User :   my son is 6 years old. He has been suffering for a couple of days from some mild stomach ache and mild diahrea. His physician suggested that we do abdomin ultrasound. the ersult is that his liver is 12.5cm and spleen is 10.8cm which was concluded by the sonar doctor to be mild hepatospleenomegaly. His physician thinks that for such age... these sizes are not alarming and might be transient with any infection in his digestive system. can you advise me?

Doctor :   Can you describe the type of pain your son has and the location of pain?
Doctor :   Since when is he complaining of the pain?
User :   just early morning sickness (feel to vomit) and very little intermittent colon contractions... his stool analysis indicated little undigested food and few pus cells
User :   it is all within 3-4 days... his physical condition is generally OK and his apetite is very normal
Doctor :   Does he have symptoms such as vomiting, jaundice associated with the pain?
User :   he vomits only in the morning either after the tooth brushing or after the long trip of the school bus

Doctor :   Is he running fever?
User :   only today his temperature elevated... all along the apst few days he was normal
User :   his younger brother (1.5 years) is having Chicken Pox and we thought that today's fever might be that he caught it? ... we did not link it to the abdomin diagnose

Doctor :   Do you have other blood report with you?
Doctor :   * I mean if any other blood tests were performed for the same problem?
User :   we have not done any lately... but his CBP 6 months ago was very normal

Doctor :   Let me explain to you.
Doctor :   The ultrasound findings reveal liver and spleen enlargment (spleen is more enlarged than liver).
Doctor :   However his clinical symptoms are not significant enough to reveal any major illness except for the infection.
Doctor :   Hence I agree with your doctor, that it could be this current episode of the infection that could have triggered the liver and spleen enlargement.
Doctor :   He will need a repeat ultrasound scans as his current infection gets controlled (probably after a few weeks)
Doctor :   if there is no improvement with the liver and spleen enlargement,
Doctor :   then blood test to look out for other causes such as a anemia, chronic infection, metabolic conditions, leukemia needs to be done.
Doctor :   These test can be done under guidance of a pediatrician.
Doctor :   Have you followed what i have suggested?
User :   yes sure
User :   you do not suggest any immediate action
Doctor :   At this point of time, a complete blood count with malarial test
Doctor :   is probably necessary.
Doctor :   If they are normal, then other tests can be done following a repeat ultrasound scan.
User :   In fact, I was wondering if his physicall wellness now is indicative of elemenating any of the above .. especially the scary Leukemia
Doctor :   His present symptoms are consistent with the feature of viral infection.
Doctor :   And yes, since leukemia can be asymptomatic previously, complete blood count
Doctor :   can rule the condition.
Doctor :   Hence I do recommend, you to consider getting the blood count.
Doctor :   If they are normal, then I do not find no major reasons to concern.
Doctor :   Hope it is clear to you.
User :   in this respect I might have to add a note on his diet system: he does not eat fruits at all.. only preserved juice... he does not eat cheese... all school sandwiches are sussage, bastrami or fried chicken pane....

User :   that was very informative thx alot... is the blood count the same as Complete Blood Picture?
Doctor :   Yes,
Doctor :   the test will include total counts, RBC counts, differential counts, platelet counts and few other markers.
Doctor :   A normal blood counts with normal differential counts can rule out leukemia almost completely.
Doctor :   A bone marrow test is the confirmatory test in diagnosing leukemia.
Doctor :   Have you followed what I have suggested?
User :   yes sir
