford it.
Doctor :   Ok

User :   i just want to be sure i am having a miscarriage and what i should expect for the next few days
Doctor :   Ok

Doctor :
Now what is the day today?
Doctor :
from the start of your bleeding
User :   this is the third day i have been bleeding. i started on monday the 8th in the morning
Doctor :   Ok

Doctor :
Usually how will it be in the day 3 for you?
Doctor :
will it be heavy or reduced?
User :   for my period?
Doctor :   Yes

User :   it is usually reducing and i never have had cramps either
Doctor :
Ok
Doctor :
Now you shall need a tablet for pain as well
Doctor :
if it is unbearable
Doctor :
take good amount of rest
User :   ok
User :   how long should this happen?
Doctor :   It should come down by tomorrow

User :   and how long will the bleeding last?
Doctor :   and if it still continues, then you will need a check up

Doctor :   to detect if there is any infection

User :   and should the cramps be low in my abdomen?
Doctor :
or a miscarriage, which you have suspected
Doctor :
Yes
Doctor :
it shall be in the lower abdomen
Doctor :
but can spread to upper areas
Doctor :
and you can feel it above sometims
User :   should they feel kinda like when having contractions?
Doctor :   Yes

Doctor :
it shall be the same
User :   ok.. and i feel as tho when i am laying still in bed like something is moving.. is this just the uterus contracting? when this happens i have to really pay attention and have my hand on my stomache as well
Doctor :   If you put your hand on stomach

Doctor :
it shall relax
Doctor :
and you would feel better
User :   if i put pressure below my belly buttom i feel a sharp pain.. but if i lay still with my hand just on top of my stomach it feels like bubble or something moving from side to side.
Doctor :   Yes, they are the uterine contractions

Doctor :
also there shall be muscle movements in response to pain
User :   ok.. i wasnt sure what it was..since i could not have been far enough to feel fetal movement
Doctor :   Yes

Doctor :
that is not the case
User :   thank you. and h ow can i know what type of miscarriage it is? or would i have to get checked to know that?
Doctor :   Miscarriage itself is not a possibility for now

Doctor :
You need not worry I guess
Doctor :   Is there any thing else I can help you with?

User :   what do you mean that miscarriage itself is not a possiblility for now?
User :   ?
Doctor :   Meaning it is not a diagnosis
Doctor :
as there are other causes for increased bleeding
User :   umm ok.. so i pretty much need to go get checked to be sure?
Doctor :   Thats right

User :   what kind of test will they do to find out
Doctor :   Transvaginal ultrasound
Doctor :   Abdominal ultrasound

Doctor :   after a vaginal examination
User :   i was told if i was bleeding an abdominal ultrasound would not be of any help
User :   will they just look for a heart beat?
Doctor :   No

Doctor :
A transvaginal one is more beneficial
Doctor :
but an abdominal is also done to know about the surrounding structures
Doctor :
like the ovaries, kidneys, gall bladder and other abdominal organs
User :   ok.. can they tell by having a blood test to check my hormone level?
Doctor :   No, it cannot be done like that

Doctor :
Right now, hemoglobin levels would be determined as you have more of bleeding
Doctor :
Hormonal levels shall be assessed henceforth if required
User :   ok. i just know i cannot afford to have those tests done, as i do not have insurance and can not qualify for other forms of medical help
Doctor :   Ok

Doctor :
There wont be necessity of all the tests
Doctor :
First a vaginal examination is all that is required
Doctor :
that too if the bleeding does not stop or decrease by tomorrow
User :   so i can just have a vaginal exam done?
Doctor :   Yes, that is the most important of it

User :   ok and they will be able to tell me just from that if i am having a miscarriage?
Doctor :   Yes, quite possible

Doctor :
as there shall be remnants which could be seen and/or felt
User :   oh ok. thank you
Doctor :   Is there any thing else I can help you with?

User :   i think that is all.
Doctor :   Fine
Doctor :   Thanks for consulting HealthcareMagic. Bye for now. Please refer your friends to our service.

User :   bye
