Doctor :   Hi
Doctor :
How can I help you?
User :   Hi Doctor..
User :   My wife has been recently having some stomach problems and we had consulted various doctors till now
Doctor :
Ok
Doctor :
What is it like?
User :   We went through general physician and then gastro who advised to take ultrasound scan.
Doctor :   Ok

User :   the output of the scan suggested a heterogeneous mass in stomach and so the gastro doctor suggested taking a CT scan in abdomen and pelvic area.
Doctor :   Ok
User :   the report of CT scan shows that there is a possibility of having a dermoid cyst or a teratoma in the right ovary. so the Gastro doctor suggested to consult a gyno
Doctor :   Ok

User :   We consulted her and see is saying that only a surgery is option and they might have to remove her right ovary as it is affected.
Doctor :   Yes, if it is Teratoma, Surgery is the right option

User :   Me and my wife are scared a lot and need a good opinion with someone else before we go for the surgery.
Doctor :   Ok

Doctor :
I will explain you
User :   What should we do now? Can you suggest a good doctor or do you have any opinion on this doctor or who will be the best in this field?
Doctor :   Sure

Doctor :   This is a problem which has to be dealt by Gynecologist
User :   Also, is this teratoma cancerous or anything? Is it normal? Will there be no problems if one ovary is removed in surgery? How sure can we be that only one ovary is good for pregnancy?
Doctor :   The lesion turning malignant is rare

Doctor :   It has to be removed now
Doctor :
There is no option left
Doctor :
One ovary is quite sufficient for pregnancy
Doctor :   There wont be any impairment as such

Doctor :
after the removal of ovary
Doctor :   Who has opted to operate it?
Doctor :
Is it a gynecologist?
User :   She is a gynecologist who has done MBBS, MD in gyno i think.
Doctor :   Ok

Doctor :
Has she said that she herself will operate it?
User :   She said she will do it.
Doctor :
Fine
Doctor :
How is your wife?
Doctor :
Like has she lost weight or anything like that?
User :   She is fine healthwise.. But we are scared and hoping that it should not be cancerous and should not affect fertility. So we are thinking of taking a second opinion.
User :   Also, which do you think is the best hospital where they treat teratoma?
Doctor :   Ofcourse there are many good hospitals

Doctor :
It could be Apollo
Doctor :   Manipal or Wockhardt
Doctor :
Anyone one of these
Doctor :
Even Accura is fine
Doctor :
If you want a non-coroporate hospitals
Doctor :
there is Chinmaya Mission Hospital
User :   We are living very close to Acura. So if you think Acura is good enough, then our opinion will be to go ahead with the same doctor as she was cordial and nice and has impressed my wife.
Doctor :   Fine
Doctor :   But you will need to ask about the OT facilities there
Doctor :
And I dont think that an ICU is there in it
User :   What is OT facilities? I am not getting it.
Doctor :   Operation Theatre
User :   The doctor said that she has done operations in Acura itself and it is she who said she can do it there. Anyways, i will confirm it there..
Doctor :   Fine
Doctor :
No problem
Doctor :
as it is an elective surgery
Doctor :
even an ICU is not essential
Doctor :
You can go ahead
Doctor :   The main procedure is to excise(cut) it
Doctor :
Thats it
User :   ?? what is elective surgery? She had actually said that the dermoid cyst / teratoma is quite large and laproscopy cannot be done.
Doctor :   Elective means planned

Doctor :
versus emergency which is unplanned
Doctor :
Bcos emergency surgeries complications will also be more
Doctor :
Thats why ICU I said
Doctor :
But for planned surgeries
Doctor :
everything will go fine
User :   Do you feel we have to go for Wockhardt, then is fine with me.. Just curious as i want to do the best.
Doctor :   Ofcourse the facility wise

Doctor :
it is better
Doctor :
But the doctor is also important
Doctor :
With your doctor very much confident about it
Doctor :
it is not necessary to go to Wockhardt
User :   Also, please suggest what other things we have to ask the doctor and other things which we have to take care before and after the surgery..
Doctor :   Plan it well wrt your time

Doctor :   Keep a list of blood banks
User :   That's good. I will confirm about the facilities once again with the doctor.
Doctor :
if it is necessary
Doctor :
Take a plastic tub with you
Doctor :
a small one
Doctor :
because they might ask for it after the surgery
Doctor :
to keep the excised part in that
Doctor :
If you have medical insurance
Doctor :
get the things processed
Doctor :
Stay calm
User :   She had actually told that they will send the excised part for further tests to confirm somethings.
Doctor :
Reassure your wife that it is a simple procedure
Doctor :
Repeat it many times
Doctor :
so that it gets registered in her brain
Doctor :
Yes they will send it
Doctor :
But to carry that
User :   Sure.. That is what i have been doing it so long.. I will continue to do it..
Doctor :
they will ask for a plastic tub from the patient
User :   Ok.. I will take a plastic box..
Doctor :
If your wife has any allergies mention that
Doctor :
Drug allergies I mean
Doctor :
One day before admission is required
User :   I am not aware of any allergies till now..
Doctor :
and 2 days after operation
Doctor :
then discharge would be planned
User :   ok..
Doctor :
as it is Laparoscopic it would be faster
Doctor :
Be prepared to rest your wife for atleast 2-3 weeks after the procedure
Doctor :
Not much strain
Doctor :
Do talk about the sexual intercourse during followup with your doctor, Not now
User :   No.. the doctor told that the cyst was little big ( 12x10x6 cms) and it can be done only through open surgery.
Doctor :   Ok

User :   For sure..
Doctor :
Then hospitalisation would be for 4-5 days after procedure
Doctor :   If possible, go for a second opinion with the reports to another gynec

Doctor :
and ask if laparoscopic method be done
Doctor :
But size does not allow
Doctor :
ofcourse
Doctor :
But second opinion is always Welcome
User :   ok.. I will go for an opinion in Wockhardt and see what they say.
Doctor :   Ok

Doctor :   Is there any thing else I can help you with?

User :   So for the operation, other than blood bank and the plastic box, should we be ready for anything else?
Doctor :   No
Doctor :
Thats it
Doctor :
Blood bank is main
Doctor :
bcos they might trouble you
Doctor :   What is her blood group?
User :   Do you know other famous doctor whom we can talk to for the opinon?
Doctor :   Dr.XXXXXXXX Sachidananda

User :   Her blood group is O+ve. Is it rare or is it easily available?
Doctor :   Positive is fine
Doctor :

Doctor :   I have provided it

Doctor :

User :   Thanks doctor.
Doctor :   Is there any thing else I can help you with?

Doctor :   Meanwhile please fill the feedback form say done when done.
User :   Done doctor. Thanks for the info.. But just to make sure, i am asking this question again.. Is it completely safe to remove one ovary? Is there any test which we can do to determine if the other ovary is fully functional and good for gestation?
Doctor :   Now that is the next question
Doctor :
But for now, excision is mandatory
Doctor :
One ovary is sufficent
Doctor :
Ovulation tests should be done to know its functional capacity
Doctor :   Ask your gynec more about that

Doctor :
She will tell you
Doctor :   If it is healthy no issues with pregnancy
Doctor :
Quite sufficient
User :   Thanks doctor. Thanks a lot for the help.
