User :   My wife is 31 YO Female. She is taking Progesterone supplement cream and has a history of sensitivity to bad food (specifically oils). She passed out after getting up quickly today and had a metallic taste in her mouth. There is no sign of shock or heart palpitation - (strong steady pulse). There was some headache (possibly due to fall). She felt warm before passing out and then was cold a few moments later. She had complained of nausea so I gave her activated charcoal in case there was some food poisoning. She is feeling better now but still has some weakness. What would you recommend and what are the likely causes?
Doctor :   Hi,
User :   Hello
Doctor :   Let me read your query for sometime to be able to have a good discussion here.
User :   We have likely ruled out pregnancy.
User :   She is from Ukraine and may had had some radiation poisoning as a child from Chernobyl. No other signs of this.
Doctor :   Do you mean she fainted ( passed out ) ?
User :   Still she complaining of weakness and tired.
User :   Yess, she passed out and fell on the floor
Doctor :   Have you checked her Blood glucose levels ?
User :   Not today, but she has had a through review in the last month with no significant reports of anything. Now she is reporting some pinching in the right ear - but only slight pain.
User :   Stiil some nausea and slight headache (could be from fall)
Doctor :   Had she ever had any ear infections in the past ?
User :   I think yes, but this appears to be more related to gastro-intestinal.
Doctor :   What other gastro symptoms does she have ?
User :   You have about everything we can think of listed above. There has been some sensitivity o foods and food poisoning in the past.
Doctor :   but i none amongst you mentioned do cause a syncope
Doctor :   unless there is hypoglycemia
User :   I am attempting to use an abundance of caution to rule out any other critical items such as gallbladder disease.
Doctor :   No, gall bladder diseas4 do not cause syncope unless its sever block to its duct causing severe jaundice
Doctor :   Syncope is a very rare symptom association with gastro intestinal problems
User :   Sorry, I do not understand this statement "ut i none amongst you mentioned do cause a syncope " what should I do and what are the likely causes and should I be overly concerned right now?
Doctor :   i mean the sensitivity to oils and the food poisoning cannot cause directly
Doctor :   also may i know the Progestrone analouge name she used ?
User :   It is Progesterone 60mg/ml 5ml each applied topically in am and night by baxa made in Denmark
Doctor :   is this the first time she used it ?
User :   She has used this for several weeks
Doctor :   ok the metallic taste can be a rare side effect of the synthetic progesterones
User :   The metallic taste was only brief as she woke up from passing out. Also she reports a hearing some sound like a grinding in background. She said she has felt this a few years ago.
User :   She was craving and eating apples and drinking water in the last hour.
Doctor :   was there a loss of consciousness , when she had fainting ?
Doctor :   that craving can happen in Hypoglycemia, and the weakness too is typical
User :   Yes, she evidently went out and woke up and wondered how she ended up on the floor.
User :   Is there any immediate concern or recommended protocol ?
Doctor :   filling her up with glucose is the main treatment line
User :   Ok I will do that - any thing else to verify or check in thje future?
Doctor :   if she mentions the wearing of the weakness after giving glucose then the diagnosis is confirmed
Doctor :   i would ask you to get her FBS ( fsting blood sugar ) and PPBS ( postprandial blood sugar done ) and get physically evaluated by a GP
User :   Ok thank you for you help.
User :   Bye for now.
Doctor :   you are welcome
Doctor :   Bye
Doctor :   take care
