User :   Hi I think i spoke to you earlier about my high bilirubin

User :   So I have had it since february 2009, everything else has always been normal
User :   however my doctor said that the abnormal cells are causing her concern
User :   stomatocytes atypical lymphs and target cells
User :   this has caused me extreme anxiety
User :   as she mentioned chronic liver disease
Doctor :   you could be having a chronic viral infection or some other metabolic issue
Doctor :   as i had mentioned earlier, you may be haivng a condition similar to gilberts syndrrome, where in you may have long standing low elevation of unconjugated bilirubin without any other findings
User :   Ok, but what would cause the abnormal cells?
User :   and the tiredness that i am experiencing?
User :   because as far as i was aware gilberts syndrome does not cause any symptoms
Doctor :   that is due to hepatitis associated with the infection
User :   what infection could it be
Doctor :   CMV Hepatitis A, B,C or EBV
Doctor :   All such viruses can be detected by serology tests detecting their specific antibodies in the serum
User :   Ok. Does my condition sound serious?
Doctor :   no
Doctor :   since you dont have major and severe symptoms it does not seem so
User :   the drowsiness is severe and is really affecting my life
User :   But i dont feel fatigue and exercise daily
Doctor :   are u taking any medications ?
User :   no
User :   could it be sleep apnea
User :   Or would it be related to the liver problem
Doctor :   no it does not seem to be sleep apnea as you are young and you are not overweight
User :   Ok so most likely a viral infection? Will this cause liver damage?
Doctor :   you may need medications to reduce the elevated bilirubin
Doctor :   and yes its a viral infection and it does cause hepatitis that can cause all your symptoms that u have mentioned earlier
User :   ok is there any additional tests or treatment that i need i have been told to go back every 6 months to have an LFT does this seem appropriate
Doctor :   a LFT , HEPATITIS SEROLOGY PANEL for hepatitis A, B & C , would be better
User :   every 6 months?
Doctor :   no serology tests , just once
Doctor :   and LFT can be done every 6 months or so
User :   ok what are the chances that there may be liver disease or something more serious?
Doctor :   the chances are less as all tests like USG, MRI have been normal
Doctor :   you can consult a hepatologist, who can allay all your fears by performing a liver biopsy, that will give the definitive daignosis and a solution to your problem
User :   I have seen a liver specialist but she states that the abnormality is so mild that she cannot justify the risk to me
Doctor :   ok
Doctor :   In that case i dont think that you should worry at all
Doctor :   just consider that you are normal and you have a variation in your normal liver function
Doctor :   and you can take some medications like prilosec or prevacid for the symptoms
Doctor :   As far as the pain under the chest is concerned, you can take some ibuprofen to see it it resolves

User :   the pain isnt so bad its got better over the last few weeks
User :   the only reason i havent been discharged yet is because of the abnormal bllod smear
Doctor :   then i guess there is no need to worry at all
User :   can you just explain to me what these cells are and what they mean
Doctor :   if you have had similar reports again and again, these could just be variations as well
Doctor :   atyical lymphocytes are altered variations in the normal, which have beem affected by a viral infection and have changed their appearance
User :   what about target cells and stomatocytes?
Doctor :   they are normal variations or precursors of red cells that have changed structure of the cell walls
Doctor :   this could be due to associated chronic inflammation or infection like situation
User :   ok
User :   how long do you think it will take for this to resolve or will i have this for years?
Doctor :   it may resolve any time or it can persist the same without causing any issues
User :   ok
User :   is there anything i can do or take in the meantime to help with the drowsiness
Doctor :   you can consult your GP who may give you some stimulants or prescription medications to help you out
Doctor :   also practice deep breathing excercises that may also help you out
