User :   For the last week I have had white patches on the back of my throat it is painful to swallow I have been gargling with warm salt water and Lysterine. It has affected my voice ( unable to speak normal tone or volume) I am 53 have SLE and had my tonsils removed when I was 4.
Doctor :   Hi
User :   Did you get my question
Doctor :   I have gone through your query.
Doctor :   May I ask you a few questions pertaining to th e problem to help you better
User :   Yes please
Doctor :   May I know if you have any fever, cough or cold or a running nose in the recent past ?
User :   Cough and with SLE I usually have a low grade fever, may I add I am exhausted all the time and can't seem to get enough sleet, more congested than runny nose
Doctor :   Ok.
Doctor :   A low grade fever , sleeplessness and getting tired or exhausted are the common most symptoms of SLE, but regarding the white patch it could be something like a simple yeast infection to a fungal infection called Candidiasis .
User :   Hard to swallow - feels like lots of mucus in my throat - mostly of a yellow-brownshish color
Doctor :   Ok.
Doctor :   Many patients with lupus develop painless ulcers in the mouth and nose at some point in their disease. Abdominal pain in lupus can be due to inflammation of the lining of the abdomen, infection of the intestines, low blood flow to the intestines caused by a clot, or inflammation of the vessels flowing to the intestines, stomach or sometimes even esophagus,
User :   I have trouble taking antifungal drugs make me sick
User :   would inflammation from SLE cause white patches
Doctor :   And as you have pain and white patches , they might be a super added bacterial or fungal infection.
User :   I also unfortunately am unable to take methyltrexate or embrel type stuff
Doctor :   Ok.
User :   Do I need to to go to er my doctor can't see me for another week and my rheumatologist is located 90 miles away
Doctor :   Ok.
I would suggest you to get a swab test for culture and sensitivity from a local doctor rather than a rheumatologist as it can be a fungal or bacterial infection for which you might need a specific antibiotic to which it is sensitive or at least a broad spectrum antibiotic.
Doctor :   I would also give you some OTC Medication.
User :   Any home remedies by chance - what type of of OTC medicines
User :   Any chances I am contagious I have 9 small grandchildren
Doctor :   I would just suggest that if you have cough by chance then stay away, otherwise they are not too contagious, except for in infants below 1 yr.
1.I would suggest a Tab.Tylenol for 3 days per day 500mg thrice daily.
User :   OK I will try to find a Dr. or clinic or something - any other suggesstions?
Doctor :   2. A mouth wash with listerine atleast thrice daily
Doctor :   3. A warm water with salt rinse thrice again with lot of gargling.
User :   Am I contagious to my 80 year old father who is not in good health
Doctor :   Not much. You do not have a repiratory problem.
User :   Can't take tylenol will ibuprofen work
Doctor :   Ok, No problem.
User :   Ok thank you so much for your advice and time it's difficult when you live in middle of nowhere (ha)
Doctor :   I hope you get well soon out of this small infection
Doctor :   Thank you for consulting me
Doctor :   Take care
Doctor :   bye for now
User :   bye
