Doctor :   Hi,
User :   gi
User :   Sir i want to know abt COPD
User :   with acute excerbation
User :   and how much life span is available
Doctor :   May i know since when is the illness is present ?
User :   6 month back it is detected
Doctor :   ok, may i know the age and gender of the patient ?
User :   59 male
Doctor :   ok, there are types in COPD like Asthma ,Chronic Bhroncitis , Emphysema and Bronchectasis do you know the specific type the patient suffering from ?
Doctor :   COPD is Chronic Obstructive Pulmonary Disease
User :   acute excerbation
Doctor :   acute excerbation is the term used for increased in the severity over short time period
Doctor :   it can be managed by hospitalization
User :   how much time is available if smoking is continued
Doctor :   and the prognosis ( life span ) can be determined and depends on the other medical illness and the Oxygen saturation in the blood
User :   my father was detected 7 month back
Doctor :   is the smoking continued then the expected life span could reduce by 30 to 40 %
Doctor :   in case of COPD
User :   yes
Doctor :   the severity of the illness can be scaled using an chest xray , oxygen saturation , symptoms ,other illness present and the medications on
User :   he was taken nebulizer for last 6 month
User :   asthalin and impravent
Doctor :   nebulization is the mainstay of treatment in many, some may stay healthy on nebulization for years
User :   just having a problem while taking breath, if he walk 10 mtrs
User :   nebulization with smoking
User :   continued
Doctor :   nebulization effect will be completely nullified by smoking
User :   does 2d echo help us to find the condition
Doctor :   no , a Chest HR CT will be of great help here
Doctor :   in knowing the severity and the life expectancy
User :   is it possible that this problem gets severe in sleep
Doctor :   yes,
User :   my father on 1st march
User :   was complaining about gas problem in the morning
Doctor :   ok,
User :   so he took 1 omez and 4 tablet of some other tablet on gas trouble on his own + 2 tea spoon of dhouti yog churn + some liquid on lever
Doctor :   ok,
User :   this he started taking on 1 st march morning
User :   then that day he took nebulizer 3 times which for the last 6 month he never took at a time
User :   at 4.15 pm he took nebulizer last time
User :   read newspaper and went to sleep
User :   at 5.10 pm my mom tried to wake him up
User :   but he was not breathing some other person checked his pulse and said its moving too fast that too for 5-10 secs
Doctor :   ok,
User :   what might be the cause of death
Doctor :   I would suspect a cardiac death here as a first possibility and the COPD the next
User :   means
Doctor :   Heart induced death ( sudden cardiac arrest )
User :   but two days before he was ok
User :   then how come sudden death
Doctor :   its called sudden cardiac arrest , as there will be no warning signs
User :   does it shows if we have taken ecg or something two days before
Doctor :   the preexisting atherosclerosis would be the prime cause here , his age indicates the same
User :   is there any way if detected early to save him
Doctor :   yes, an ECG or an ECHO could have given some information about the risk he had
Doctor :   but an Angiography would be a gold standard
User :   but he didnt show any sign two days before such as chest pain , but was having breathlessness but he took nebulizer and was ok then
Doctor :   Ok , yes that could be the presentation in some
User :   and sir at 5.10 pm some one check his pulse and said it is beating too fast which last for very few seconds what this mean
Doctor :   it could be ventricular tachycardia ( its a sign of heart failing )
User :   how much time was avaialble to save him
Doctor :   once in tachycardia a defibrillator shock and a ventilator support could have revived him , but the chances of survival would be not more than 20 to 30 %
User :   if i could have been arrange ambulance un 15 min what was the chances
Doctor :   Thats very hard to say , according to the history it would be helpful as the shock would have given earlier
Doctor :   could have increased by 10 to 15%
User :   after recovering from this condtion could he have continued his normal life
Doctor :   Not much normal
Doctor :   as it could worse his other systems too
User :   what abt brain
Doctor :   the same , there would be a irreversible brain damage as the time prolonged
User :   plus COPD what his life had been then, how much normal
Doctor :   a much less than a half of normal life
User :   so what should i consider what happened was atleast releived him for pain or ...
User :   because i was at work and was unable to arrange ambulance although i contacted many friends ..but ambulance reached after 35 minutes
Doctor :   could be considered so
User :   i m feeling that i couldnt save my father and i m gettong in the same feeling of pain again and again
Doctor :   i would say no need to worry , it was 59 . this can be expected at this age
User :   his body mass index was high say more than 30
User :   chronic smoker + he used to take tablets on his own
Doctor :   ok that could be a contributing factor for his underlying condition
User :   also he was having problem with his brain , he used to take tablet , this tablets help him to keep his brain activitiy under control , if he miss that tablets he used to repeat same activity again n again , does this tablets had any effect
Doctor :   may i know the name of the tablet ?
