Doctor :   Hi
Doctor :
How can I help you?
User :   My wife was 9 week pregnant when she had fever and detected Plasmodium faciparum. Our local gynecologist suggested us to get it treated through the general physician. The general physician admitted her in the hospital and injected rez-q injection through saline in her blood. For some personal reasons we had to taken a forced discharge after 2 days of hospital treatment. Thereafter the malaria was cured she did not had fever and ever the blood report did not detect Plasmodium faciparum.

After some 4 days time we did sonography and found that the fetus was dead and was a missed abortion. Now two weeks after the D&C in the blood report we have elevated SGOT & SGPT.

We have the following questions: -

(1)     Reasons for elevated SGOT & SGPT.
(2)     How can we be sure that she would not be affected by Plasmodium faciparum in out next pregnancy?
(3)     Do we need to check if the ovulation is properly happening after the D&C
(4)     When can we try for the next pregnancy, Her first periods have not arrived as yet.
Doctor :
Let me read it
Doctor :
I shall answer the questions in reverse order
Doctor :
You can try for the pregnancy after 4 months
Doctor :
3) You need not check for the ovulation
Doctor :
next time
Doctor :
D & C does not necessarily affect the ovulation
Doctor :
2) If the treatment is completed for plasmodium falciparum
Doctor :
the recurrences are very uncommon
Doctor :   If you have a slight doubt that you have not used the complete treatment

Doctor :
You have to take it completely even now
Doctor :
1) The reasons for elevated SGOT and SGPT are due to two things a) The malaria b) Medicines for treating malaria
Doctor :
However the elevated SGOT and SGPT return to normal if it is a reversible damage to the liver
Doctor :
I hope I answered all your questions
Doctor :
Do you have more?
User :   The treatment of malaria was incomplete, malaria was not dected in blood since 3 weeks she did not had fever for 3 weeks however still after 3 weeks the SGOT & SGPT have not been in normal range
Doctor :
SGOT and SGPT should take long time to recover
User :   She is normal now feeling good however just as a concerns do we need to do some more tests to find the reasons for increase in SGOT & SGPT
Doctor :
You may wait for some more time
Doctor :
Mild to moderate elevations of the liver enzymes are commonplace.
Doctor :   If a repeat LFT show the levels are reduced than the previous values then you should not worry
User :   Now malaria is no longer detected in blood however since the treatment was incompelete do we need to compelete the treatement now or after her first period
Doctor :   It is good to complete now

Doctor :
Please see a General Physician for a prescribed dose
Doctor :
and schedule
Doctor :
Take the script of medicines that you had taken
Doctor :
previously
Doctor :
so that there is no confusion
Doctor :
and hint him about the elevated Liver function tests
Doctor :
Ok?
Doctor :
Did you follow?
Doctor :
Are you with me?
User :   yes
Doctor :   Did you follow?
Doctor :
Do you have any questions?
User :   Yes, please wait for 2 min
Doctor :
In case you are busy with something else you can always chat with me later as we are available 24/7
User :   She was pregnant when we visited the general physician for malaria treatment. However since malaria was not detected in blood Doc said since she is pregnant he would not prescribe any medicine and even malaria is not detected in blood after the 2 day incomplete treatment in the hospital
Doctor :
Why did the doctor give the Rez q injection which is contraindicated during pregnancy
User :   We are not aware but we had to leave the treatment incomplete for some personal reason, Malaria got cured however the fetus was dead.
Doctor :
The doctor could have put antimalarial treatment seeing her clinical status
Doctor :
not on the basis of test done
Doctor :
The test is not gold standard for the malarial treatment
Doctor :
If the doctor feels that the symptoms are suggestive of malaria then he can go with it
Doctor :
since malaria is endemic in this part of the world
Doctor :
Do not worry about what has happened
Doctor :
You can always try for next pregnancy
Doctor :
and take precautions liek using mosquito nets and repellents
Doctor :
See that your house is not infested with mosquitoes
User :   Now, since malaria has become shock of our lives do we need some more treatment for this to be sure it does not comes against our next new born. Please help
Doctor :
Precautions against mosquito bite is the best way to avoid it
Doctor :
and you can seek a chemoprophylaxis against malaria with the General physician while planning for pregnancy
User :   Some one told me malaria returs back after 6 mts if treatment is incompelte. Who is a chemiprophylaxis, where can we find one
Doctor :
It is a preventive medication to prevent against malaria given by a General Physician before you start planning for pregnancy
Doctor :
So seek the opinion from a General Physician in your area to complete the treatment
