User :   i am not sleeping well..have tightness in chest and throat and pooping balck liquidy poops constant ringing in ears...and pain in my lower right chest front and back

User :   i drink around 18 to 20 beers a week
Doctor :   Do you use antiinflammatory medications regularly?
User :   not daily
User :   like for acid reflux
User :   i take zantac when it aks up

User :   yes i take excedrin ..helps with the ringing in my ears i get headaches from it

Doctor :   Let me explain you now.
Doctor :   As I mentioned presence of black tarry stool is a sign of upper GI bleed.
Doctor :   Presence of completely dark stained stools, indicate that the bleeding is a significant one.
Doctor :   You probably may be having this bleeding due to multiple factors
Doctor :   such as alcohol intake,
Doctor :   Painkillers (excedrin) that you take and
Doctor :   past history of gastritis and acid reflux that you suffer.

Doctor :   Ideally, you will need an endoscopy to visually examine the upper gut and accordingly treat the condition either by medical drugs, injectables or by banding techniques.
Doctor :   I would suggest you to consider consulting a gastroenterologist for this purpose.
Doctor :   In the mean time, other measures that you need are
Doctor :   1. Stop use of excedrin further, you can use simple analgesics -paracetamol containing drugs such as tylenol for the headaches.
Doctor :   2. Antacid syrups such as syp gelusil one teaspoon thrice a day before each meal.
Doctor :   3. Prilosec OTC one tablet a day before a meal for the next one week
Doctor :   4. Avoid all hot and spicy diet.

Doctor :   Dry pepper, coffee, or any beverages has to be restricted.

Doctor :   7. Watch out for your stools, if they continue to get darker, then you may need to visit an ER.
User :   i am going to mine after we talk..i wanted an idea on what direction....i have not been to work since last tuesday over all this
Doctor :   8. Lastly since you need the endoscopy and evaluation of liver functions, I would strongly recommend that you fix an appointment with a gastroenterologist as soon as possible.

User :   how about the pains in my lower right chest near ribs and tightness in chest and throat
