User :   hi
Doctor...I spoke to Dr. XXXXXXXX and then Dr. XXXXXXXX earlier and as suggested I got a through check up done for my 26 months old daughter
Doctor :   Ok
User :   she is suffering from persistent cold and cough with no fever from past 6 weeks
User :   in between for a week she becomes ok and then again she gets cold
User :   and more its only during nights
User :   x-rays were done blood test were done

User :   and with different doctors opening nothing concrete was diagnosed
User :   she doesnt have chest congestion
User :   only caz of cold she has enlarged tonsils and adenoids
Doctor :   Ok
User :   so duanase nasal spray and asthaline nebulization were given
User :   it helped initially but then she was ok only for 1.5 weeks and again she got it
User :   so today we consulted the pulmonologist again
User :   he has now channged the medicines so I want to know if this is ok or not

User :   budamate inhaler with spacer, as they say she has mild infection so antibiotics called as azithral,t mantec lc and rhinocot aqua nasal spry
User :   and asked to stop duonase spray and asthaline nebu

User :   i got only azithral
User :   i couldnt get any other medicine
User :   so can i give her budecort nebulization just for tonight as she is coughing a lot while sleeping
User :   doctor says we cannot say now as its too early to tell if she is allergic or not
User :   her AEC is little high

User :   is budamax and budecort the same ? if not then which is less strong
Doctor :   The are the same and both containing a drug known s budecortisone, which is an inhalational steroid.
User :   so tonight can i give her budecort till i get budamate
Doctor :   Yes, the only difference being the nebulisation and the spacer form.
User :   so which is better? or the same?
Doctor :   Spacer is a mobile product and is much easy to handle, can be carried along easily while compared to the nebulisers.
Doctor :   I would suggest you to try obtain the spacer for the purpose for comfortabilty alone.
User :   so can i give her budecort instead of budamate tonight as i have the nebulizer
Doctor :   Yes, you may use the nebuliser till you obtain the spacer.
User :   ok
User :   second question is are all the above medicines ok for such a small kid? what are the side effects?
Doctor :   As mentioned this is a steroid product, but since the mode of administeration is inhalation alone and not oral tablets, they do not cause systemic side effects compared to the oral tablets.
Doctor :   Hence they do not major side effects.
User :   doctor says she will get better as she grows...right now we can only prevent her from getting these symptoms by controlling the allergy causing things like dust etc

Doctor :   Yes, children are more commonly predisposed to these symptoms compared to the adults.
Doctor :   Her symptoms are expected to improve as age grows.
Doctor :   However due to the same reasons, they are also commonly predisposed to infections

User :   one last question...ho will she take montec tablet she is so small?
Doctor :   Hence if you notice that she develops fever or has severe cough she would need antibiotics along with these inhalers,
Doctor :   You can dissolve the tablets in water or juice and can be given.
User :   she doesnt have fever but since it relaposed in 2 weeks doc says there mighyt be little infection

User :   so azithral given
Doctor :   Azithral, the antibiotic can also be given similarly,
User :   and the duanase spray was also steroidal so can i just stop is abruptly? doctor here asked to stop it and asthaline
Doctor :   Duonase spray are not steroid sprays.
User :   is it but my doctor said it a steroidal local spray
Doctor :   I mean they contain antihistaminic apart from the steroid.
Doctor :   and since the dose compared to oral steroids are less
Doctor :   and is used in inhalationally, it would not cause those effects as noticed with oral ones.
Doctor :   Hence you can stop them abruptly.
User :   and same with budedcort that us is it ok to give budecort once in a while or stop inbetween rather than giving at a stretch for say 7-10 days?
Doctor :   I would suggest you to follow the instructions given by the pulmonologist regarding the dosage and frequency of administerations.
Doctor :   Have you followed what I have suggested?
User :   ya that I am following but like they earlier said budecort when she was fine then they changed to Asthaline do Budecort was stopped and now agin budamate or just wanted to check that in these nebulizations tapering is not required unlike oral steroids and if stopping abruptly is ok
Doctor :   No, inhalational steroid does not need tapering.
User :   im scared as have heard a lot about steroids....mainly oral steroids

Doctor :   Tapering is done, if the levels of the steroids are high in the body, as in administeration of oral steroids.
Doctor :   However, if her symptoms get aggravated following the use of the newer products, I would suggest you to consult back to your pulmonologist.

User :   yes the symptoms were aggraveted when she was using asthaline and duonase...intitally it helped and she was ok...then suddenly again she got cold and cough so now doctors changed the medicines
Doctor :   Steriod inhalers are more potent and effective, in other form of inhalers are unresponsive.
