Doctor:Can you explain since when are you having palpitation?
User:offlate started
User:since one week
User:Showed it to Doc - For b.p he adviced to ake envas 0.5 for a month
Doctor:Do you have it on all occasions or only during exertion?
User:no not all the time
User:when i get excited / or have urgent work & not able to do so
Doctor:If you met a doctor, did he check this palpitation also?
User:that time it wasnt
User:since 3-4 days i am feeling
User:what do u advice ? if i had b.p , I should have gone for B.P medicne or not ?
User:since , I have heard that if B.P medcne is started , I can nt leave
User:i guess palpitations is coz of anxiety
User:r u a physician?
Doctor:As far as your blood pressure is concerned, it is common these days to start blood pressure medicines when blood pressure is slightly high. Try if for couple of months and stop it once blood pressure drops.
Doctor:A pressure of 140/96 is not very significant, further if it was just a single reading
User:normal B.P reading is 120/80
Doctor:it is not conclusive of high blood pressure.
User:next time it was- 120/90
User:then third time 116/86
User:it was fluctuating
User:have got few blood test done - u can c my reports
User:have uploaded it
Doctor:Blood pressure fluctuation is common.
User:i was also having headache & pain in left arm - is it coz of b.p ?
Doctor:As long as the fluctuations are not very gross and under limits which is 140/90mmHg
Doctor:it generally isn't concerning.
User:what is the limit
User:normal limit
Doctor:I usually don't start blood pressure medicines for a single high reading.
User:does it have side effects ?
User:Then what should i do ?
User:please advice
Doctor:I guess you have multiple questions in your head. Can we discuss them one after the other.
User:my cholestrol was bit high
User:ok sure
User:ans this question- Pain in left hand and headache - Is it coz of B.P ?
Doctor:If you wish to know about how your blood pressure should be managed, I shall address that first. Lets talk about your cholesterol and medicine side effects later once you have no doubts regarding blood pressure management.
Doctor:It would be extremely difficult to give you a satisfactory answers if you have random questions
Doctor:you can tell me which one should I address first.
User:blood pressure management
Doctor:As I described earlier, blood pressure is known to fluctuate.
Doctor:If a single blood pressure reading is above 140/90mmHg (which is the normal upper limit of blood pressure)
Doctor:I do not prescribe medicines.
Doctor:However as i said it is common these days to see physicians starting medicine with single high reading.
Doctor:So you can continue envas for a month. Recheck blood pressure again
Doctor:If the value is under 140/90, you can withdraw from blood pressure medicines.
User:even if it is 120/85
Doctor:Your doctor should have started the medicine taking into the single high value into account.
Doctor:Therefore my suggestion to record blood pressure
Doctor:if it is under the upper limit value, you can withdraw from antihypertensive medicines.
User:when to check & how many times?
User:what is antihypertensive mdcn ?
Doctor:As you are on envas tablets now, you can recheck blood pressure 3 times on different days of the week.
Doctor:you can plan the check after a month of taking envas.
Doctor:Antihypertensive medicines are those prescribed for high blood pressure. It is the scientific name given by doctors for blood pressure medicines.
Doctor:Are your clear about BP treatment.
Doctor:if yes, proceed with your next question.
User:so envas is ok ?
Doctor:Envas for one month is ok.
Doctor:It can cause some amount of dry cough on long term use.
User:so my next question is - headache & pain in left arm is coz of B.P?
Doctor:Headache and left arm pain are non specific complaints. I don't feel BP of 140/96 is the cause of pain.
Doctor:It probably may be related to anxiety issues that you have.
User:then ?
User:can u suggest mdcn for palpitations - mild one
Doctor:It is important to understand the cause of palpitation.
User:how to detect ?
Doctor:I mentioned anxiety in the first place, since you described it.
User:the cause?
Doctor:palpitation and anxiety may be related to thyroid disorder or generalised anxiety disorder.
Doctor:on the contrary the condition may be related to irregular heart beats.
User:my thyroid tests are fine
Doctor:Have you undergone ECG?
User:not yet
User:pls suggest mdcn
Doctor:If palpitation issue is bothering you, I suggest taking ECG.
User:if ecg is fine then ?
User:cant u suggest mdcn for palpitations ?
Doctor:If ECG is normal, then palpitation should be treated along with anxiety symptoms with antianxiety drugs. I put my patients on clonazepam a low dose at night.
Doctor:And refer them to a psychiatrist for medical management of anxiety.
Doctor:However if ECG shows abnormal rhythm or high paced heart rate then you will benefit from beta blocker.
User:can i take clonazepam ?
User:what is beta blocker ?
Doctor:It is a benzodiazepine with slight addictive potential; therefore the next step is getting ECG
Doctor:Beta blocker is a class of medicine given to slower heart rate.
Doctor:I don't recommend you try medicines randomly.
User:then what is your advice ?
Doctor:As the palpitation is bothering you, you should get an ECG.
User:ok , will get it done
Doctor:Visit a cardiologist who can ausculatate the heart.
User:how to manage B.P?
User:ausculate the heart means ?
Doctor:i am sorry, I just explained earlier about how to manage the current high blood pressure recording.
Doctor:I meant a cardiologist who can examine the heart with a stethescope.
User:no no means what precaution shud i take frm my side ?
Doctor:He will decide further to put you on medicine or investigate further.
User:ok thanks
Doctor:It is absolutely necessary that you avoid all physical and mental strain.
Doctor:This helps to keep blood pressures down.
User:can u go thru my medical reports ?
Doctor:Eat regularly, sleep well and start mild exercise.
Doctor:Reducing salt intake also helps.
User:i do treadmill daily fr 30 min
User:is it fine ?
Doctor:Treadmill for 30 minutes is fine; however you need to include other measures that I suggested too.
User:yes will do
User:thanks .
