Let me brief you.
Sleep apnea has two different forms of it.
what are those
1. Obstructive apnea where the laxity of tongue, presence of extra fat all narrows down the throat especially while sleeping resulting in apnea episodes
2. Central apnea where there is no obstruction but higher centres in the brain doesn't work in cordination resulting in apnea episodes.

The treatments that I suggest are as follows:
1. He should shed extra weight from the body.
He is in over weight category based on his height.

The extra fat may be deposited in the throat regions leading to apnea.
Encourage him to shed the weight down.
and tring a lot of ways to reduce weight but not helping so far
We can talk about weight reduction measures later.

Before I come to the treatment, I would like to give you the options that you can try at home.
2. Sleep on the sides instead of supine (on the back).
3. Try steam inhalation or nasal strips that dilate the nostrils before going to bed.
4. If sleeping on the sides is uncomfortable, encourage him to sleep in a semirecline posture by placing couple of pillows or elevating the head end of the cot.

Simple means is to place couple of pillows so that his head and neck region is atleast 10 inches above the body.
6. There are certain medications that can aggravate sleep apnea such as medicines for cold/allergies (allergies) and sleeping pills.

Since he has obstructive causes, it would be a good option to see what that cause is.
If he as an enlarged uvula or extra pad of fat behind the throat we can use surgical means to correct it.
The procedure can be performed by ENT doctors / oral surgeons.
That will be an option you can consider if feasible.

he is trying ...but i dont see much improvement to be honest

Weight is not difficult to shed. I have seen plenty of people who have lost weight with dedication and motivation.
