User :   Hi Sir,
User :   I have been suffering from sore throat since last sunday (16th Jan)

User :   I logged in here on monday and a doctor told me to gargle with dispirin and i will be ok in 48 hrs.

User :   there was absolutely no improvement and I logged in again last friday. and a doc told me to gargle Chlorhexidine
User :   even this isn't working. I am still having extreme pain and inflammation in throat, (from tonsils to lower end of throat) whenever I cough. this becomes even worse in evening and night.

User :   even if i gulp air, i feel extreme pain in the troat.

User :   yes... even last night, i was awake all night coughing.

User :   except for a bit of nasal congestion (which I have almost all thru the year) I have nothing else.

Doctor :   Do you have history of diabetes, recurrent throat infections?
User :   i am 37 and do not have any history of diabetes. throat infections & sore throat (like the current one) occur almost once or twice every year (usually with weather change)

Doctor :   Let me explain to you.
Doctor :   Sorethroat and difficulty in swallowing is a feature of upper respiratory tract inflammation - either pharyngitis or tonsillitis.
Doctor :   The most common cause is a viral or a bacterial infections.
Doctor :   Your symptoms are suggestive of a viral one.
User :   BTW, I guess I haven't told that my cough is dry one (non mucus producing) and this is what I commonly catch every year
Doctor :   Allergic inflammation is probably the other cause.
Doctor :   Now since your symptoms have been recurrent and has been there for more than a week, there are possible chances that you may need surgical correction - tonsil removal.
Doctor :   This can be confirmed only be a physical examination alone.
Doctor :   Hence my suggestion to you would be to continue gargling - chlorhexidine gargles.
Doctor :   2. Use soft diet containing less spice
Doctor :   3. Consult an ENT surgeon who can physically examine you
Doctor :   He will be the best person to predict if surgical treatment is necessary,
User :   one QQ. if tonsils are removed, I can get rid of both pharygitis and tonsilitis?
Doctor :   or an if alternative prescription medication can be used for the treatment of your current problem.
Doctor :   An upper respiratory tract inflammation involves both the structure.
Doctor :   Most often both pharyngitis and tonsilitis co exist together.
Doctor :   Tonsilitis cause more pain and difficulty in swallowing than pharyngitis.
Doctor :   Hence most doctors suggest removal of tonsils if symptoms are very recurrent, and if the symptoms are not controlled with medical drugs.
Doctor :   Pharyngitis usually involves a medical treatment, unless infection is severe and there are pus collections.
Doctor :   Your symptoms are not suggestive of pus collections in the pharynx,
Doctor :   however severe tonsiliar inflammation needs to be ruled out,
Doctor :   in which case the treatment can be surgical.
Doctor :   Hence I suggest you not to wait for more.
Doctor :   A physical consultation will provide better information if you need the surgical correction or
Doctor :   if the conservative treatment approach can be extended.

Doctor :   You can try dispirin warm water gargles along with oral painkillers such as Ibuprofen 400mg for pain relief.
Doctor :   Since antibiotics are prescription medication and should be used cautiously, I cannot suggest them online.
Doctor :   Antibiotics are for bacterial infection control.
User :   hmm... got it.
Doctor :   The need for antibiotics and the other conservative line of management is decided after a physical consult.
Doctor :   You can avail them(if necessary) after consultation with the ENT surgeon.

User :   Ibuprofen shall be used on SOS basis, I guess?
