Doctor :   hi
Doctor :
how can i help you
User :   Hello Doctor
Doctor :
yes proceed
User :   Well I had a query on Swine Flu epidemic going on
Doctor :
yes
User :   The Tamiflu website by Roche claims that tamiflu can be used for preventive as well as for cure
User :   and infact my friend who is in London says that some UK doctors indeed precribe the medicine for prevention
User :   prescribe*
User :   However in India, this is being discouraged
Doctor :
yes
Doctor :
Tamiflu can be used to prevent when exposure is there
User :   ok
User :   so the current situation is an exposure?
Doctor :
and Tamiflu is one of the effective drug in the treatment of swine flu right now
Doctor :
yes but India the Doctors
Doctor :
discourage its use as preventive without symptoms
Doctor :
is due to the fear of resistance
Doctor :
that the virus can develop against it
Doctor :
virus or one of the creatures which can undergo mutations very fast
Doctor :
and in particular the flu virus
Doctor :
that is why humans get cold every season
User :   So if doctors in the west are giving preventive care and the virus develops a resistance there, then such resistant strains can get imported to India as well as the original strains
User :   so that can create a bigger problem, isn't it?
Doctor :   one sec
User :   ok
Doctor :
yes it can be imported
User :   ok
User :   I had a different question too
Doctor :
okay ask
User :   not about swine flu though :)
Doctor :
okay proceed
User :   A few months ago (maybe 7-8 months back), my son who is now 1.5 years suffered from UTI
User :   A urine culture, ultrasound scan and MCUG was performed
User :   All the results turned out to be normal
Doctor :
okay
User :   A six weeks Sporidex dose was prescribed while he was undergoing these scans and tests
User :   At the end of these tests since everything was normal, the doctor simply advised us to stop Sporidex
User :   My question is...do we still need to test him regularly? He never had fever during this time but he still had UTI
User :   I mean about 8 months ago, he had UTI but never showed any sysmtoms
Doctor :
not required until any synptoms
Doctor :
appear or fever
Doctor :
ior pain during urination
User :   but 8 months ago also he never had any symptoms...no fever, no pain...it was just a routine test because he was not gaining weight
User :   and we got to know he had UTI
User :   So how can we be so sure that he does not have UTI now as well?
Doctor :   sporidex was the only treatment right
User :   yes
Doctor :
it is an antibiotic effective against UTI
User :   ok
Doctor :
and it cures the disease in one course
User :   he was given six weeks course...about 8 bottles of 125mg
Doctor :
and if he gets it again then it is totally a new infection
Doctor :
can be a new type of organism other than the previous one
Doctor :
okay that is far more enough
Doctor :
UTI is one of the disease which can present in varied symptom
User :   so how do we know if he has or does not have UTI now....8 months back the urine culture said no organic activity after 48 hours of incubation
User :   but his pus cells were numerous/HPF
Doctor :
only one culture was done right
User :   yes
Doctor :
pus cells itself indicates there is infection
Doctor :
as urine is normally streile
Doctor :
that is no organism or pus cells seen
User :   correct but he never had any pain, no fever, no crying,...he was active, playful
User :   so he is active, playful now also...how can we be sure?
Doctor :
it can be an subclinical infectionwhich didnt manifest
User :   what does that mean?
Doctor :
it means that the disease is in a form not strong enough to produce symptoms
User :   so he had mild infection
User :   ?
Doctor :
these types are very difficult to diagnose
Doctor :
a sort of
User :   is that good or bad?
Doctor :
which will have slow course
Doctor :   if the immunity is good it will be taken care of by the body
Doctor :
if not it is bad
Doctor :
as it is not detected so easily
Doctor :
but as you said
Doctor :
numerous pus cells
Doctor :
he would have presented soon in a day or two with symptoms
User :   yes
User :   Doctor said he could have that infection for atleast 4-5 months before he was sent to the test...how is that possible?
Doctor :
because when the pus cells appear it is a transition phase from mild to severe
Doctor :
thats what is subclinical
User :   that means he can carry the infection for 5 months without showing any symptoms?
Doctor :
subclinical are always detected incidentally or accidentally when getting treated for others
User :   The only problem was he was not gaining weight
Doctor :
yes subclinicals can presist even longer
Doctor :
it is a presentation
Doctor :
even adults loose wait
Doctor :
when UTI is there
User :   so that brings back to my original question...how can I be sure he does not have it now?
Doctor :
okay
Doctor :
again an urine examination will help
User :   so should I test him regularly?
User :   he is now 10.5 Kgs ...1.5 years
Doctor :   when it is negative there is no need
Doctor :
and you can monitor is height and weight
User :   8 months ago he was just 8 kgs (he would have been 1 year then)
Doctor :
but always remember there are other causes also
User :   like?
User :   yes...he was wearing lot of diapers then...we have since stopped using diapers
Doctor :
protein energy malnutrition
Doctor :
and other practices
Doctor :
like wet clothes
Doctor :   less sleep
Doctor :
improper diet
User :   he has this practice of thumb sucking
User :   does that contribute?
Doctor :
yes you know children pick all the things they see
Doctor :
good or bad
Doctor :
so it does
User :   right
User :   he always has his thumb inside his mouth...we tried putting a adheshive tape but it does not help
Doctor :
it is care that is required to stop it
User :   is there a medicine or will it go away with age?
Doctor :   it will go away with age
Doctor :
you can train him
User :   ok
Doctor :
if he is busy in other activities normally he forgets it
User :   ok I noted that
User :   so a urine test is required now?
Doctor :
like playing or building which ever he is interested in
Doctor :
yes if it sterile that will solve your doubt
User :   ok
Doctor :
and it is best and even cheap also
Doctor :
one of the main indicator of UTI
User :   ok..whats the best time to train him for potty training?