User :   i dont know know but he becomes unstable if he miss those tablets
Doctor :   oh , ok
User :   he was taking that tablets
Doctor :   ok,
User :   he was also having high BP
Doctor :   BP can have bad effects on heart, that can lead to death
User :   sorry m troubling u a lot
Doctor :   no no , its my duty here
User :   one more ... 6 months back my mother was enjoying some songs in satsang
Doctor :   ok,
User :   she suddenly started sweating ..and feeled tired
User :   she immediately went to doctor and at that time doctor checked his BP and it was low
Doctor :   ok,
User :   what was the case of sudden lowering of BP and what precaution i need to take
Doctor :   is she a Hypertensive patient ?
User :   means sir
Doctor :   does she have BP issues ?
User :   no sir she is 65 and only one time she felt like this
Doctor :   then i would suggest her to take good amount of water
User :   sorry 62
Doctor :   also get her BP checked every week for atleast a month or two
User :   why that happened
Doctor :   could be due to water imbalance
Doctor :   was she on any medication that time
User :   no
Doctor :   ok, water imbalance could be a cause here
Doctor :   age too adds to such kind of incidences
User :   what precaution i should take
Doctor :   good water intake , and a BP check up every week for a month and twice every month thereafter
User :   my age is 31 and i do not smoke and drink
Doctor :   ok,
User :   but whenever i climb staircse and talking on phone sometimes i get moving towards breathless ness
Doctor :   may i know your height and weight ?
User :   feeling short of breath
User :   5 feet 7 inch weight 68 kg
Doctor :   ok, fine do you exercise regularly ?
User :   this happens if i m talking on phone while climbing
Doctor :   regular exercise will help here
Doctor :   and causes like stress and general fatigue could be considered
User :   i have triglyceride level as 261
Doctor :   its hight , you are at risk of getting overweight
Doctor :   you may have to burn more calories and reduce the calorie intake
User :   does it depends on when the test was taken
User :   by thyrocare
Doctor :   yes,
User :   before lunch after lunch like that
Doctor :   yes, typically its done before food
User :   so may be my level is ok but test was not
Doctor :   yes there is a chance , but exercising is always good
User :   my friend has triglycerides level as 561
User :   is this may be the case like he has taken the test after lunch because he is completely fine with his height and weigh
Doctor :   may be
User :   should he retest
Doctor :   yes,
User :   when to have body checkup
User :   my company generally does it every 2 years
Doctor :   some tests are to be done before food and some random
User :   what to be done to reduce triglycerides
Doctor :   fat free diet and regular exercise
User :   any medication
Doctor :   there are medications too
Doctor :   but not preferd at your age
User :   what exactly are triglycerides
Doctor :   these are fat particles in the blood
User :   how they are more dangerous than fat
Doctor :   they get converted into cholesterol and LDLs which can cause atherosclerosis in the blood vessels and can cause heart disease
User :   sir few questions more
Doctor :   yes, please
User :   when COPD is detected and smoking is continued what is the expected life span
Doctor :   reduced by 30%
User :   so if my dad detected at the age of 59 , he might be suffering from COPD long before ?
Doctor :   yes,
User :   high BP, COPD, smoking how much it has contributed
Doctor :   either of them can be a cause of death
Doctor :   there would be no specific scale , but could say pretty much
User :   how early it was
Doctor :   i am sorry i didnt understand ?
User :   means it was bound to happen
User :   or we should have averted the condition
User :   considering he was not ready to leave smoking
Doctor :   every cause there was avoidable , early and strict intervention would have helped
User :   sir how copd affects the heart
Doctor :   it will cause back pressure in to the right heart leading to right heart failure
User :   sir how to come out of the feeling that i feel i m responsible for not interveing or understanding it earlier, by now u might be feeling i m talking same copd again and again
User :   although other says that he might be suffering from inside and what had happened was destined as we all were out and mother went to sleep for that 30 minutes which usually she avaoids
User :   please suggest
Doctor :   i would consider the main cause of death in him was a sudden event which could have not been determined or intervened
Doctor :   and his age agrees that
Doctor :   you are being in obsession , you cannot be any reason here
Doctor :   i have provided you the facts over last 1 hour , just go through them again you will figure out on your own
