User :   Hi, my father is suffering from compressive myelopathy i.e. spinal tuberclosis at d6 and d7. the recent Mri done shows erosion of d6 and d7 vertebrae. He was operated 6 months back on d6 and d7 and accumulated granulation tissue was partially removed. The patient is on complete bed rest. TB medicines are being continued but the spine is very unstable. Doctors in kolkata are suggesting for another operation to be conducted to plate the d6 and d7 area so that the spine can be stabilise. Please suggest

User :   between 59 - 60 years. he is also a cardiac patient with blockage on the right side.
Doctor :   what all medications he is on now ?
User :   He is only continuing with TB medicines since april 2010.... no cardiac medication..... his heart problem is under control now.
Doctor :   so your main concern right now is if he should undergo another surgery or not ?

Doctor :   well your father is at a age where its advisable for him to undergo another operation to make sure the spine is stable .
Doctor :   it is not recommended in older patients but your father case he can undergo the operation to make it stable

User :   another concern is that since TB germs still exists how successful the operation would be in such circumstances... i mean since d6 and d7 vertebrae has been eaten up by the germs and there is a gap between the spinal bones.... now when post operation since the tb germs still exists the germs would again eat up the artificial fillings.
Doctor :   TB is completely cured with the medications thats Why he is still on them .. It takes anything from 6 months to 12 month of course. so by the time he undergoes the operation the medicines are already doing their work to curb the ingection

User :   what about the compression ...according to the doctors here the nerve compression is from the back and also from the front side of the spinal... the patient at present is not mobile as his left leg is paralysed and he has also lost his control on urine and bowl....he is on cathedral

User :   During the operation will the doctor inject some vaccine inorder to prevent TB spreading further. What more should i ask the doctor here would be conducting the operation

User :   what if neuro surgeon handles this case is it right that the neuro surgeon handles this. What is MCH?

User :   despite nearly 6 months TB medication there is a erosion of one bone below d6 and d7 ....what is your opinion on this.
Doctor :   it takes minimum 6 months for TB medications to work
Doctor :   the course itself is anything from 6 to 12 months
