User :   Hi,
Doctor :   hi
Doctor :   How can I help you?
Doctor :   Dr. XXXXXXXX here. Please proceed with your health query
User :   what is your specialty?
Doctor :   I am a General practitioner.
User :   i was looking for an oncologist
User :   anyway
User :   maybe you can help
User :   my grandma has cancer
Doctor :   yes, I can help you with my expertise to the best of my ability
User :   primary is in the lung
Doctor :   and suggest you an oncologist if needed.
User :   its metastisized through the lymph
User :   shes going to get chemo and radiation therapy at the same time
User :   they are doing radiation on the lymph nodes in the neck
Doctor :   ok. has the type of cancer been diagnosed?
User :   chemo drugs are Etoposide and Cisplatin
Doctor :   ok
User :   i understand that the radiation therapy will cause scarring and its a huge dose that is localized
User :   how do the two chemo drugs work?
User :   feel free to talk technical
Doctor :   ok. i will explain.
User :   im a physicist not a doctor
Doctor :   could you first tell me the type of lung cancer that was detected? i mean to ask the cell type of the cancer
User :   im not sure
User :   i only hear what my parents tell me
User :   im guessing small cell lung carcinoma
Doctor :   ok. do not assume things, as treatment for various cell types in the cancer differs
Doctor :   now coming to your query of the chemo therapy drugs
Doctor :   etoposide works by formation of metabolites in the lactone ring, that causses oxidative transformations in the dimethoxyphenol ring, which could damage the DNA of the cancer cells
Doctor :   leading to cytotoxicity and induction of DNA damage in the dividing cancer cells
Doctor :   such that it does not grow
Doctor :   coming to Cisplatin, it is a platinum compound containing drug
Doctor :   this drug can be used to treat a wide variety of cancers
Doctor :   it causes cross linking and mitosis in teh DNA, causing the cells to undergo Apoptosis
Doctor :   or silent death
Doctor :   such that cancer cells die and do not divide and grow
Doctor :   have you followed what I have said?
User :   barely but i can look it up
Doctor :   i am sorry, I did not understand what you said.
User :   i dont understand what you said but i appreciate the info
Doctor :   it is difficult to understand the technical terms, I said them as you said you could understand.
User :   i can look up the stuff online later
Doctor :   in simple terms, it affects teh DNA of cancer cells
Doctor :   both the drugs have affect on the DNA of the cancer cells which are difrenet from normal cell DNA
User :   so for side effects of these treatments
Doctor :   and cause damage to it such that DNA multiplication stops
User :   what is to be expected
Doctor :   and the cancer cells do not grow further
User :   and what drugs should she be using to combat them
Doctor :   the side effects are many, but the benefits outweight the side effects.
Doctor :   main side effects include toxic effects on liver kidney, ears and eyes.
Doctor :   the drugs used to treat cancer as I said earlier depends on the cell type
Doctor :   the combination of drugs can vary if the cancer is of small cell type or round cell type or large cell type.
User :   Etoposide and Cisplatin along with radiation therapy of the lymph
Doctor :   yes
User :   what are the side effects and what drugs should she be taking to combat them
Doctor :   main side effects of chemotherapy drugs include toxic effects on liver kidney, ears and eyes.
Doctor :   they usually set in late, the dose given will be less for her since they are using Radiation therapy too
User :   do you know if there is an oncologist online on this website?
Doctor :   to control the cancer
Doctor :   I can provide you the link to consult our panel oncologist, you can get answers from them either via e-mail or a phone answer
Doctor :   please give me a moment
Doctor :   visit this link to get the list of our panel oncologists
Doctor :   and their area of specialization on treating various cancers
User :   thanks
Doctor :   is there anything else I can help you with?
User :   can you think of some things i could tell her to make her chances of survival increase?
User :   she has hardly any appetite
Doctor :   she needs to have emotional and mental support, comfort her
User :   shes on 2 liters of oxygen almost all the time
Doctor :   focus on the quality of life that she lives than teh quantity, keep her happy, she will have loss of appetite due to cancer, but it will slowly improve as teh treatment takes its full effect
Doctor :   since when is she on treatment?
User :   she starts chemo tomorrow
User :   at 11am
Doctor :   have the doctors informed about the stage of teh lung cancer?
User :   im going to drive up to visit around dinner time
User :   i dunno
User :   that stage stuff makes me mad
Doctor :   if the stage is reaching advanced stage, then life span will not be much, but if in initial stages, then life span can be prolonged with adequate chemo and radio therapy
User :   it seems to be just a probability of survival
User :   its all statistics
Doctor :   so do you mean to say that she is having advanced stage cancer?
Doctor :   it would be good if you could get hold of few facts in detail before speaking to the oncologist
User :   from what i know its around 3
User :   ok
Doctor :   like: type of cancer, stage of cancer, duration of therapy planned.
User :   well last time i went to visit we looked at her medical records
User :   and found out that her calcium level was high
User :   for a while
Doctor :   yes, in cancer calcium levels do go high
User :   her gp didnt think a thing of it
User :   just took her off of her calcium meds
User :   kinda pissed me off
Doctor :   get hold of few facts like: type of cancer, stage of cancer, duration of therapy planned in your next visit
Doctor :   such that you can have a fruitful discussion with the oncologist
Doctor :   about her current status and what further can be planned or discussed with her doctors there
Doctor :   to help her fight this out
User :   ok
Doctor :   have you followed what I have said?
Doctor :   is there anything else you would like to ask me?
Doctor :   the biopsy report and the IHC (immunohistochemistry) report would be of great help
Doctor :   look for them too
