User :   Since long time i am having severe lower back pain...
User :   just above the tale bone...

User :   i feel like i am having a swelling in that area..

User :   and when the pain get more, immediately the after i get fever, feel exhausted, my joints start paining..

User :   this is happening now for long time..i even consulted general physian for this..but they give some medicines for fever..and after two day my routine is back...
User :   again after a month or so my back pain starts

User :   once i had tested my blood and all..the Doc said about rheumatism...but in the next report there was nothing..
User :   for more than a year now..

User :   when i reached the doc..i got fever..and i think the doc didnt releazed the swelling...this is what i now noticed..

User :   i do not have any other mediacl illness, but yes bcoz of this back pain i often get on and off of fever..

Doctor :   Before explaining to you, let me confirm your complaints.
User :   also sometimes, when i get up from my bed in the morning and turn my neck downwards, i get severe pain in my spine..even that pain is sometimes non-torreable..
Doctor :   1. You get on and off severe pain located at just above the tail bone since 6- 8 months.
User :   this back pain is for more than a year now..
Doctor :   2. You find the swelling in the same area of the pain.

Doctor :   Pain in the spine region can occur due to multiple reasons.
User :   ok
Doctor :   Without a proper physical examination, I cannot specify the exact cause of the pain.
Doctor :   But based on your description I can think of the following possibilties
User :   ok
Doctor :   1. It is a muscular pain secondary to posture and sitting on a chair for long duration - such as while working on a computer

Doctor :   2. Spine deformities
Doctor :   3. Soft tissue pathology - such as an infection, pilonidal cyst and so on.

Doctor :   4. Chronic inflammation/degenerative changes of the spine bones.
Doctor :   These 4 are the possible causes.

Doctor :   A visible swelling with tenderness can indicate the 3rd possibilty, which needs appropriate treatment, in the form of antibiotics or minor procedure.

Doctor :   The rest can only be diagnosed based on blood tests and scans.
Doctor :   Hence my suggestions to you are
Doctor :   1. When in severe pain, avoid sitting on a chair for long duration, bed rest(lying on the side) is advisable

Doctor :   2. Use local heat therapy in the form of heat fomentation at the region of pain

Doctor :   3. Use oral antiinflammatory drugs such as Ibuprofen 400mg twice a day after food for 2- 3 days.
Doctor :   4. Watch out for the appearance of swelling, redness over the region of pain.

Doctor :   If you notice them, you need to report to a doctor.
Doctor :   A physical examination of the affected site will rule out possible soft tissue pathologies such as infection/pilonidal cysts
Doctor :   5. Involve in back strengthening exercises, early morning physical exercise

Doctor :   6. If symptoms are very frequent, then you may need blood tests and scans to look for spine pathologies
Doctor :   7. Lastly avoid severe physical activities such as lifting heavy weights.

Doctor :   By spine pathology, I mean a vertebral column (spine problem) such as disc prolapse, inflammation, osteoarthritis and so on.
Doctor :   The can be diagnosed based on physical examination and scans.
Doctor :   And with regards to the appointment, it is best to see the doctor when you have the symptoms of pain with or without swelling.
Doctor :   If you are in terrible pain now, I suggest that you use bed rest, and we shall help you in fixing an appointment with a doctor.
