Doctor :
this is called as "Phimosis"
User :   is this the cause of his present ailment
Doctor :
this is the cause for frequent UTI and stomach pains with/without vomiting
Doctor :
as phimosis can cause recurrent UTI and symptoms thereof
Doctor :
of low/high grade fever, lower stomach pain/pain near XXXXXXXX, and sometimes vomiting
User :   how should be the line of treatment now
Doctor :
i will explain
User :   ok
User :   as and when he has or had fever he complaints of abd pain and some vomitting
Doctor :
ok
Doctor :
let me explain you the condition first
Doctor :
then you will be able to understant the
Doctor :
treatment options better
Doctor :
Phimosis is constriction of the opening of the foreskin due to which it cannot be drawn back over the tip of the penis.
User :   ok
Doctor :   it is a normal occurrence in newborn boy babies, but over time the skin that adheres to the tip of the penis can be retracted as the foreskin loosens after sometime
User :   ok
Doctor :
By age of 3 years, 90 percent of uncircumcised males are able to completely retract their foreskin from the tip of the penis.
User :   ok
Doctor :   and by the age of 17, 97 to 99 percent of males are able to completely retract their foreskin.
User :   ok
Doctor :   Phimosis can also occur if the foreskin is forced back before it is ready to be done so. This then can cause a fibrous scar to form in that place, which may prevent future retraction of the foreskin in these people.
User :   ok
Doctor :
the permanent cure lies in a small operation which is decided after seeing the child's general health and his needs
Doctor :
temporary and conservative treatment includes application of steroid creams
User :   ok
Doctor :   so what you need to do now is get a physical examination done by a Pediatric surgeon or a Urologist for your son
Doctor :
who will tell you what is best that can be done to solve the problem
Doctor :
have you followed what I have said?
User :   is this the right time to approach a urologist. he can retract the foreskin littlebit without any pain
Doctor :   your child is 11 years old, so it will be fine to take him for an examination and treatment
Doctor :
also he will ask you few more things regarding this problem like
Doctor :
is there any ballooning of skin while passing urine?
Doctor :
so even your son can answer those questions directly making it easy to decide the treatment options
User :   yes there is balloning
Doctor :
ok, so then the urologist will try to retract the sin and see what is the underlying problem
User :   ok
Doctor :
if he finds that it cannot be retarced at all, or if he finds that there is a scar tissue preventing the retaction
Doctor :
then he might decide to do a small surgery to relieve it
Doctor :
which will be a minor surgery
Doctor :
requiring only 1 day of hospital stay if child is fine
User :   ok i will approach the urologist and how much time it take
Doctor :
if the problem is small, and not due to scar tissue
Doctor :
then he will advise to use some steroid creams and medications to relive it
User :   ok the operation is under total anisthisia or local one
Doctor :
it depends, it can be done under local too, but if child is not able to co-opertae then it is better to do it under general
Doctor :
that will be decided based on the specific condition of your child
User :   ok
Doctor :
after the examination and evaluation
Doctor :
do want to discuss this with our panel specialist? a Urologist?
User :   before three years back our child specialist tried to open it by hand and
User :   a person having phimosis his urin test report may be normal or shows urin infection
Doctor :   this kind of problems are best handled by a Pediatric Surgeon or a Urologist
User :   ok
Doctor :
the urine test may come normal if antibiotic were already usd
User :   thank you dr. for guidance and help. i will report you after taking advice from urologist in our city
Doctor :
sure you can follow up with me after the consultation.
User :   thank you dr.
Doctor :   also you can take second opinion or a super specialist opinion from our panel specialists.
