User:My 2 1/2 year old was pushed down at school 4 days ago and has a knot and bruise on his lower back close to his spine. It's not getting any smaller. He is still running around and playing but doesn't seem to be sleeping as well as usual? I called the dr. Office and they said just to watch it but it's not getting any better.
Doctor:Hi,
Let me read your query for few minutes to be able to have a good discussion here.
User:Does he need to be seen for this?
Doctor:Why don't you please take him to the ER and see if it requires an x-ray to be done?
Doctor:Yes, he definitely needs to be seen for this
Doctor:You cannot ignore this as this is at the spine level
Doctor:And we must rule out what that knot kind of thing is
Doctor:Hopefully, there should be no internal injury to the spine
User:It is on the right side extremely close to his spin. Part of the bruise is on the spine. I've just been worries and they said to watch it but I notice the knot isn't getting any smaller.
Doctor:Could you upload a picture of the same here on this chat window?
User:He hasn't been acting like it's bothering him but I have noticed the past few nights he doesn't seem to be sleeping well.
Doctor:I can wait for it
Doctor:It would give me a better picture of how the lesion looks
Doctor:That is right, kids may not be aware of the injury
Doctor:Also, it need not present with pain
Doctor:That may be the reason he is not complaining
Doctor:are you there?
User:Yes trying to send it
Doctor:great
Doctor:let me look at it
Doctor:Yes, I have seen it
Doctor:Seems like nothing serious
Doctor:Have you palpated the knot?
User:In the picture you can't really see the knot. He wouldn't be still.
Doctor:How does it feel? is it smooth, bulging, hard?
Doctor:Yes, I cannot appreciate the knot much
User:It is hard.
Doctor:do you think it is the vertebra or the spine bone that is normally seen or you think this is something different apart from the spinal bone?
User:I can feel his spine next to it
Doctor:Ok, give me few details more - is he actively playing around without any problem of back pain or limping?
Doctor:Is he walking normally?
User:Yes he seems to be playing normal. And walking normal. The day it happened he didn't want you to hold him and was walking slow. That was before i even knew about it. He was at daycare and another kid pushed him and he fell and no one seen it.
Doctor:all right, I get the depth of it now
User:He usually wants to wrestle at night and he hasn't since that happened.
Doctor:I would suggest you to wait and watch for a week more
Doctor:Wrestling would be painful for him
Doctor:The knot should be gone by then
Doctor:If not, you can review him with the doctor
Doctor:Meanwhile, apply hot water bag locally three times a day
User:Thank you. I just wanted to double check and have a little piece of mind.
Doctor:you are welcome
Doctor:he should be fine soon
