User :   I have a problem

User :   What ever I have in the night, it is not getting digested and have sleepless nights and
User :   the acidity levels are increasing and especially around 3 AM it is even bad

User :   I also have a bad taste (like tamarind water) in mouth and the food pipe between my
User :   throat and stomach has burning sensation
User :   if i have some gelusil or digene it becomes ok

User :   I am not able to sleep on one side or up side down since i have a vomiting sensation
Doctor :   Ok
User :   I tried omeprozole (omez) for a month in empty stomach but still i am not able come out of this. please comment
Doctor :   Since when are you having these symptoms?
User :   last one month
Doctor :   Ok
User :   I also take Victoza (diabetes Injuction for the last 2 mnth) and I am also HT

Doctor :   Now let me guide you.
Doctor :   Digestion is affected by various factors.
Doctor :   Offcourse what you eat is one major factor.
Doctor :   Apart from this other factors such as time of your dinner, regularity of meals, presence of stress, body weight, other medical conditions
Doctor :   can significantly affect digestion and its symptoms.
Doctor :   Apart from taking the medications these factors also has to be addressed if you want to find a cure for your symptoms.
Doctor :   My suggestion to solve your problems are as follows
User :   I have 3 dosa at 8.30 AM, Rice at 2 PM, 4 Chapatis with dal at 9PM and in between I have 2 teas at 11 A M and 4PM
Doctor :   Your diet is fine, but let me explain.
Doctor :   My suggestions are
Doctor :   1. Continue antacid syrups for another 2 weeks, you may have to take them before a meal thrice a day.
Doctor :   2. You may also continue to take proton pump inhibitor drugs such as omez once a day for another 7 more days.
Doctor :   3. Your diet is fine. Avoid getting to bed immediately after a meal.
Doctor :   you will need to go to the bed atleast an hour or 2 following your dinner. Gettting to the bed will impair digestion and hence acidity.
Doctor :   4. While sleeping it is preferable to raise your head by using 2 pillows. Avoid sleeping on stomach.
Doctor :   Are you following?
User :   I feel comfortable sleeping on stomach or side ways.
Doctor :   Side ways is fine, if you lie on stomach, reflux symptoms can occur both due to pressure and gravity.
Doctor :   hence it is preferable that you sleep on back with 2 pillow.
User :   but even on side ways is also not possible since I feel the pinch
Doctor :   If you are getting the acidity and reflux symptoms while lying sideways, you may have to try sleeping straight on back with 2 pillows under the head.
Doctor :   There are more measures that you need to follow. They include
User :   elevated position with two pillows is not possible since I have spondylyte problem also
Doctor :   Ok, if there are neck issues, then next option is to raise the cot on which you are sleeping.
Doctor :   You could place a stable block under the legs of the cot near the head side.
User :   because of space constraints i sleep on floor
Doctor :   So that the cot is elevated to some extent at the head region.
Doctor :   Try as far as possible to sleep with the head elevated than the other body.
Doctor :   The other measures are
Doctor :   5. Reduce the impact of stress on your body. This can be achieved by destressing methods. They include
Doctor :   a. Rest as much as possible when ever you are free.
Doctor :   b. Deep breathing exercises.
Doctor :   c. Physical exercises
Doctor :   d. Yoga and transcendental meditation
Doctor :   e. Relaxation therapy such as music therapy, acupuncture and so on can be tried.
Doctor :   6. Concentrate on reducing weight by proper diet and exercises.
Doctor :   7. I hope you are not smoker. Smoking can affect your symptoms.
Doctor :   8. Your diet is good, but restrict coffee and tea. Instead milk and yoghurts can be taken.
Doctor :   9. Lastly if the symptoms are persisting inspite of other measures, you will need upper GI endoscopy.

Doctor :   The metallic taste and burning sensation that you have are part of this symptoms and will be better once your indigestion symptoms are better.
