User :   Hi doctor, I am suffering from the sudden drop of my blood sugar when I fast for more than 8 hours and much more after eating 4 hours. sometimes I feel that I am going to faint.
I had a GTT results, TSH which is high(6.09), Insulin(1.55) and c-peptide in addition to CBC which shows a mild decreased in white blood cells and increase in lymphocytes
Doctor :   Hi
User :   Hi doctor
Doctor :   Have you consulted an endocrinologist for this and were you prescribed any medication ?
User :   the tests were ordered by an internal dpctor, but I didn't go again yet
Doctor :   How are your HbA1c ?
User :   sorry what is HbA1c?
User :   is it something from blood count?
Doctor :   It is the blood sugar level for more than 3 months
User :   I have not done any thing before
Doctor :   Are you a known diabetic or do you have hypertansion or any thyroid problem ?
User :   cause I thought it is normal for every body to feel like that after 4 hours of meal
User :   I am not, but the result of TSH was 6.09 and 1 year ago was 9
Doctor :   Ok.
User :   my sister is diabetic
Doctor :   oK.
Doctor :   oK, That shows you have hypothyroidism.
User :   will you explain more doctor plz?
Doctor :   Do you feel sober, feel very dull at times, sleep a lot, etc ?
User :   yes and I am depressed, on zoloft medication for it
User :   almost tired and dizzy most of the time
Doctor :   Ok.
Doctor :   Your Insulin levels are fine, not a big problem.
Doctor :   But I would recommend you to do further 3 tests : Doctor :   1. HbA1c
Doctor :   2.PPBS
Doctor :   3.FBS
User :   what they suppose to show, doctor?
Doctor :   These 3 tests will give a total picture of your blood sugar status .
Doctor :   And you have to consult an endocrinologist for your hypothyroidism.
Doctor :   You have to take thyroid hormone substitute medication.
User :   If the problem left without treatment, it may goes worst?
Doctor :   Yes very much, you have to get it treated.especially hormonal problems have no other alternative, you have to take medication.
User :   is it related to the decreased white blood cells?
User :   white blood cells 3900, lymphocytes 45
User :   are these results fine?
Doctor :   These are the early symptoms of Hypothyroidism : constipation, carpal tunnel syndrome (wrist pain, and numbness of fingers and hands, usually associated with repetitive movement, such as typing or piano playing), getting tired very easily, and very low cold tolerance. Later symptoms are: dry skin, anorexia, increasingly deep voice, hair begins to fall out very easily (for example, when running a comb through the hair it is full of hair that has fallen out, this is especially true with long hair), increased weight, depression, and puffy eyes.
Doctor :   Ok, You also have infections due to poor metabolism.
User :   OMG, my hair started to fall increasingly 5 days ago, and that time i start to feel constipated and lost appetite
Doctor :   Advanced hypothyroidism, known as myxedema, is rare, but when it occurs it can be life-threatening. Signs and symptoms include low blood pressure, decreased breathing, decreased body temperature, unresponsiveness and even coma. In extreme cases, myxedema can be fatal.
Doctor :   I would hence advise you to switch over to treatment.
User :   is 90 over 60 considered low?
Doctor :   Yes.
Doctor :   May I know your age please ?
User :   32
Doctor :   As you are very young You should not delay and take treatment.
Doctor :   It has to be 120/80 at least 110/80
User :   well doctor, there will be any specific medication to control my blood sugar? or only the hormonal medication will help ?
User :   hello
Doctor :   you have to use anti diabetic drugs
User :   anti diabetic? like glucofag?
Doctor :   There are huge no. of drugs for that, not to worry.
Doctor :   Yes, there are now more than 200 combinations of drugs
Doctor :   to stimulate pancreas , to stimulate insulin secretion, etc.
User :   fine, but do my symptoms mean that I have that problem from along time?
User :   I gave birth 5 month ago
User :   and I breastfeed my baby
User :   does it has any effect?
Doctor :   That can be diagnosed by HbA1c which gives the blood sugar values for last 3 months.
User :   so, that test will take 3 months?
User :   or i shows previous results?
Doctor :   Usually Pregnancy induced Hypertension, Diabetes , called gestational diabetes and pregnancy induced hypothyroidism.
Doctor :   is seen.
Doctor :   In most of them it decreases after 3-6 months
Doctor :   But in some of them it continues to stay.
Doctor :   So you might have got during gestation.
User :   like my sister
Doctor :   Has she got it during her pregnancy ?
User :   i gained 18 kg during pregnancy for no reasons
User :   yes she did
Doctor :   That is the major risk factor for GD & GHthyroidism.
Doctor :   You should not have gained more than 12 kgs.
Doctor :   it is international norm.
Doctor :   hasnt the gynecologist advised you ?
User :   I know , usually I gain 9 kg in pregnancy, except this time
Doctor :   Ok.
User :   I tried even to put myself on strict diet, but it didn't work
Doctor :   Ok.
Doctor :   May I know if this is your second pregnancy ?
Doctor :   Are you a primi or muti gravida ?
User :   it was the forth
Doctor :   Ok.
User :   gravida4
User :   it was para2 plus one
Doctor :   As the no. of pregnancies increase the risk of getting diabetic and hypothyroidism also increase as you might be knowing.
User :   aha I c
User :   but it doesn't mean I may develop diabetic in the future, right?
Doctor :   Mothers who had gestational diabetes and had got better after a while, have greater risk of being diabetic than normal woman.
Doctor :   I would recommend you to have a very strict exercise regimen, diet.
User :   feelin muscles and bones pain are related to the same problem?
Doctor :   But also take care so that you have to support your baby as you are a feeding mother.
Doctor :   Usually there will be pain, due to diabetes
User :   thank you doctor
User :   good bye, take care
Doctor :   Thank you for consulting me
Doctor :   take care
Doctor :   bye for niow
