Doctor :   Are you back?
User :   *yes
Doctor :   Yes XXXXXXXX Let us continue
User :   yes, i do smoke
Doctor :   Ok Can you describe me a typical episode end to end
Doctor :   to have a clear idea
User :   so, i would be just doing the usual day to day things, wether im walking or sitting in a car. Then all of a sudden i would just start getting dizzy, feel the pain in my left arm and then i would feel my throat swell up.
User :   ...then in about a minute or so, after i get abit of fresh air or a drink of water it just goes away and im back to my normal self again
Doctor :   SO how much time does each episode stay
User :   only a couple of minutes
Doctor :   Since How long you have been having this?
Doctor :   Is there a particular precipitating factor for each such episode?
User :   not that long. it only started to happen about a month ago and then i started having them every couple of days. i tried thinking if i was eating something that was causing it but i never found anything. it just pops up out of nowhere
Doctor :   Are you in some kind of stress ?
User :   no, not that i can think of, unless my job, it can be stressfull at points but not that much.
Doctor :   Do you have any unfortunate incidents in your life?
User :   not that i can think of, no
Doctor :   Do you have fears for anything?
Doctor :   phobia?
User :   god, thats a good question now
User :   i dont think i have any phobias
Doctor :   any one in the family with the similar problem
User :   im not afraid of heights, spiders, confiened spaces etc.
Doctor :   that you know of
User :   nope
Doctor :   So do you mean to say the attacks are a replication of the same symptoms that you feel when you are in danger or suddenly something panicky happens
User :   yes, they are always the same symptoms, pain in left arm, swelling of the throat, dizziness
Doctor :   Ok the symptoms that you are narrating typical of a panic attack
Doctor :   but I am trying to find out the risk factors or the possible cause of this
Doctor :   so that you can try to reduce them or rectify them
User :   so, i wonder why i keep having them every couple of days even if im not in danger or scared, i could just be walking down the street, happy and then it hits me?
Doctor :   A panic attack is a sudden episode of intense fear that develops for no apparent reason and that triggers severe physical reactions
User :   oh right,
Doctor :   So most of the times one cannot find out the exact reason a person suffering with them
Doctor :   However the typical causes or risk factors are loss of someone very dear, hereditary, Stressful situations
User :   althought i have never lost anyone close to me, my family have told me that they havent ever had anything like it and like as i said, i could be walking down the street relaxed and then it would just hit me, i judt dont know.
Doctor :   But I could not find anyof these in you
Doctor :   Ok
Doctor :   except for the risk factor of Cigarette smoking
Doctor :   which sometimes can be remind of the similar episdoes
User :   does it increase it? is there anything i can do to try and reduce the attacks?
Doctor :   It is because of the gases it emits
Doctor :   and the effects of gases emited in the smoke trigger the same reactions
Doctor :   of panic attacks
Doctor :   So you can try to reduce the smoking and favourably quit it
Doctor :   as soon as possible
User :   although i have been smoking for almost 2 and a half years and it has only started about a month ago?
Doctor :   Yes even if your panic like attacks have a reason, it certainly increases with smoking
Doctor :   the frequency and the severity
Doctor :   There is no reason of how long you smoke
User :   ok
Doctor :   Now I can give you some self care tips
Doctor :   to reduce the frequency
User :   ok, good, thanks
Doctor :   which may benefit
User :   hopefully
Doctor :   Avoid caffeine, smoking, alcohol and illicit drugs, all of which can trigger or worsen panic attacks.
Doctor :   Practice stress management and relaxation techniques. Meditation, yoga are good options
User :   ok
Doctor :   Get physically active, since aerobic activity may have a calming effect on your mood.
Doctor :   Get sufficient sleep enough so that you don't feel drowsy during the day.
Doctor :   Wise decision is to seek the help of a psychiatrist
User :   i always feel drowsy though, i would feel better and more refreshed on less than 4 hours sleep than what i would on 8+ hours sleep.
Doctor :   or else first you can see a GP
Doctor :   who will examine you physically
User :   ill see my doctor and find out what he says and why would he examine me physically?
Doctor :   and see if there is a physical medical condition that is obvious of your attacks
Doctor :   One should not miss out the other reasons for such attacks
Doctor :   Remember the episodes that you get are panic like attacks
Doctor :   and it is not yet confirmed
User :   what else could it be appart from panic like attack?
Doctor :   To be diagnosed with panic disorder, you must meet the criteria spelled out in the Diagnostic and Statistical Manual of Mental Disorders (DSM).
Doctor :   Even if yu dont have the panic attack you will still benefit from the treatement
User :   why, what treatment would they give me?
Doctor :   anti anxiety medications
Doctor :   and if needed some counselling
User :   you see i dont want to start any medication
Doctor :   so you may not need medication
Doctor :   Counselling sessions will suffice
Doctor :   you mentioned about the sleep
User :   yip
Doctor :   please mention that to your GP too
User :   why, is it bad?
Doctor :   I mean to say a proper sleep is very much essential
Doctor :   a continous sleep of 7 -8 hours
Doctor :   at night
Doctor :   is the better way of leading a stress free healthy life
User :   i know it is but i just feel better on less than 4 hours sleep and if i do sleep for more than 8 hours i feel like crap that day.
Doctor :   For say 2 weeks yu feel crap later you feel it better sleeping for 7 hours is always good
User :   well, ill give it a go
Doctor :   Yes
Doctor :   I suggest you to get back to me after your consultation with the GP
Doctor :   his opinion
Doctor :   after physical examination
Doctor :   I would be glad to help you basing on his physical findings
Doctor :   though
Doctor :   Ok?
Doctor :   Is there any thing else that I could help you with?
User :   is there not loads of you doctors on this thing? how would i know it is you?
