Doctor :   Hi
User :   hello
Doctor :   How can I help you?
Doctor :   I am Dr.... here
Doctor :   Are you with me?
User :   just a min sir
Doctor :   Ok
User :   my father has diagnised with lower lip squamous cell carcinoma
Doctor :   Ok
User :   in the initial stages
User :   docs decided to first go with chemos + radiation
User :   after the dose is over docs said us to visit them again
Doctor :   Ok
User :   with in a month, another hard lump appeard on the other side of the jaw.
Doctor :   Ok
User :   they did FNAC and said that it is malignancy
Doctor :   Ok
Doctor :   was carcinoma detected only after that?
User :   again the chemo had started with much higher dose, i suppose, after 5 rounds of chemo
Doctor :   ok
User :   doc perfomred complete removal of both tumors followed by reconstruction surgery of lower lip
Doctor :   ok
User :   things were fine till 4 months of surgery
User :   everything appeared normal
User :   but suddenly the place where reconstruction surgery was done was not healing.
Doctor :   ok
User :   agian docs performed a biopsy and the results showed that the cancer has recurred again
Doctor :   ok
User :   at the same place where it was initally
User :   i.e. at the lower lip
Doctor :   ok
User :   the docs repeated the operataion and they informed us that the operation might be an extensive one
User :   the doc removed much larger portion of the affected area and cosmetic surgeon placed a flap removed near the chest
User :   he was recovering well.
Doctor :   ok
User :   and the removed portions were again sent for biopsy with the lymph nodes
User :   the tests revealed that out of 17 lymph nodes 2 are affeted with cancer cells
User :   remaining 15 are uneffected
Doctor :   ok
User :   and the wound again reappeared at the same place
User :   this times doc ruled out operation as well as chemos
User :   he is traating with medicines now
User :   i dont know the exact name
User :   of the medicine
User :   it is manufactured by Dr. redyy's lab
User :   want to know his future condition with regard to the unhealing ulcer he is having
Doctor :   Ok
Doctor :   First of all, such type of recurrence is common in many cases of squamous cell carcinoma
User :   oh
Doctor :   recurrence is not a good sign
Doctor :   especially after surgery
User :   yes dr., we are feeling the same
Doctor :   as surgeries are very much helpful
User :   can't he be operated in near future again
Doctor :   It is not advisable for repeated surgeries
User :   the condition he is having now is: the flesh that was placed over the lip bone is not at all attached to the bone
Doctor :   An oncosurgeon can comment on it after examining the case in person
Doctor :   Ok
Doctor :   for that, cosmetic reconstruction is a feasible option
User :   and more recent update is some kind of white liguid is coming from the walls of the ulcer, a kind of pus, occasionally
User :   and the bone that was chissled protruded from the flesh and is clearly visible if u see him
Doctor :   Ok
Doctor :   it shall be excised deeper levels
User :   we were in dilema
Doctor :   for good results
Doctor :   but it has not helped much
User :   generally whay will be the survival periods after the extensive surgery and the consequent recurrence
User :   more importantly what are the kind of symptoms one can observe from now onwards
Doctor :   if it is completely recovered the survival chances are good enough
Doctor :   Nw, the prognosis wise, it is difficult to comment for me
Doctor :   what is to be done is regular follow ups
User :   ok dr.
Doctor :   as only 2 lymph nodes of the recent operation
Doctor :   were malignant
Doctor :   out of 15
Doctor :   that is a positive sign
User :   since we are using medicines, doc said the cancer cells are arrested to that place only
Doctor :   Ok
User :   for how long it wil be
User :   ?
User :   difficult to say i guess
Doctor :   If the lesion has not reached to that level
Doctor :   it shall not recur after its excision
Doctor :   if it has invaded already
User :   oh
User :   what wil be the future symptoms if it has spreaded to other organs
Doctor :   the lesion recurs
Doctor :   that is the symptom
Doctor :   growth is the sign of cancer
Doctor :   after that sign
Doctor :   confirmatory tests shall be done
User :   oh i see
User :   does lip carcinoma is completely cured if diagnized early?
Doctor :   For your case, you should take him for a regular follow up with the treating oncologist/oncosurgeon
User :   yes dr. we are in constant touch with him
Doctor :   Ok
User :   wil have an appointment this week
Doctor :   that is the best thing to follow
User :   rest of his body is perfectly alright it seems
Doctor :   Follow ups to prevent further complications
Doctor :   Ok, it is good
Doctor :   squamous cell carcinoma is very much invasive one
Doctor :   and hence there are such recurrences
Doctor :   I wish him all Good health
User :   thank you dr.
Doctor :   Give him good amount of supportive care
Doctor :   and the very much essential emotional support
User :   will keep u updated once i talked to onco surgeon
User :   sure, we are supportive
Doctor :   that is the essential for his life maintainence
User :   ok dr.
Doctor :   to uplift his mood
Doctor :   as far as possible
User :   wil be in touch with u
User :   bye for now
Doctor :   Bye, wish you Good luck
