User :   Hi Doctor,I am 8 weeks pregnant and have been diagnosed with UTI prior to my pregnancy. I have taken Cipro around 3 months before my pregnancy twice, and it has always returned. I then took Zinnat at the beginning of my pregnancy, but to no avail. I was prescribed Noroxin by my obgyn, but he initially said i needed nitrofurantoin. i have looked up noroxin and it is not rated category C and generally not recommended during pregnancy. I am considering visiting my urologist tomorrow. Any suggestions?

User :   yes he said the bacteria is resistant to most antibiotics
User :   and i am sensitive to most treatments... he initially said i would have to take nitrofurantoin
User :   then he prescribed noroxin
User :   i dont understand
User :   i did a kidney ultrasound and they were fine
Doctor :   Can you describe your symptoms?
User :   previously they were quite severe... used to recur at the time of my period
User :   i urinate frequently, but not a large amount of urine appears, burns, with little blood
User :   i now take triple strength cranberry tablets and drink cranberry juice
User :   i dont have those symptoms anymore

User :   ive been taking those tablets for a month
User :   those symptoms disappeared
User :   it hurts very little rarely when i urinate... but apparently the bacteria is still there

User :   i dont bleed anymore when i urinate... i pee frequently because i drink over 2 litres of liquids a day, mostly water
Doctor :   When was the urine test done?
User :   i got the results on saturday, so i did it on thursday

Doctor :   Are you still on the antibiotics?
User :   i am not taking any antibiotics
User :   i have not taken noroxin

User :   im afraid of taking it
Doctor :   Let me explain it to you.
User :   ok
Doctor :   The choice of the antibiotics during the pregnancy is based on the severity of the infection and the safety on pregnancy.
Doctor :   Since, I do not your urine reports now, I cannot comment if it was appropriate for starting the antibiotic.
User :   would you recommend taking noroxin?
User :   during the first trimester?
Doctor :   However, since you feel your symptoms are reduced antibiotics can be stopped and the use of general measures
Doctor :   such as drinking plenty of water, cranberry tablets can be followed till it your symptoms improve.
Doctor :   You would however need a repeat of the urine test to confirm this.
Doctor :   Have you followed?
User :   i will be seeing my urologist tomorrow
User :   i will hopefully be doing a repeat urine test...
User :   so its best i dont start noroxin right?
Doctor :   If your symptoms have improved, then its better that antibiotics are avoided.
Doctor :   But since I cannot examine you or have access to your report the final decision on this would be taken by the urologist.

User :   yes of course... thank you doctor. Just one final comment. is it okay to take nitrofurantoin in the first trimester? and does it work well for resistant e coli strains?
Doctor :   The safety of nitrofurantion in early pregnancy is not known.
Doctor :   However few doctors use them.
Doctor :   It is however not safe during last trimester of pregnancy.
Doctor :   And finally to your other question, nitrofurantion is effective in Ecoli infections.
Doctor :   In resistant strains, if there is no other choice of antibiotics available, then this can be used.
Doctor :   Have you followed?
User :   yes of course... thank you. any suggestions you have for me while pregnant? any tests you recommend i should do? I am 24 years old
User :   its my first pregnancy and i am being very cautious
Doctor :   If you have been completely healthy, then you just need general check ups.
Doctor :   Specific test that are needed are
Doctor :   hemoglobin, Hiv status and the thyroid profiles.
Doctor :   This are routinely done by the gynaecologist.
Doctor :   Do you have any other query?
User :   yes one more please doctor... i havent been eating much since i am always nauseous
User :   especially at night... whenever i have dinner i automatically fall sick
Doctor :   Nausea is a common complaint during pregnancy.
User :   but i try to have fruits and whole grains as much as possible
Doctor :   Especially during the first trimested.
Doctor :   You just need dietary modifications.
Doctor :   This would definitely improve once you cross first trimester.
User :   so i dont need to force myself to eat right?
Doctor :   Medications would be needed only if your symptoms are severe and your nutrition is severely affected.
Doctor :   yes do not force yourself.
Doctor :   Eat small quantity meals but in very frequent spells.
Doctor :   Avoid food that make you cause nausea.
Doctor :   Avoid fatty food and also spicy food.
User :   yeah our food here is so spicy and oily... rice and curries... i cant have any of it anymore
Doctor :   It should be alright once you cross your first trimester.
Doctor :   Oily food and spicy food can add on to your existing nausea, hence try avoiding it.
Doctor :   And make sure do not eat to the brink.
Doctor :   Have you followed?
User :   of course
