User:Hi Sir,

User:She is around 55 years age. From last one year she is saying her heart beats are suddenly increasing and which causing some kind of tension and little shivering of hands and legs and after 30 min she is coming to normal. Now a days she is telling her mouth is having bitter taste i mean something opposite to sweet taste. But she is facing this problem frequently now a days.
User:We do body checkups for every 6 months usually which is having BP, Sugar kind of tests. Recently also she gone through all these checkups and all are normal
User:BP - 120/70 Sugar - 76 ( doctor said normal ) ECG - ( doctor said normal ) so recently we consulted both lady doctor ( gynecologist named maha XXXXXXX ) and a cardiologist ( rangaswami ) all said normal
User:Reg Medication:
User:Cardiologist suggested DOLO 650mg when ever she feels little tension nature in heart area. And she uses longifene tonic, becadexamin capsules one per day, bio d3 plus 2 per day, trika 0.25 at night
User:But still problem persists... No idea what to do.
User:you need any further info reg my mother or any thing
Doctor:Was an ECG of the heart done?
Doctor:I hope that and echocardiogram was normal?
Doctor:What about thyroid check?
User:i am not sure of what is echocardiogram but ecg was done and doctor said nothing to worry
Doctor:Was thyroid profile also done?
User:thyroid was done 6 months back it was normal
Doctor:Let me brief you.
User:Yes sir
Doctor:Palpitation which refers to an abnormal pounding sensation of heart beat occurs due to numerous reasons.
Doctor:Arrhythmia's and cardiac problems are one of those cause.
Doctor:It was pleasing to know that heart check and ECG was normal.
Doctor:In the absence of a known cardiac disease other causes that can cause similar symptoms are considered.
Doctor:The list of other causes include anxiety, hyperthyroidism and underlying fever/infection.
Doctor:Since you mentioned all her check up was normal, her symptoms may be due to anxiety neurosis.
Doctor:Anxiety can also cause other non specific symptoms that include altered taste.
User:sometime she says fever sensation then i usually check for temperature which shows normal or below than that with .5 or 1 degree difference
Doctor:The typical symptoms of anxiety neurosis are palpitation, shivering, sweating, feeling of increased body temperature, feeling of doom, increased urination and so on.
Doctor:This can be associated with other non specific symptoms.
Doctor:Therefore all her current symptoms could be secondary to the same anxiety phenomena.
Doctor:Trika is one antianxiety pill.
User:ooh.. can i observe that anxiety behavior in any way
User:actually trika was suggested by on family doctor for sleep..
Doctor:Anxiety is more of a personal symptoms.
Doctor:Other observable signs are very subtle and can be observed only by an experienced expert.
Doctor:Therefore, I am not sure if you or any one else in the family can be able to make out when an anxiety attack (other wise called panic attack) starts.
Doctor:Having said that, anxiety is confirmed only when all other possible causes are ruled out.
Doctor:*that was the reason why I wanted to know if all other investigations were normal.
Doctor:Now with regards to its treatment;
User:yeah i am sure about reports all are just one month back only (BP, sugar, ecg)
Doctor:If her symptoms are not better, you can report back to her treating doctor or a psychiatrist (who are experts in treating anxiety).
Doctor:He/she can adjust the dose of trika or perhaps add another anxiety pill.
Doctor:In addition to the medication, counselling - professional and by your family members is important.
Doctor:During her symptoms, one of you can be with her and give her moral supports.
Doctor:You can instruct her to take deep and slow breathes / counting numbers.
User:so finally shall i consult any psychiatrist because we already talked with gynaecologist and cardiologist
Doctor:These small measure can make difference.
Doctor:In addition to a psychiatrist, I also would want her to see an ENT surgeon.
User:yes doctor actually we are 3 people at home... my my father my mother...
User:we are always in support of her always...
Doctor:Unidentified nose problems can have affect on the smell as well as taste.
User:ok i will suggest her regarding breathes also..
Doctor:And lastly it is important that she consumes plenty of fluids, sleep well and take regular and timed meal.
Doctor:All these have important role in treating her and making her healthy.
Doctor:Have you followed?
Doctor:Do you have any queries?
User:Here in this point i want to mention something
User:Regarding fluids: she drinks very less amount of water... i guess 2 liters XXXXXXX only... for last few months only i pushing her to have more water... and regarding sleep one more problems is "Dreams"
User:she lost all her mother, father related persons... in a span of 5 years... which was in 10 years back... from there onwards she tells all those are coming in dreams...
Doctor:2 litres of water a day would be sufficient.
User:i dont know how i am describing but... one of her problem is this nightmares
User:if 2 liters is fine then i am happy because after so much of push only she will have that much
Doctor:Anxiety is a pscyhological problem. And at times there can be more than one psychological conditions overlapping.
Doctor:Without an opportunity to talk and examine your mother it would be hard to predict / suspect the other associated conditions.
Doctor:Having said, such night mares are quite common post trauma.
User:so i would brief your suggestions let me know if i miss anything.
User:In addition to a psychiatrist, consulting an ENT surgeon. deep and slow breathes / counting numbers. consume plenty of fluids, sleep well and take regular and timed meal.
