User :   I have had irregular periods for several years now. I get a pap smear yearly. Last year my gyno thought it could be my birth control pills. So she changed them. My periods are still irregular. I take my pill between 9 and 11 every evening. I noticed today that after intercourse I had blood. I didn't and don't have any pain but do have pain during my period in my pelvic area on the right side. What are your thoughts? Any help would be appreciated.

User :   It has been off and on for about a year I did go to the doctor about this and they didn't have any concerns
Doctor :   Can you describe the nature of the pain?
User :   just a constant stabbing pain on a scale of 1 to 10 i would say about a 4 or 5 just bothersome and I know its there

Doctor :   How irregular are your periods?
User :   It varies sometimes I will go 15 days sometimes 27-30 days
Doctor :   Is the flow normal during the periods?
User :   No sometimes very light and other times heavy

Doctor :   The pain which you mentioned, is it present only during the periods?
User :   About a week before my period during my period and rarely the week after

User :   no

Doctor :   Have you been evaluated by a doctor for the possible causes of irregular periods?
Doctor :   In the past?
User :   yes and they just thought it was my birth control pills I was taking ortho tri cyclen lo and they changed it. I can't remember the one they changed it too sorry

Doctor :   Let me explain to you.
Doctor :   Since, I cannot perform a physical examination, I would not able to give you a definitive cause for irregular periods.
User :   Does the same go for bleeding after intercourse
User :   My obvious concern is cancer
Doctor :   Since the bleeding is noted for the first time, it is unlikely that any cancerous lesion is responsible for it.
Doctor :   Besides you do not seem to have other features of cancer according to your history.
User :   What about endometrosis?
Doctor :   Hence it is probably not related to cancer.
Doctor :   Endometrosis is one among the causes for irregular periods.
Doctor :   There are few other causes which also may be responsible for it.
Doctor :   It would include polycystic ovarian cyst, fibroids.
User :   I was diagnosed with HPV 6 years go but have never had an outbreak and have never had a irregular pap
Doctor :   Other rare possibilities are anaemia, endocrine abnormalities.
Doctor :   Since you are following up with pap smear, and if the reports are normal, then it is unlikely to be cervical cancers too.
Doctor :   So do not worry about the cancer.
Doctor :   It should be one of the above mentioned causes that may be responsible.
User :   Thank you so much. Do you think I should go ahead and make an appt with my doctor or just wait for my yearly exam in August
Doctor :   Since you also report pain during menstruation, I would suggest that you would need evaluation, which would also include an ultrasound.
Doctor :   It is not an emergency, and you can report to the doctor at the choice of your own.
Doctor :   However, if the pain is severe and if the pain last long or if they are frequent, I would suggest you to consult one very soon and not after a year.
User :   Ok thank you so much for your time I really appreciate it. I am assuming the other things aka polycystic ovarian cyst, fibroids, endocrine abnormalities can be treated

Doctor :   Yes, these conditions are treatable.
User :   Yes I have followed very closely thanks again for your time and I will follow up with my doctor
Doctor :   However the choice of the treatment can be either through drugs or through a surgical procedure.
Doctor :   That can only be decided by the doctor who is evaluating you.
