User :   I have recently been diagnosed with endometriosis. They thought it was my gallbladder, and took it out but I'm still having the severe pain. I'm confused though because my pain is in my side, but it looks like I have endometriosis as I have cysts and am also on birth control. My doctor told me I was going to need laproscopy to remove the cysts and scar tissue. He also said he was going to start me on lupron. I have heard so many horror stories about it and am scared to death to take it. The pain is severe, when I have it, but only about 1-2 times a month. I don't all the side effects of lupron are worth it honestly. Can someone tell me why I would HAVE to take it, especially since it just treats the symptoms and doesn't cure it. And is there anything else I could do, possibly just birth control? I read that it's mostly for the pain during the period, which isn't really my problem, and for the heavy periods which I am not having as much now thanks to the birth control. What do I need this for? I am very scared and frustrated right now and my surgery is 2 days away.
Doctor :   hi
Doctor :   Dr.XXX here.
User :   hello there
Doctor :   let me read what you have typed.
User :   k
Doctor :   i will need to ask you few more questions to be able to give you a better answer
User :   ok
Doctor :   what are your symptoms at present? and when do these occur?
Doctor :   how long back was the gall bladder surgery done?
User :   it's very sporatic, but it starts in the stomach above the belly button, then moves to my side, just like the gallbladder issue, and it hurts so bad that i can't breath and when i can catch my breath i scream bloody murder, and then it moves to my back and hurts in my side and back, it's really scary for anyone around me and puts me in the ER all the time
User :   the gall bladder surgery was in march...still having the same symptoms though
Doctor :   ok. is there any relation of this pain to your periods?
User :   it's usually right before or right after, but not during
Doctor :   was repeat ultrasound and hormonal tests done after the surgery?
User :   after that gallbladder surgery?
Doctor :   is there any problems related to passing motion or passing urine before during or after periods?
Doctor :   yes
Doctor :   is there pelvic heaviness associated?
User :   i don't remember ever having a hormonal test, and the ultrasound never showed a gallbladder problem, i ended up having a bunch of tests and that was everyones best guess
Doctor :   ok.
User :   there isn't really a pelvic heaviness i would say
User :   usually the first day of my period i have really bad cramps but i wouldn't say it's anything unusual
User :   also when i try to have a bowel movement during my period it hurts really bad
User :   like in my pelvic area
Doctor :   for the suspected endometriosis, get your hormonal tests done that should include TSH, FSH, LH, estrogen, progesterone and prolactin, insulin levels.
User :   but i just thought that was normal
User :   that should be done before the surgery?
Doctor :   no the pain while passing motion during periods is one of the characteristic symptoms suggesting endometriosis
User :   yeah i never realized that
User :   til i looked it up
Doctor :   go ahead with the tests and the laparoscopy first to remove the implants and the adhesions
Doctor :   this will reduce the pain and its intensity
Doctor :   for further prevention of endometriosis to occur you should be put on hormonal pills or progesterone pills
User :   i just feel like it's so when i read about people that have had that done, it's because they are in severe pain ALL the time and I don't have that
Doctor :   it need not be present all the time.
User :   my only real problem, that bothers me, it this really frightening pain i get in my side
User :   but it's weird that it would be there, when it's a problem around the uterus
User :   could i have the surgery and not be put on the hormones or lupron and see what happens?
Doctor :   but it should be confirmed and treated, the people who have the symptoms most of the days in teh cycle usually have advanced disease and for then hysterectomy is advised
User :   oh ok
Doctor :   so get started on treament soon
User :   could i take birth control continuously and not lupron?
Doctor :   avoid estrogen containing pills , estrogen tends to aggravate the endometriosis
User :   i just feel like honestly, the risks outweigh the benefits for me
User :   oh ok
Doctor :   having low doses of estrogen is neded, to make sure you do not have bone problems
Doctor :   so a low dose estrogen combined with high progesterone can be started by your gynecologist after the laparoscopy
User :   yeah that's another issue too, i know i probably have bone problems as well as i really don't get calcium...i hate milk, probably should be taking calcium pills but dont
Doctor :   to make sure you dont have recurrence
User :   can't i just risk it and see what happens?
User :   or is that a terrible idea
Doctor :   then in such cases you will need calcium supplements too to avoid sudden fractures.
Doctor :   it is a terrible idea, the more you delay, the more intense your problems will get
Doctor :   so do not delay your treatment
User :   the pain is terrible, don't get me wrong, and literally makes me wish i was dead, but it's only every other week or so, and i just feel like it's so much of an issue with this lupron that id rather have the pain
User :   no other suggestion but the lupron?
Doctor :   no, i suggested you the combination pills
Doctor :   they have nuch less side effects than the lupron
User :   oh ok
Doctor :   discuss with your gynecologist about getting started on them rather than lupron.
User :   ok i will
Doctor :   after the laparoscopy
User :   thank you!
User :   yes, i will have that done
Doctor :   is there anything else I can help you with?
User :   no, thank you very much
Doctor :   thanks for consulting me. bye for now.