Doctor :
1 1/2 year
Doctor :
if he listens
Doctor :
to the parents
User :   otherwise?
Doctor :
and understands
Doctor :
it can be delayed for 2- 2 1/2
User :   otherwise?
User :   ok
Doctor :
but normally 1 1/2
User :   ok
User :   I have another area of query
User :   can I?
Doctor :
yes
User :   I suffer from high BP
User :   I take Nebicard (5mg)and Losar (50mg)
User :   everyday
Doctor :
okay
Doctor :
is it under control
Doctor :
with these
User :   now the BP is stabilised to around 120/80 or sometimes 130/80 or 110/80 etc
Doctor :
thats good
User :   sometimes 100/70 also
Doctor :
okay
User :   but the doctor said its ok...nothing to worry
Doctor :
yes
Doctor :
its under control
User :   When I was given Aten 25, I didnt respond and my BP was around 140/90 or 150/100
Doctor :
okay
Doctor :
so it is good now it is under control
User :   From my childhood days I am an exercise freak...Iam a good runner, martial arts expert and do lots of exercise
Doctor :
okay
Doctor :
now
User :   even now too...I have never discontinued my exercises for the past 15 years
Doctor :
thats really good
User :   but how did I get BP when I was doing so much exercise for the past 15 years
Doctor :
it keeps the disease away and helps maintain the cholesterol level
User :   I dont smoke, no drinks
Doctor :   okay
Doctor :
your age
User :   iam 30
Doctor :
any one in the family have BP
User :   15 years I have been doing exercises, how did I ever get BP?
User :   no...no one
User :   Iam a bit overweight but that's ok..within the normal range
Doctor :   BP is one of the disease with many causes and involve all the organ
Doctor :
and it is one of the disease with > 90% primary cases
Doctor :
that is with without any cause
User :   This is a real shock for me as Iam a very healthy person with an excellent body flexibility
Doctor :
and the cause in such patient is not yet detected
Doctor :
but are well controlled
Doctor :
with medicine
User :   but with tablets?
User :   are tablets really required for a person like me?
Doctor :
it is seen that in such patients an hormone Angotensin and Renin are raised
Doctor :
in there blood
User :   Did the doctors overreact? When I was diagosed, I was suffering from severe respiratory infection
Doctor :
which are the mediators of high BP
Doctor :
what was that
User :   It was a lung infection for which a 2 weeks antibiotic course was given
Doctor :
as Respiratory disease also can cause HIGH BP
User :   So did the doctor overreact at that time?
Doctor :
Ans as it gone
Doctor :
any hospital admission
Doctor :   during that time
User :   No...none. Just a 2 weeks antibiotics thats all
User :   the doctor said the lung walls were very strong since I was doing so much exercise
Doctor :
most of the time blood pressure in India is diagnosed like this only
Doctor :
okay
User :   it was shocking for my family to learn that a person like me who was active and exercising atleast 3 days a week should get high BP
Doctor :
sometimes the pressure can be maintained normally by exercise and relaxation
User :   even more shocking was I didn't respond to Aten 25
Doctor :
it happens
Doctor :
both the things are there
User :   so any chance of reducing the dosage?
Doctor :
you are on Losar and NIcard right
User :   Nebicard...yes
Doctor :
okay
User :   Initially I had taken Nebicard H
User :   then the doctor reduced it to plain Nebicard
Doctor :   okay
reducing the dose depends on the control
User :   he said Aten 25 could cause potency problems
Doctor :
it happens that certain exercises performed without relaxation can cause this
User :   can cause HIGH BP?
Doctor :
aten as that side effect only in long run and not for all
User :   ok
Doctor :
yes there are exercise which can increase the pressue
User :   I dont do weights
Doctor :
and if the body doesnt make
User :   I do running, martial arts
Doctor :
okay
User :   martial arts has the technique of meditation and combat movements
Doctor :   okay then you also meditate
User :   I heard from fitness experts that doing weights can cause heart problems
User :   yes...thats part of martial arts...and I have been doing that for the past 10 years
Doctor :
not for all and not within limits
Doctor :
everything as its own limits
User :   so 15 years of running, 10 years of meditation and martial arts....still can cause BP? unbelievable
User :   Even my cardiologist didn't believe that :)
Doctor :
If your body is not getting adjusted
Doctor :
it can lead to it along with other factor
User :   I don't feel any tiredness also
User :   so my case is a bit special then?
Doctor :
like the environment in the house workplace , relation
Doctor :
food habbits
Doctor :
travels
Doctor :
excess work
Doctor :
improper diet shedule
User :   everything else is fine...I have non-veg once in a week, sometimes yes the work is too much, and relationship is good
User :   I have a controlled diet as well
Doctor :
and importantly your body hormones and enzye production and regulation
User :   ok
Doctor :   which when upset
Doctor :   this can hapen
User :   so given this...any chance of eliminating the medicine?
User :   in near future?
Doctor :   yes if controlled
Doctor :
and slow reduction of dose if effective
Doctor :
it is possible
User :   ok
User :   so I can look forward to it then :)
Doctor :
but keep cool and relax without worrying look forward without tension
User :   I have a mental block in taking this medicine everyday
Doctor :
you have plenty of time
User :   I don't like to take this...but no choice
Doctor :
that shouldnt be
Doctor :
Even in attitude you should not hesitate
User :   ok
Doctor :
that destroy the qualiites you have in your favour
User :   yes I understand
Doctor :
because BP is said HYPER TENSION
User :   yes
Doctor :
so tension free only can achive it
User :   I agree
